RESPONSIBILITIES
Patient Care & Service. Promote and restore patients' health by completing the nursing process; collaborating with physicians and multidisciplinary team members; performing various treatment procedures; providing physical, educational and emotional support to patients, friends and families; supervising assigned team members
Planning & Communication. Develop and document individualized care plans customized for each patient's unique needs, with support from the interdisciplinary health team as needed; maintain effective communication to convey patient health status, treatment plans and progress
Electronic Health Record (EHR) Management. Demonstrate proficient use of an EHR - including accurate patient and provider documentation and communication
Compliance. Adhere to required department and system protocols, regulations (local, state, federal) and education requirements

A skilled nurse performs a variety of duties to care and comfort for patients requiring medical services. A skilled nurse must utilize specialized skills and judgment when delivering care to patients in a long-term care setting. A skilled nurse implements and evaluates patient care while performing nursing procedures to treat and rehabilitate patients.
Specific duties include, but are not limited to the following:
Provide professional nursing services which may include, but are not limited to, nursing interventions and their documentation, case finding, health teaching/counseling and provision of supportive and/or restorative care.
Participate in staff training, prepare clinical and administrative reports, maintain infection control standards, and function as a member of a treatment team. You may also act as a charge nurse on a unit or in a residence, or as house or unit supervisor in a nursing home.
Assuring that the safety and physical comfort of the patient meet as well as the patient's emotional and spiritual needs.
Performing nursing procedures for which his or her training has provided the necessary skills and judgment.
Assisting with the rehabilitation of patients according to the care plan directed by the physician.
Observing and recording all pertinent information and reporting to the appropriate supervisor and physician.
Medication management.
Completes documentation as required and in accordance with the facilities' policy.
Accurately report patient status to supervisor.
Consistently practice standard precautions.
Performs all duties promptly in a competent and caring manner.
Maintain good communication among patients/residents and all healthcare personnel.
Ability to communicate effectively both verbally and in writing.
Ability to operate and handle many kinds of medical equipment.
Frequently move/lift between 50-100 pounds, depending on assignment requirements.
Stand for long periods with frequent squatting, twisting, bending, kneeling and reaching to prepare equipment, materials or objects in order to provide patient care or maintain the patient environment.
Complies with all facilities' policies, procedures, and practices.
Assists with transfers and ambulation.

Preferred Long Term Care/Skilled Nursing experience.
Shift: Shift: Evenings (With occasional Days/Nights)
Part-Time (24 hours week), every other weekend & some holidays.
Per Diem - 1-8 hour shift a week, with 1 weekend every month.
