At FutureCare, our Regional Care Managers (Hospital Liaisons) are an integral part of the admission process, serving as the liaison between the hospitals and our facilities. The Regional Care Manager is responsible for completing the pre-admission screening of patients for appropriateness to FutureCare by reviewing the clinical record, and talking with discharge planners to determine if there is a skilled need. The care manager helps to educate the hospitals to the programs and services we can provide at our various locations. The Regional Care Manager works on-site at the hospital, reviewing referrals, communicating with the discharge planners and other key clinical personnel, the patient and family, and our facility admission departments. The Regional Care Manager is also responsible for building new business from within their hospitals through ongoing business development.

Referral management and clinical review of patients for potential admission

Qualifications

Ability to maintain confidentiality of resident information in compliance with the prevailing legal requisites
Experience with Microsoft Office and Excel software required

OUR MEDICAID PATIENT ACCOUNTING SPECIALIST POSITION
The primary focus for our Medicaid Patient Accounting Specialist is collecting the Resident Resources Accounts Receivable with accuracy and within the required time frames. This role will address all financial questions and concerns from residents and their families related to the calculation of the Resident Resources as well as the obligation of the organization to collect the Resident Resources as determined by the State of Maryland. MAIN RESPONSIBILITIES
This position requires a thorough understanding of the Medicaid program as it relates to skilled nursing facilities and the ability to functionally apply the program requirements to related areas of responsibility. Must be able to explain these complex requirements to family members and residents.
Must possess excellent communication skills. This position demands continuous professional interaction with family members, state agencies, attorneys, financial institutions, as well as peers, supervisors and other departments.

Monitor all Medicaid approved Resident Resource accounts to include the analysis, timely collection, and disposition of the account. Ensure the accuracy and timeliness of reported balances on the A/R aging report and resident Statements of Account.
Ability to analyze complex accounts. This includes a comprehensive understanding of the flow of the RFMS accounts and the calculation of the Medicaid and Resident Resource payments.
Notify MA approved resident/family members of their obligation to pay the monthly resources to cover the cost of care. Ensure payment is received timely each month.
Provide back-up support for the annual Medicaid Recertifications completed each month to ensure all are completed timely.
Assist with the Medicaid application process as requested by the Medicaid Billing Supervisor. This role provides back-up support to the Medicaid Customer Support Specialist as needed to ensure all Medicaid approval requirements are met timely and effectively for all MA pending residents.
Contact insurance companies for cash values and any other policies or plans related to the financial status of the resident.
Must be able to properly interpret an NOE, determine any required changes and update the system accordingly. In addition to resolving the account, must be able to interact with the RFMS team to ensure they are updated with changes, as both systems must accurately reflect the correct information as provided by the Medicaid program.
Assist in PEME applications as requested by the Medicaid Billing Supervisor.
Assist in the calculation and appropriate recordation of the UCC charges into the correct payer account as requested, and as instructed by the Medicaid Billing Supervisor.
Attend scheduled A/R review meetings with the Accounts Receivable Supervisor, Medicaid Billing Supervisor, Director of Resident Accounting and CFO.
Maintain accuracy of PointClickCare (PCC) data base.
Ensure that all approved days and charges have been properly accounted for in PCC. Review resident accounts for accurate charges and payments.
EDUCATION & EXPERIENCE
High school diploma.
Two (2) years of experience in a health care business office under the Medicaid payer, preferably in a long term care setting.

This is a FULL TIME, NON-EXEMPT position on the DAY shift, Sunday through Thursday, with hours from 9:30AM to 6:00PM.
MAIN RESPONSIBILITIES
Accommodate occasional changes in schedule to meet the needs of evening programs and weekend availability as needed.
Assist in planning, developing, organizing, implementing, and evaluating the activity programs of this facility.
Transport residents to and from unit programs and special activities off the unit.
Observe resident attendance, mood, behavior, and degree of involvement so that facility activities and resident progress are evaluated.
Provide documentation on each resident's participation in activities, small groups and one-to-ones.
Communicate with resident/family about activity programs.
Develop and maintain a good working rapport with other departments within the facility to assure that daily activities can be performed without interruption.
Coordinate activities with and refer resident / family to other departments when necessary.
Provide routine contact with EVS, Dietician, Nursing Personnel, and other team members to exchange information regarding residents' progress.
Serve on, participate in and attend various committees of the facility as appointed.
Attend scheduled staff meetings to assist in identifying and correcting problem areas and improvement of services.
Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the facility.
Attend and participate in workshops and seminars to keep abreast of current changes in the long-term-care field. Maintain a professional status.
Attend and participate in monthly & annual in-service training programs.
Report all unsafe conditions and equipment immediately.
Maintain virtual communication software systems like Cubigo and PCC
EDUCATION & EXPERIENCE
