PROGRAM/LOCATION: Behavioral Health Bridge Housing Program (BHBH) - San Luis Obispo
Behavioral Health Bridge Housing (BHBH) is a California-based initiative that provides temporary housing and supportive services to people experiencing homelessness who have serious behavioral health conditions. BHBH Case Managers are expected to supervise the general health, safety, and social rehabilitation of clients. They also perform moderate facility maintenance, function as positive role models, promote cooperation and harmony in communal living, and uphold and promote the program philosophy in all duties performed with a trauma informed approach.
HOURS/ SALARY:
Full-time, non-exempt position. 40 hours per week. Anticipated hiring range is $25.50-$26.50. Includes a generous individual benefits package that includes health, dental, vision, life, 403b retirement (with up to a 5% employer match) paid vacation/holiday time, paid sick time and a variety of helpful employee perks as well. Responsibilities and Duties:
1. Resident Services: Includes participating in intake interviews, facilitating house meetings, meeting with residents weekly, and on an as needed basis for case management, advocacy, and ILS counseling purposes, providing information and referral services for community resources.
2. Provide crisis intervention, and communicate the emotional, physical, and mental state and functioning of clients to the Program Manager and other staff members by attending scheduled staff meetings and by completing daily, weekly and monthly charting.
3. Site management: Includes inspecting sites, assisting with cleaning and/or repairs, arranging for professional repairs or services, participating in the purchase or donation of furniture or household supplies, communicating with the Program Manager regarding physical repairs, enhancements, and/or cleaning that is needed. Assist residents with move in/out activities.
4. Liaison: Communicate with other service providers regarding current clients, communicate with the Housing Authority, when appropriate, regarding client intake, rents, and Section 8 status, attend direct service provider meetings, and serve as a representative of the program to the community.
5. Extended Services: Provide case management and support services to clients who have moved on to independent housing situations. When applicable, serve as a Section 8 resource and aid the client throughout the process, work closely with the Housing Navigator to assist the client in all aspects of attaining independent housing, and communicate the progress of extended service clients to the Program Manager and other staff members through staff meetings and charting.
6. Complete program paperwork. This may include, but is not limited to individual client files, evaluating and charting client progress, service plans, admission/ discharge paperwork, HMIS data entry, Excel, Word, One Note, collect and record client rents, enter data into electronic record keeping system for MediCal billing, and security deposit receipts, and other required forms. Ability to meet program deadlines.
7. Medi-Cal billing provides a critical portion of funding for this program. You are expected to reach documentation and billing goals as set by Team Leader and Director.
8. All direct service staff are expected to spend at least 50% of their work time in direct service and support of clients, unless otherwise assigned by your supervisor.
9. Promote, encourage and maintain trauma-informed practices in all services and activities. Practice and promote, both individually and in team meetings, TMHA's Collaborative Communication Guidelines in order to facilitate clear, empathetic and person-centered communication.
10. Attend weekly staff meetings.
11. Assume other duties and responsibilities as assigned by Program Manager and Program Director.

1. BA/BS AND 6 months of full-time equivalent experience with target population, OR AA/AS or CADAC/CPRP certification AND 2 years of full-time equivalent experience with target population, OR 4 years of full-time equivalent experience with target population.
2. Some work on weekday evenings and/or weekends may be required, due to the nature of the SSI/SSDI application process.

Data Entry & Documentation Management
1. Enter and maintain ECM and Community Supports service data into the EHR, billing platform, or other required databases.

17. Follow established policies, procedures, and workflows related to ECM and Community Supports documentation.
18. Identify inconsistencies or errors in documentation and escalate to the manager for correction.
19. Participate in continuous quality improvement efforts.

1. Direct the day-to-day operations of the program staff including scheduling staff work hours; lead the regular organizational staff meetings; evaluate the status of clients, conduct appropriate planning and coordination of support services to ensure immediate attention to their changing needs.

5. Promote, encourage and maintain trauma-informed practices in all services and activities. 6. 7. 15. Employees in this position are required to participate in a rotating on-call schedule as part of their regular job duties. Monitor program budget, implement agency policies, and ensure accurate/ complete program paperwork; this may include rental agreements, resident files, client notes, Homeless Management Information System (HMIS) documentation, client rents, security deposits, time sheets, petty cash, grocery scrip, inventory, vehicle maintenance and operation logs, quarterly and annual reports.
21. Medi-Cal billing provides a critical portion of funding for this program. You are expected to support, guide, and hold accountable program staff to spend at least 50% of their work time in direct service and support of clients AND to reach billing minute goals as set forth by TMHA's Administrative and Management Leadership. Goals are set by an analysis of what is required for program to balance quality client care and maintain fiscal stability.
22. Assist the Division Director in enhancing and expanding the program and its services; this may include developing and implementing innovative services, approaches to service, funding for service, and/or improved management systems.
23. Select, train, supervise, and develop program staff, interns, and volunteers; this may include interviews, evaluations, consultations, terminations, hiring and scheduling staff.

The Full Service Partnership (FSP) Program provides intensive, community-based support to adults living with serious mental illness who are at risk of acute hospitalization or long-term institutional care. The primary objective of the program is to help clients achieve and maintain stability in the community through structured, individualized support. FSP serves individuals facing complex challenges, including co-occurring substance use disorders, significant trauma histories, and substantial functional impairments. Services are provided in the community-where clients live and feel most comfortable-rather than in clinical or hospital settings. This approach emphasizes hope, resiliency, and recovery, and is grounded in principles of cultural humility, professional integrity, and a commitment to ongoing self-reflection and personal growth for both clients and staff. The FSP Mental Health Therapist provides a comprehensive range of trauma-informed clinical services. These include diagnostic assessments, individualized treatment planning, therapy, care coordination, psychosocial rehabilitation, co-occurring disorder treatment, medication support, social and daily living skills development, supported housing and employment services, and linkage to educational and 24/7 crisis support resources. FSP operates with a “whatever it takes” philosophy. Therapists will be based at the San Luis Obispo TMHA office but are expected to meet with clients throughout the county in diverse, client-centered settings such as homes, parks, libraries, coffee shops, and community agencies. 4. All direct service staff are expected to spend 50% of their work time in direct service and support of clients, unless otherwise assigned by your supervisor.
5. Work with and train team members, clients and family members on mental health services and/or drug or alcohol abuse prevention strategies as part of developing a holistic wellness and recovery plan for the clients.
6. Participate in daily case review and coordination meetings with teammates. Assess and interview potential clients to determine if requirements for admission into FSP Program services are met, or if referral elsewhere is most appropriate. Collaborate with County Behavioral Health on assessments and referrals for admission into FSP services and stepping down of services back to the County BH Clinic.
7. Provide therapy, education and rehabilitation services for individuals, couples, or members of a family. Assist clients in achieving productive social adjustments reducing the impact of disabling disorders, and facilitate the achievement of family, vocational, recreational, legal and interpersonal goals.
8. Document clinical activity in a timely manner, meeting program outcomes, in an electronic medical records system, presenting diagnostic and interview findings, clinical evaluations, and recommendations as a member of an interdisciplinary team; function as the SAI (Single Accountable Individual) for client caseload. Must document and provide at least one contact with each client per week.
9. Provide crisis intervention in the field and in the office for individuals experiencing acute episodes; evaluate clients and arrange for voluntary or involuntary admission to facilities for observation and treatment. Become trained and designated to conducting evaluations and writing of applications for 5150 holds (training provided).
10. Confer with and advise clients' families to promote their understanding of and cooperation in treatment and rehabilitation programs.
11. Employees in this position are required to participate in a rotating on-call schedule as part of their regular job duties. This includes providing client support services during weekday after-hours and all weekend hours, in accordance with our program's contractual obligations. Flexibility and reliability are essential. Behavioral Health Bridge Housing (BHBH) is a California-based initiative that provides temporary housing and supportive services to people experiencing homelessness who have serious behavioral health conditions. Under the general direction of the Program Manager, the BHBH Housing Navigator assists and advocates on behalf of TMHA BHBH residents in the BHBH Rental Assistance Program and in TMHA's Adult Transition Program II (ATP) programs to locate, secure, and retain affordable permanent housing using a trauma informed approach. The Navigator plans and coordinates services, identifies appropriate housing options, and interacts with landlords and other organizations on behalf of participants. The Navigator uses a comprehensive knowledge of housing and other supportive services to facilitate linkages before, during and /or after permanent housing has been established, and works with participants to enhance their shills in utilizing these various resources. This position also serves as a general housing resource of households served in other TMHA programs.
HOURS/ SALARY:

Responsibilities and Duties:
1. Direct the day-to-day operations of the program staff including scheduling staff work hours; lead the regular organizational staff meetings; evaluate the status of clients, conduct appropriate planning and coordination of support services to ensure immediate attention to their changing needs.
2. Direct and coordinate the program service, residential assessments and intake process, rehabilitation and support services of the program in coordination with the County of San Luis Obispo, State of California HDAP and Department of Social Services regulations; assign the most appropriate staff and provide supervision of the development of the comprehensive service plans program clients.
3. Coordinate the comprehensive assessment of client's psychiatric/medical history (e.g. onset, course and effect of illness, past treatment and responses, and risk behaviors), mental status, and diagnosis; physical health and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living (e.g. self-care, living situation, nutrition, money management); and family structure and relationships.
4. Participate in staff recruitment, interviewing, hiring, work assignments, and orientation and performance supervision according to work rules, regulations, and policies; develop and implement staff orientation and training.
5. Promote, encourage and maintain trauma-informed practices in all services and activities. Practice and promote, both individually and in team meetings, TMHA's Collaborative Communication Guidelines in order to facilitate clear, empathetic and person-centered communication, Maintain appropriate program office headquarters with suitable furniture, office equipment, and telephone lines, directly or by delegation.
6. Administer the program budgets including line items to purchase necessary furniture, equipment, communications devices (e.g. telephones, cellular phones, computers, fax/copy machines), supplies and to cover travel and transportation (e.g., purchase or lease program cars, or cover mileage and insurance for use of personal staff vehicles or both).
7. Supervise client records management assuring maintenance of the records in compliance with agency policies, and other third-party payment requirements; train staff on client record requirements; regularly review client assessments, service plans, and progress notes written by the staff; and supervise individual staff for client records mastery.
8. Carry out and document quality-assurance activities and review of the use of program services.
9. Develop and maintain program policies and procedures and revise as necessary.
10. Supervise/approve staff time cards through TMHA Paylocity payroll accounting system.
11. Supervise and submit monthly Facility Checklist and Vehicle Logs.
12. Monitor staff training to ensure all staff members are meeting annual and one time training requirements through the TMHA training program (Relias).
13. Initiate and maintain relationships, in coordination with staff, law-enforcement, landlords, employers and other human services agencies.
14. Perform on-call duty for crisis intervention and for staff supervision and consultation if required.
15. Employees in this position are required to participate in a rotating on-call schedule as part of their regular job duties. This includes providing client support services during weekday after-hours and all weekend hours, in accordance with our program's contractual obligations. Flexibility and reliability are essential. Employees must respond, sometimes in person, in accordance with our on-call policy.
16. Participate in client intake interviews; supervise, monitor, and evaluate individual client progress with program plans and goals; advocate for all aspects of a client's health needs; and coordinate referrals to other community services, agencies, or programs.
17. Promote general cooperation and support for the clients. This may include coordinating transportation, recreational, and educational activities; assisting clients with medical, familial, self-esteem, or other needs; encouraging clients to develop a community support system; mediating staff and/or client interactions; and/or providing crisis intervention.
18. Attend all client and agency meetings as needed; lead program staff meetings; serve as agency liaison which may include interactions with other agencies or treatment services.
19. Be responsible for, vehicle, and program maintenance and safety as required by funding sources and OSHA. In addition, program manager administers the Neighborhood Relations Policy and communicates with property owners/landlords.
20. Monitor program budget, implement agency policies, and ensure accurate/ complete program paperwork; this may include rental agreements, resident files, client notes, Homeless Management Information System (HMIS) documentation, client rents, security deposits, time sheets, petty cash, grocery scrip, inventory, vehicle maintenance and operation logs, quarterly and annual reports.
21. Medi-Cal billing provides a critical portion of funding for this program. You are expected to support, guide, and hold accountable program staff to spend at least 50% of their work time in direct service and support of clients AND to reach billing minute goals as set forth by TMHA's Administrative and Management Leadership. Goals are set by an analysis of what is required for program to balance quality client care and maintain fiscal stability.
22. Assist the Division Director in enhancing and expanding the program and its services; this may include developing and implementing innovative services, approaches to service, funding for service, and/or improved management systems.

The Full Service Partnership (FSP) Program provides intensive, community-based support to adults living with serious mental illness who are at risk of acute hospitalization or long-term institutional care. The primary objective of the program is to help clients achieve and maintain stability in the community through structured, individualized support. FSP serves individuals facing complex challenges, including co-occurring substance use disorders, significant trauma histories, and substantial functional impairments. Services are provided in the community-where clients live and feel most comfortable-rather than in clinical or hospital settings. This approach emphasizes hope, resiliency, and recovery, and is grounded in principles of cultural humility, professional integrity, and a commitment to ongoing self-reflection and personal growth for both clients and staff. The FSP Mental Health Therapist provides a comprehensive range of trauma-informed clinical services. These include diagnostic assessments, individualized treatment planning, therapy, care coordination, psychosocial rehabilitation, co-occurring disorder treatment, medication support, social and daily living skills development, supported housing and employment services, and linkage to educational and 24/7 crisis support resources. FSP operates with a “whatever it takes” philosophy. Responsibilities and Duties:
1. Provide a wellness and recovery approach to service delivery for all clients. Foster and maintain a collaborative team approach with the FSP team members. Model a client centered approach to service delivery, bringing a “whatever it takes” approach to meeting the needs of clients in all aspects of their treatment (ie: “doing alongside” clients, recreational/art/music therapy, attending wellness groups with clients).
2. Provide and participate in trainings on best practices and evidenced based approaches to service delivery including: Assertive Community Treatment, integrated treatment of co-occurring disorders, Dialectic Behavior Therapy, Eye Movement Desensitization and Reprocessing, Motivational Interviewing, Seeking Safety, Trauma Informed Care (trainings provided, as needed, and approved by FSP Clinical Team Leader).
3. Medi-Cal billing provides a critical portion of funding for this program. You are expected to reach documentation and billing goals as set by Team Leader and Director.
4. All direct service staff are expected to spend 50% of their work time in direct service and support of clients, unless otherwise assigned by your supervisor.
5. Work with and train team members, clients and family members on mental health services and/or drug or alcohol abuse prevention strategies as part of developing a holistic wellness and recovery plan for the clients.
6. Participate in daily case review and coordination meetings with teammates. Assess and interview potential clients to determine if requirements for admission into FSP Program services are met, or if referral elsewhere is most appropriate. Collaborate with County Behavioral Health on assessments and referrals for admission into FSP services and stepping down of services back to the County BH Clinic.
7. Provide therapy, education and rehabilitation services for individuals, couples, or members of a family. Assist clients in achieving productive social adjustments reducing the impact of disabling disorders, and facilitate the achievement of family, vocational, recreational, legal and interpersonal goals.
8. Document clinical activity in a timely manner, meeting program outcomes, in an electronic medical records system, presenting diagnostic and interview findings, clinical evaluations, and recommendations as a member of an interdisciplinary team; function as the SAI (Single Accountable Individual) for client caseload. Must document and provide at least one contact with each client per week.
9. Provide crisis intervention in the field and in the office for individuals experiencing acute episodes; evaluate clients and arrange for voluntary or involuntary admission to facilities for observation and treatment. Become trained and designated to conducting evaluations and writing of applications for 5150 holds (training provided).
10. Confer with and advise clients' families to promote their understanding of and cooperation in treatment and rehabilitation programs.
11. Employees in this position are required to participate in a rotating on-call schedule as part of their regular job duties. This includes providing client support services during weekday after-hours and all weekend hours, in accordance with our program's contractual obligations. Flexibility and reliability are essential. Employees must respond, sometimes in person, in accordance with our on-call policy.
12. When necessary, perform the duties of a case manager by coordinating services for a caseload of 15-20 clients to assure continuity of care and assure that clients receive appropriate types of service.
