*Home Care RN Position Purpose:*
Provides primary nursing care to home based care patients as ordered by the physician, and in accordance to the organization's policies and procedures. The RN is responsible for the initial assessment and care plan development in collaboration with the Population Health Case Manager. Provides functional support/supervision to Home Healthcare Aides and LPNs as needed. Provides therapeutic intervention, overall care management and achievement of top decile client outcomes and patient satisfaction.
*What You Will Do:*
* Accurately assesses patient condition and risk at admission, at required time points, and as needed, utilizing appropriate standardized documentation. Assesses the patient's condition and needs and determines the appropriate level of care.
* In collaboration with the physician, Population Health Case Manager, and patient, develops appropriate assessment-based plan of care including: attainable patient goals and services needed to improve and/or stabilize patient's medical condition, functional abilities and promote independence.
* Maintains a current working knowledge of applicable Federal, State and local laws and regulations, the Organizational Integrity Program, Code of Ethics, as well as agency policies and procedures and follows Code of Conduct.
* Reports changes in patient condition in a timely manner to the physician and/or Population Health Case Manager. Obtains orders for changes in the plan of care to respond to the patient's condition.
* Follows orders for treatment and provides services as indicated on the plan of care.
* Utilizes and implements appropriate educational tools and pathways per guidelines and/or policy and procedure
*Minimum Qualifications: *
* Graduate of an approved nursing education program, BSN preferred.
* *Licensure as a Registered Nurse in the State of Connecticut.*
* *One (1) year experience as a professional acute care nurse*
* Home care experience preferred but not required
* *Must have current Driver's license and reliable transportation*

MANAGEMENT

JOB CONDITIONS
Must have a driver's license and be willing and able to drive to patient's residences.
The ability to access patients' homes which may not be routinely wheelchair accessible is required. Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform and demonstrate patient care.
Physical activities will include, walking, sitting, stooping, and standing and minimal to maximum lifting of patients and the turning of patients.
The ability to communicate both verbally and in writing is required as frequent communication by telephone and in wrirting in English is required.

QUALIFICATIONS

Must have knowledge of Medicare and Medicaid guidelines.
Must have a working knowledge of home health care and the principles and techniques of professional nursing and required documentation that pertains to it.
Should be skillful in organization and in the principles of time management and have knowledge of management processes.
Must be able to contribute to the quality of care being rendered through constructive communication with nursing managers and staff.

Address and answer client questions using the Restore Nursing Reference Guide under the supervision of the Medical Director and network of licensed Advanced Practice Providers.
Communicate to the medical compliance team any issues or adverse effects clients experience.
Provide therapeutic communication along with exceptional customer service
Assist staff with store services.
Assist with off-site nursing events and special projects.

Qualities You Need to Succeed as a Restore Nurse
You're a licensed Registered Nurse (RN).
You have at least two years of experience as a nurse in an acute care setting (ER, ICU, Med-Surg or similar).
You are BLS (Basic Life Support) certified
You have at least one year of experience administering injections.
You're comfortable using electronic medical records to document client details.
You maintain a non-judgemental outlook and have a demonstrated ability to work with a diverse clientele.
