Responsibilities with no on-call or weekend hours required. InnovAge PACE (Program of All-inclusive Care for the Elderly) is an alternative to nursing facilities. It provides customized healthcare, social engagement, and exercise to older adults to help them stay independent and in their homes. PACE participants receive most of their healthcare and social support at an InnovAge center in their community. Each person in the program has a team of healthcare experts and a personal care plan that is focused on providing a personalized experience.
Our Behavioral Healthcare Provider (BHP) is an integral part of developing and delivering a personal care plan ensuring high quality, clinically appropriate, team-based behavioral health care. Provides initial assessments, clinical intervention, referrals for participants and families, and treatment recommendations. Physical attendance at an InnovAge center is essential to performing the job duties with the ability to travel to meet participants outside of center as needed.
You will
* Confidently navigate ambiguous situations

* Be open to feedback and able to reflect productively
Clinical Care - 70%
* Completes mental health assessments and document in the EMR (diagnosis, recommendations, team-based care planning, and other regulatory requirements)
* Provides solution-focused (monitoring, therapeutic, or crisis) intervention to participant and/or caregiver(s)
* Consults with team members regarding clinical findings, provide interventions, and case conferencing
* Collaborates with IDT/external providers/caregivers to support participants at risk for inpatient psychiatric hospitalization (e.g., referral and coordination with outside resources to promote stabilization)
* Monitors response to psychotropic medications and coordinate with psychiatric consultant to ensure continuity of care
Administration - 30%
* Educates IDT to manage and resolve behavioral complexities or placements
* Liaisons with external psychiatric providers to ensure appropriate referrals, continuity of care, monitoring/follow up and report back to IDT
* Participates in IDT meetings, conferences, committees, and events as requested
REQUIRED
* Minimum of Master's degree in behavioral health, social work, or psychology
* 1+ year working in an integrated care setting
* Licensed Mental Health Professional
* Clinical Social Worker (LCSW) or
* Licensed Psychologist (PsyD or PhD) or
* Licensed Marriage Family Therapist (LMFT) or
* Licensed Professional Counselor (LPC)
* Current CPR certification or the ability to obtain CPR certification in first 30 days of employment
PREFERRED
* Knowledge working with geropsychiatric populations
* Training or experience with group and family interventions
* Experience with conflict management, mediation, de-escalation, and crisis mitigation
* Knowledge of the chronic disease management and its psycho-social implications
* Bi-lingual
* A doctoral degree in clinical psychology or integrated primary care experience
* Licensure or certification in substance use treatment

Education and Experience
Master's degree or higher in a behavioral health discipline.
A Colorado unrestricted license in a behavioral health profession and/or addiction counseling licensure. Licensures include LAC, LCSW, LMFT, or LPC.
A minimum of three years clinical experience in a behavioral health setting is required.
Experience working with children, adolescents, parents, and families is desired.

Functions as part of the hemodialysis health care team in providing safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse in accordance with FMCNA policies procedures and training and in compliance with regulations set forth by the corporation state and federal agencies. Responsible for the setup and operation of hemodialysis machines. Assist in the maintenance of a safe and clean working environment. CUSTOMER SERVICE:

* Evaluate vascular access for patency and report any unusual findings to nurse supervisor.
* Obtain necessary pre and post treatment vital signs and weight. Report unusual findings to nurse supervisor.
* Perform vascular access evaluation pre-treatment and report unusual findings to nurse supervisor.
* Perform vascular access cannulation and report any difficulties in cannulation or unusual findings to nurse supervisor.
* Perform administration of Heparin as delegated or as allowed by state law.
* Initiate dialysis treatment according to prescribed orders including blood flow (QB) and dialysate flow (QD).
* Evaluate intradialytic problems and provide intervention as prescribed by physician order or as directed.
* Monitor patients' response to dialysis therapy and report any unusual findings to nurse supervisor.
* Obtain vital signs prior to reinfusion; perform all relevant functions necessary for the discontinuation of treatment - document.
* Discontinue dialysis treatment according to established procedures
* Evaluate patient prior to termination of venous access - standing & sitting blood pressure.
* Obtain Hemostasis and apply appropriate dressings.
* Evaluate the patient for prior to discharge and report any unusual findings to nurse supervisor.
* Perform and record Pre and Post dialysis evaluation weight and vital signs with initial identification.
* Responsible for calculating and entry of individual patients' dialysis machine programming for Ultrafiltration (UF) goal; treatment time; Sodium (Na) modeling; and UF modeling as prescribed.
* Responsible for ensuring appropriate safety alarms are enabled including Narrow Venous Limits (NVL) 160.
* Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification.
* Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and if applicable progress notes.
* Report any significant information and/or change in patient condition directly to the nurse supervisor.
* Observe patient and conduct machine safety checks according to facility policy. Report any change or unusual findings to the nurse supervisor.
* Perform and document any intervention for unusual patient status and document patients' response to intervention.
* Evaluate patients' vascular access during treatment including arterial and venous monitoring pressures provide appropriate intervention as needed document and report any unusual findings to the nurse supervisor.
* Obtain all prescribed laboratory testing and prepare specimens for collection.
MAINTENANCE/TECHNICAL:
* Maintain environmental integrity and aesthetics - ensure all areas are safe and clean.
* Clean and disinfect dialysis machine surface chair equipment and surrounding area between treatments according to facility policy.
* Conduct all tasks necessary for preparation for dialysis treatment and document where appropriate.
* Perform all required pretreatment dialysis machine alarm testing including Pressure Holding (PHT).
* Initiate Solution Delivery System (SDS) system.
* Ensure that all blood spills are immediately cleaned with appropriate disinfectant according to facility policy.
* Prepare organize and efficiently utilize supplies and equipment to prevent wastage.
CLERICAL & ADMINISTRATIVE:
* Complete and document ongoing participation in Continuous Quality Improvement (CQI) activities.
* Enters all treatment data into the designated clinical application accurately and in a timely manner.
* Review treatment sheets for completeness ensure nursing signatures are documented and ensure omitted entries are completed or corrected where appropriate.

Summary:
+ Cross trained position to be assigned either in a Unit Secretary or Nursing Assistant role. The Unit Secretary is responsible for general office management, clerical and receptionist duties on the Nursing Unit. The Unit Secretary facilitates order entry and processing coordinates administrative support activities. The Nursing Assistant position provides patientcare under the direction and supervision of a Registered Nurse. Both the Unit Secretary and Nursing Assistant performs duties that support the operation of the nursing unit and adheres to Nursing Policies and Procedures and Organizational Goals and Objectives.
Responsibilities:
+ Communications and Customer Service Welcomes and greets patients and visitors to the unit. Demonstrates pleasant, professional communication skills when speaking with patients, visitors, nursing staff, physicians, volunteers and personnel from other departments. Answers the phone promptly using proper telephone etiquette. Identifies self and unit. Refers phone calls to appropriate person and is reliable with messages. Assists visitors find patient rooms, staff members and other resources. Facilitates transport of patients at discharge. Facilitates the patient belongings process as needed, including that the handoff of belongings occurs at admission, transfer and discharge. Facilitates calls to physician on behalf of nurses, avoiding duplication. Demonstrates correct use of communication systems such as intercom, pager system, phone functions, pneumatic tube, and other department based systems. Uses unit specific communication tools and applies to practice. Checks email regularly and posts items at the request of nursing leadership. Seeks clarification from Nurse Manager or Director as needed. Performs follow up phone calls to patients if requested by nursing leadership. Performs customer service rounding and surveys for patients upon request of nursing leadership. Answers call bells in a timely manner and communicates needs to responsible staff, or handles themselves if a simple request. Follows up to ensure that call bells have been addressed.Electronic Order Management and Order Processing Provides assistance to physicians, nurses, and other healthcare professionals as needed in accurate and efficient entry of computer orders (CPOE). Promotes and encourages CPOE. Demonstrates knowledge of order sets that the practitioners may use with CPOE. Actively uses Soarian feedback to report to CMIO and physician analysts any physicians who need monitoring or follow up on their CPOE practices. Checks hourly for Order Session Summary reports while still in existence. Reprints orders from Reprint Facility or Operational Reporting as needed. Ensures that the Transfer Level of Care Med Rec Report is on the chart of all preoperative patients. Flags the physical chart when any new orders have been written or electronically entered. Follows unit protocol for dispatch of written medication orders to Pharmacy. Communicates stat orders to the nurse immediately upon receipt. Enters into Soarian any written non-medication patientcare orders. Ensures that labeled specimens are sent to laboratory promptly, when requested. Notifies RN if there are any unclear or incorrect orders. Recopies Kardex or other forms when needed. Reviews Kardex or Daily Clipboard for Daily Lab Orders and enters these for the next day. With RN input, addresses and removes all remaining new orders from the work list before shift change and discharge. Performs Order Entry, Cancel Order, Order Inquiry functions when needed. Reviews orders regularly. Facilitates the patient schedule. Coordinates when there are multiple tests ordered and keeps track of patient location. Monitors specialty bed ordering and discontinuation when patient no longer needs. Initiates the location order set for inpatients who are going to surgery.Chart Management Places correct labels on all paper chart forms (front and back). Ensures all chart forms are properly labeled. Adds blank forms to charts as needed. Keeps charts neat and pages secure (reinforced). Maintains charts in accordance with CCH Policy & Procedure for sequence. Neatly files any remaining paper chart forms into their appropriate location. Thins charts as needed per policy & procedure, Prepares charts for admission, transfer and discharge. Prints ExitCare documents upon request and puts chart copy in chart prepares education packets at request of nurses to use in their patient teaching. Facilitates record requests from or to CCH or other facilities as needed. Assists with metrics and reporting requirements that may arise, for example recording quality improvement information onto logs or printing workflow reports.Patient Flow Responsibilities Prints ExitCare documents upon request and puts chart copy in chart prepares education packets at request of nurses to use in their patient teaching. Prints or copies records for patient transfer to another institution when case management is not available. Uses the Soarian Financial Application to enter Admissions, Transfers, Discharges, Service Updates, status updates (outpatient to observation status for example), Patient Location management, and other ADT functions as needed and in a timely manner. Monitors isolation census in consultation with RN- removing patient from isolation and discontinuing isolation equipment if screening has returned negative. Updates isolation code in Soarian if requested. Maintains census logs. Prints census sheet for the next shift as requested. Ensures that required Medicare paperwork has been signed and given to the patient and the chart copy placed in the chart when Case Management is not available. In ACC, assists in preparing stretchers and picking up blood products from the blood bank.Nursing Assistant - PatientCare Responsibilities Provides age appropriate care for the patient population served. For example, gives consideration to growth & developmental phase of life, to age related factors such as thermal regulation, nutrition, safety and functional needs of the patient. Uses appropriate resources for patients with special needs such as language barriers, sensory problems, altered cognition, cultural and religious needs. Integrates principles of infection control & infection prevention into practice. Uses standard precautions and uses personal protective equipment. Patient care assistant job at InnovAge
Responsibilities The Day Center CNA (Certified Nurse Assistant) works at our Adult Day Health Center assisting participants with ADLs (activities of daily living) and providing both restorative and supportive care. * Follow the care plan and assist participants with daily activities like bathing, dressing, grooming, hair care, mouth care, eating, and toileting.
* Document care provided in accordance with established policies, guidelines, and applicable regulations.
* Carry out, reports and documents care given in an effective, timely manner as observed by the RN and through periodic record reviews.
* Assist participants with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, fills oxygen and provides other aides as assigned or needed.
* Monitor participants health status such as vital signs, weights, skin condition, appetite and changes in functional ability and report pertinent information to clinical supervisor in a timely manner.
* Collaborate with multidisciplinary teams in care planning for participants.
* Monitor appetite and look for changes in functional ability and skin condition.
* Monitor/assist participants in a courteous manner with meals and helps with feeding, as necessary.
* Report any variance, accident, or unusual occurrence to the appropriate personnel.
* Assist with loading and unloading of participants from vehicles.
* Assist the transportation department with participants' take-home supplies.
* Serve as a bus attendant on company vehicles while InnovAge drivers transport participants to/from center or medical appointments supporting participants while in transit and accompanying participants in and out of facilities.
* May rotate into In-Home Services as a Homecare CNA. as deemed necessary by the Center Manager.
* Creates and participates in group(s) and one-on-one activities in accordance with daily activity calendar when needed.
* Attend in-service and continuing education programs as scheduled and necessary.
* Other duties as assigned.
REQUIRED
* Active and valid state-issued Certified Nursing Assistant (CNA) license
* Current CPR certification or the ability to obtain CPR certification. Acceptable vendors for certifications are from either American Heart Association and/or American Red Cross.
* High School Diploma or GED equivalent
PREFERRED
* Experience in a care setting for frail and elderly patients participating in a group environment
* Bi-lingual
