Primary responsibilities will be:
• Develop and implement strategic plans to generate new referrals and grow hospice admissions in the Houston area.

Educate patients and their families about health conditions, treatment plans, and self-care techniques.
Adhere to all agency policies, procedures, and regulatory requirements related to home health care.
Qualifications:
Valid and current LVN or LPN license in the state of Texas or compact state.
Minimum of one (1) year of experience in a home health setting.
Knowledge of home health regulations and general healthcare compliance.
Experience with HCHB EMR. (preferred but not required)
Excellent communication and people skills to effectively interact with patients, families, and healthcare team members.
Compassionate and empathetic demeanor with a commitment to providing patient-centered care.
