The Care Transistion Coordinator is responsible for developing strategic relationships in key hospital accounts, facilitating the transition of prospective patients to ProMedica Senior Care centers, and serving as a liaison between key hospital accounts and ProMedica Senior Care centers.
Responsibilities:
Maintains a thorough knowledge of the centers' products and services, acuity capabilities and physician relations.
Invests a minimum of 75% of work time on referral management, interacting with hospital discharge planning staff daily, providing updates on pending referrals and gathering new referrals.
Completes patient assessments as needed to make sound admission decisions.
Uses IMPACT training to gain admission commitments from prospective referrals.
Invests 25% of work time on face-to-face referral development completing integration calls to physicians, hospital and medical professionals and other related contacts.
Provides and/or coordinates educational and informational presentations on ProMedica Senior Care programs and services for hospital departments and personnel.

We are in search of a qualified Care Transition Coordinator to join our team of dedicated professionals.
This position maintains resident census based on facility goals, develop, and implement a marketing program, act as a liaison between the facility and community, keeps abreast of any changes to the Census and coordinate efforts of the Admission Committee and the facility. This position must also coordinate the admission process and assist the family with administrative questions as needed.
Additional Responsibilities include:
+ Take interested families on tours of the facility
+ Complete admission paperwork
+ Verify insurance
+ Coordinate with hospital case managers
+ Utilize nursing skills to assist in the assessments to align admissions that support the goals of the facility.
+ Conduct market research and make marketing suggestions to the committee as a routine part of the job. He/she will coordinate marketing efforts of Admission Committee and the facility staff.
