POSITION SUMMARY: Provides hands on resident care, proper and timely administration of medications and treatments, accurate verbal and written reports and rapid response to any/all emergency situations.
ESSENTIAL FUNCTIONS:
Performs professional and restorative nursing services which include, but not limited to, resident assessments, recording of vital signs, administering medications and treatments.
Keeps timely documentation of written reports and verbal reports ensuring that quality care is provided.
Interacts with the resident's physician, pharmacist and medical personnel as needed.
Following the guidelines as outlined in the Resident Care Plan; assists residents with activities of daily living including bathing, dressing, eating, toileting, hygiene, and mobility.
Assists with in-service and on the job training and mentoring of Team Members.
Participates in and attends all required in-service training and education programs as scheduled.
Other duties as assigned.

EDUCATION AND EXPERIENCE REQUIREMENTS:
Current state licensure as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing.
Certification in CPR, AED, and First Aid
High school diploma or GED
Four year college or university degree desired.
1-2 years' experience in a similar healthcare position preferred.
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:
Ability to read, write, speak and understand English fluently.

As the Registered Nurse you will provide and direct provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. You will also coordinate total plan of care with other health care professionals involved in care and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources.
Primary Responsibilities:
Provide high-quality clinical services within scope of practice and infection control standards
Coordinate care with other members of the patient/client's care team from admission to discharge
Complete clinical nursing assessments per federal/state program requirements and payer needs
Ensure patient/client eligibility and medical necessity for services as defined by payer and agency policy
Develop and revise individualized plans of care/service plans with other community providers
Ensure plan of care frequency and duration meets patient needs and initiate revisions with physician approval

Required Qualifications:
Current and unrestricted RN licensure in the state of practice
Current driver's license, vehicle insurance and access to a dependable vehicle or public transportation
Current CPR certification
Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
Preferred Qualifications:
1+ years of RN experience
Able to work independently
Good communication, writing, and organizational skills
