Completes home health and hospice admission visits and resumption of care visits during hours scheduled to work. 10. Covers territory assigned by supervisor and conducts on call visits during hours scheduled to work. Triages all patient calls timely and professionally according to patient acuity. 11. Notify attending physicians, staff members, and family members of patient deaths when they occur. 12. Collaborates with Procare Pharmacy regarding medication changes, administration, and recommendations as needed. 13. Maintains regular communication with supervisor.
