Plexus transition manager job description

Purpose Statement: The Transition Manger Sr role is to lead the Plexus Transition System in the development and execution of Transition Programs and Projects across Plexus Regions and Plexus Market Sectors globally. Transition Management leads the Transition Program of Work & associated Projects by working with internal & external stakeholders including Plexus Program Management & CFT (Customer Focus Team) in addition to other SME's (Subject Matter Experts) inside & outside Plexus.
Key Job Accountabilities:
Align with business sectors and engage with Sr Sector Management to understand customer requirements, Plexus business needs, CRM funnel to successfully execute Transition Projects.Collaborate and communicate with stakeholders to support business development solutions and new programs. Also works to manage customer stakeholders' expectations.Apply project management tools, processes, and standard operating procedures to ensure flawless execution of Transition Projects.Ensure common processes are implemented across all sites supporting a customer account in order to create standard work to have a more consistent positive engagement with the customer by deploying best practices, using common metrics, and creating common communication formats Responsible for analyzing quoted cost and to actual cost at conclusion of Transitions.Support CD/CM with recovery of Transition & Start Up costs, also will deploy a process to understand quoted costs, actual costs and transform costs and take action to minimize negative variance.

Additional Accountabilities:
Works within the Plexus organization to design transition project teams Lead wider teams when managing international programs. Train, coach, and mentor Plexus wide staff in Transition tools and processes. Practice a continuous improvement mindset by facilitating and supporting senior level Transition Project reviews and conclusion case studies by working to document lessons learned and make improvements.Ensure a Program Management Governance model is followed to report Program Status using STD Work aligned with Transition department.Flexible working is required which requires evening or early morning conference calls.Travel is required typically up to 25%.Additional activities as required by Plexus in line with Transition skills.Provide timely status reports and project updates to Global Operations Management teams through application of the Transition Management Governance Process.Support quotation preparation, capture Transformation Costs & support ‘Actual to Quote' gap closure.

Education/Experience Qualifications:
Bachelor's degree or equivalent (preferred) PMI Membership with PMP Certification preferred (or equivalent project management training) CLSSGB - Certified Lean Six Sigma Green Belt (preferred) Plexus Problem Solving Training (preferred) Training will be provided as required10 years Industry experience in an EMS or Product company: business, operations, project management, technical or supply chain role is required.See also KFLA & BARS section

Manages the provision of care transitions within an integrated health care system

Collaborate with integrated healthcare system partners to ensure that their clients transitional care needs are met

Develop and manage benchmarks and performance indicators to evaluate and improve care transition for all entities involved within the integrated health care system. Establish tracking mechanism and report out process

Coordinates staffing needs and assesses care transition performance in all settings - acute care, skilled nursing facilities, assisted living, and physician practices

With Business Development Organization, collaborate and assess the market for services outside of the integrated healthcare system. Market accordingly

Represent VNAHC in internal and external committees promoting care transitions management in a coordinated care model and establishing VNAHC's role in it externally

Manage, coach, mentor staff

Will actively participate in the performance improvement process known as H3W

Qualifications

  • A diploma in nursing, Bachelor's Degree preferred
  • 5 years progressive experience working in health care preferably with physician practices, hospitals, and home health
