The role
The RN Case Manager provides intermittent skilled nursing services; communicates the patient's progress with other disciplines and directs, supervises and instructs hospice aide staff in the provision of personal care to the patient. As a RN Case Manager you will:
Key Responsibilities:
Under the physician's order, admit patients eligible for hospice services
Assess and evaluates patient needs/problems, identifies mutually agreed upon goals with patients
Report patient status and need for other disciplines to clinical leadership, attending physician and hospice physician
Update care plans on an ongoing basis; revise and resolve patient problems and goals as changes occur and/or recertification
Complete informational visit and obtain patient consents for hospice admission per office procedure
Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care
Skills and Experience Required:
Current unencumbered registered nurse in the state of practice or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC)
Must maintain a valid driver's license and good driving record
Ability to work in a field setting and exhibited ability to make sound nursing judgments
Ability to assess patient needs and formulate individualized patient care plans to meet those needs

Sentara Leigh Hospital Intensive Care Unit, AACN Gold Beacon Award recipient, is comprised of 20 licensed critical care beds and has an average daily census of 18 patients. Staffing ratios vary and can be 1:1 to 1:3 with 1:2 being the usual assignment.
The ICU RN's are required to have a wide array of knowledge, skill and ability to competently and safely provide critical care to our patients. Frequent ongoing education, new skill acquisition and skill validation are provided and supported by an experienced ICU CNS for new and experienced ICU nurses.
This unit welcomes experienced nurses moving into this specialty. The 11-week Transition to Practice program focuses on the orientation transition experience with intentional knowledge support and skill acquisition for practice in IMC areas.
ICU staff are supported by an interdisciplinary care team which in part includes unit based respiratory, physical, speech and occupational therapists. Interpreter, nutrition, palliative, chaplaincy and ethical services are readily available as well. Our ICU has onsite and eICU pulmonary critical care intensivists and their team of physician extenders 24/7. Physician lead, nurse driven interdisciplinary rounds are held daily for continuity of care and to address nursing's concerns or questions. These rounds are made educational and even fun at times by our very engaged intensivist MD's.

The ideal candidate will have cardiac experience and be comfortable reading telemetry independently.
The unit prepares and recovers patients for procedures in the Cath lab, EP lab, and Minor Procedure Room as well as the Cardiac OR for surgery.
The types of procedures that we prepare and recover patients for include but is not limited to: Cardiac Catheterization (interventional and diagnostic), Electrophysiology procedures such as cardiac ablations, Cardioversions, surgery, and ICDs/Pacemakers etc.
Education:
