The Care Guide Plus - Community Based role is responsible for participating as a member of the inter-disciplinary Care Coordination Team to coordinate care for members, meeting their individual needs and the needs of the population. The Care Guide serves as a single point of contact for care coordination when there is no CCE or OhioRISE Plan and/or CME involvement and short-term care coordination needs are identified. The Care Guide Plus serves as a single point of contact for care coordination when there is a CCE, OhioRISE Plan, and/or CME involvement and short-term care coordination needs are identified. Members needing Care Guide or Care Guide Plus assistance for longer than 60 calendar days should be considered for referral to a Care Manager.
Essential Functions:
Engage with the member in a variety of settings to establish an effective, professional relationship. Settings for engagement include but are not limited to; hospital, provider office, community agency, member's home, telephonic or electronic communication
Participate in the identification of the individual's needs and prioritizes efforts in collaboration with the member and caregivers.
Gather information to identify and manage barriers to care
Take appropriate steps to close gaps in care where appropriate
Under the supervision of the Care Manager, implement effective interventions based on clinical standards and best practices
Maximize the client's health, wellness, safety, adaptation, and self-care through effective care coordination
Educate the member and other stakeholders about treatment options, community resources, insurance benefits, etc. so that timely and informed decisions can be made
Gather information to assist the Care Manager to evaluate the member's response to the plan of care as requested
Evaluate client satisfaction through open communication and monitoring of concerns or issues; assist members in filing of Grievances & Appeals as appropriate
Collaborate with Care Managers and providers to plan for post-discharge care needs or facilitate transition to an appropriate level of care in a timely and cost-effective manner
Document care coordination activities and member response in a timely manner according to standards of practice and CareSource policies regarding professional documentation
Starts each interaction with members wondering, "What does the world look like for this person, and how can I meet him or her where they are? What are his or her unique needs, and how can CareSource help?" In each interaction, the employee will aspire to help the member to feel informed, empowered, and supported by CareSource
Looks for ways to improve the process to make the members experience with CareSource easier and shares with leadership to make it a standard, repeatable process
Document all transitions of care, including sentinel events, in the MCO and OhioRISE Care Coordination Portals (CCP)
Once documented, will send electronic notifications of sentinel events to the member's authorized users in the MCO or OhioRISE Care Coordination portals
Serve as Central Point of Contact for OhioRISE, CCE, and CME entities
The Care Guide Plus will collaborate with OhioRISE, CCE, and CME entities to obtain and share any Release of Information (ROI) information and documentation in order to maintain HIPPA and other privacy requirements
Trained to administer the CANS assessment
Regular travel to conduct member visits, provider visits and community based visits as needed to ensure effective administration of the program
Perform any other job duties as requested
Education and Experience:
Associate's Degree or equivalent years of relevant work experience is required
Minimum of one (1) year of clinical experience in nursing, social services, or healthcare field (discharge planning, case management, care coordination, and/or home/community health experience) is required
Medicaid and/or Medicare managed care experience is preferred
Competencies, Knowledge and Skills:
Proficient with Microsoft Office, including Outlook, Word and Excel
Sensitivity to and experience working within different cultures

In addition to base compensation, you may qualify for a bonus tied to company and individual performance. The Bureau of Land Management-Upper Snake Field Office (BLM-USFO) is recruiting up to 4 members to assist with natural resource conservation work on public lands in Eastern Idaho for up to 16 weeks beginning between May 4 to June 1, 2026, with a minimum commitment through July 31, ideal end date mid- August. Individual Placements will be integral to Field Office natural resource conservation and restoration activities. Individual Placements will assist with on the ground conservation and restoration activities, noxious weed control, vegetation monitoring, and other Field Office activities.
Duties and Responsibilities:
Specific activities include:
Noxious Weed Control and Inventory- Control of noxious weeds through mechanical and chemical methods to protect and improve public lands and restoration project area. Noxious weed control will include use of backpack sprayers, UTV's, and other control equipment. Inventory of noxious weeds will follow standard protocols (60%).
Implementation of Conservation Actions- Assist with the implementation of Conservation Actions to improve and restore public lands for wildlife and public use. Work may include, but not limited to, Juniper Removal; Seedling Plantings; Fence Removal, Maintenance, and Construction; and Construction and Maintenance of Beaver Dam Analogs and other Low-Tech Process Based Restoration Efforts (20%).
Vegetation Monitoring. Assist in vegetation monitoring efforts to document pre and post Conservation Action implementation to determine action effectiveness and assist with adaptive management. Monitoring efforts will include line-point intercept, fixed plot diversity measurements, and other standard methods (10%).
Assist in Field Office Activities- Assist other Field Office programs such as fuels, fire rehabilitation, fisheries, rangelands, recreation, weeds, and wildlife with ongoing programs and projects (5%).

Work will focus on projects in the South Fork and the Henry's Fork of the Snake River with additional work occurring throughout the Field Office. Summer in the Upper Snake Field Office provides for extensive recreation opportunities including, fishing world famous rivers, and hiking and camping in nearby mountains, forests and National Parks. The office is located in Idaho Falls, ID a community of over 70,000. Idaho Falls has all the necessary amenities including a well-served airport. Short term rentals are available in the Idaho Falls area and in nearby Rexburg.
Terms of Employment:
Individual Placement will complete the 16-week program starting May/ June through July/ August 2026. The Individual Placement will average 40 hours of labor per week, Monday through Thursday, 7am to 5:30pm. Over 80% of work will be performed in the field with the remainder in an office setting. Individual Placements are responsible for providing their own housing, personal transportation, and food. Housing funding may be available.
Program Benefits:
Individual Placements will earn $10,240 living allowance dispersed on a monthly basis throughout the 16-week program (640 hours). The living allowance is taxable. Upon successful completion of the term, Individual Placement could be eligible to receive a hiring authority within the federal service.

Responsibilities
Instructing and leading groups of up to 9 guests through our 3-hour Zip line tours. This position requires applicants to be very safety conscious and comfortable working in the outdoors (All weather) and at heights. Applicants are preferred to have strong presentation skills and be knowledgeable or willing and able to learn the local natural history, environment, and ecology of the region. They also must be prepared to accept responsibility in real risk scenarios while calmly managing participants at heights.
Qualifications
At least 18 years of age.
Valid drivers license.
Able to work at heights, including traversing along cables over 70 feet above ground.
Able to lift and carry up to 25 lbs.
Minimum height of 5'4'' and maximum weight of 250 lbs.

The Parks Division is responsible for the functions necessary to achieve the state park's program goal of managing Montana's scenic, natural, historic, archaeologic, scientific, and recreational resources to meet present and future needs. This work unit is responsible for the support of State Park operations.
Specific Job Information:
These seasonal variable positions run from 3-5 months and are responsible for leading groups of up to 30 visitors on the two-mile, two-hour cave tour and one mile, 1 ½ hour cave tour. While conducting tours the guide conveys to visitors the natural, historic, and cultural features of the cave and park. Guides are also responsible for the safety and comfort of visitors. Guides provide information and direction to visitors on a regular basis and perform routine maintenance tasks when necessary.
Interpretive and Guest Services
Conducts up to three guidedtours of the cave daily with up to 30 visitors per tour. Interprets geological and historical aspects of the cave in an entertaining and informative manner. Monitors tour groups to ensure visitor safety and that cave resources are protected. Uses effective communications skills and practices quality customer service principles regularly.
Conduct tour in a specific time frame covering historical and geological information on the cave. Follow Park procedures in handling any incidents in the cave in a professional manner. Ensure that visitors are aware of the nature of the cave tour and are aware of proper dress and cave safety. Notify supervisors of any known problems or incidents in the park. Must conduct the tour in a manner to meet the expectations of a wide variety of visitors on a cave tour.
Staffs the visitor center information desk; answers personal inquiries from the public and explains and interprets rules and regulations for the public regarding the park and Department programs. Disseminates information on fishing access sites, wildlife management areas, historical sites, and other state parks or tourist attractions in the area. When working in this position the incumbent must gain a working knowledge of Parks, Department policies, and a good understanding of the area's tourist attractions.
Must organize tasks efficiently, establish positive working relationships with co-workers and management, work well with the public, and be able to communicate effectively. Must be able to effectively deal with angry or verbally abusive individuals who are dissatisfied with FWP policies and actions. And needs to show the ability to use tact, courtesy, poise, alertness, and good judgment in public contacts as well as emergency situations.
Must be able to follow written and verbal instructions. Must be willing and able to obtain a working knowledge of the park and the region, including site location and available recreational opportunities for public recreational sites within 2 hours of the site. These include wildlife management areas, fishing access sites, state parks, forest lands, and major waterways.
May provide support for staffing the two park visitor centers and/or cave tour ticket booth, answer inquiries from park guests, do light janitorial duties, and potentially other duties given extraneous circumstances.
