Position Summary:
Provides individualized and quality evaluations, assessments, prognoses, treatments and education to meet each patient's needs based on medical necessity and clinical indication in accordance with the Physical Therapy Practice Act and Title 16 requirements. These services will be provided in settings across the entire continuum of care including: inpatient acute care, inpatient acute rehabilitation, and outpatient services. Adheres to professional standards of conduct and ethics as outlined by department expectations and professional organizations. Maintains productivity, weekend and holiday requirements as specified. Ongoing professional development, and continuing education to promote clinical excellence and evidence based practice. Assists patients in reaching their maximum performance and level of functioning, while learning to live within the limits of their capabilities.

Qualifications:

Position Summary:
Provides individualized and quality evaluations, assessments, prognoses, treatments and education to meet each patient's needs based on medical necessity and clinical indication in accordance with the Physical Therapy Practice Act and Title 16 requirements. These services will be provided in settings across the entire continuum of care including: inpatient acute care, inpatient acute rehabilitation, and outpatient services. Adheres to professional standards of conduct and ethics as outlined by department expectations and professional organizations. Maintains productivity, weekend and holiday requirements as specified. Ongoing professional development, and continuing education to promote clinical excellence and evidence based practice. Assists patients in reaching their maximum performance and level of functioning, while learning to live within the limits of their capabilities.
Major Position Responsibilities:

* Maintains a safe, clean working environment, including unit based safety and infection control requirements.
* Accurately perform individualized and quality evaluations, assessments, prognoses, treatments and education to patients and families/caregivers.
* Provide care in inpatient acute care, inpatient acute rehabilitation, and/or outpatient settings based on patient care and business needs.
* Meet weekend and holiday requirements based on FTE status.
* Independently assess all areas relevant to patient function, utilize sound clinical decision making skills, develop a comprehensive and individualized treatment plan, and complete all required documentation in a timely manner.
* Accurately enters data in Tri-City Medical Center's Electronic Health Record System
* Participates as a member of the interdisciplinary team.
* Provide mentorship as requested (voluntary)
* Follows organization's rules, policies, procedures, applicable laws and standards while carrying out the mission, vision, and values of Tri-City Health District.
* Other duties as assigned by Supervisor/Manager.
Qualifications:
* 1 - 3 years' experience as a Physical Therapist, preferred.
* Competent in provision of Physical Therapy modalities, interventions, and procedures, required.
* Knowledge and proficiency of standard office equipment (i.e., fax, photocopier), personal computer and computer software skills (i.e., MS Windows Outlook, Excel, Word, PowerPoint, internet), required.
* Excellent communication and problem-solving skills, required.
* Excellent interpersonal and organizational skills, required.
* Strong Customer Service skills, required.
Education:
* Graduate of an approved professional school or graduate of an accredited college or university with a degree in Physical Therapy, required.
Licenses:
* Licensed or eligible for licensure by the Physical Therapy Board of California, required.
Certifications:
* Current BLS certification, required upon hire; American Heart Association Healthcare Provider.

Join our team as a Certified Hand Therapist at our out-patient clinic at Scripps Memorial Hospital Encinitas. You will partner with other experienced Certified Hand Therapists to build strong relationships with both our patients and physicians and see a variety of cases from post-surgical to orthopedic or neurological. Our approach is patient-centered, and the ideal candidate is eager to learn and ready to jump into this multi-disciplinary environment and make a difference.
The Certified Hand Therapist is a Physical or Occupational Therapist who evaluates and assesses patient function or ability, including establishing a plan of care according to standards of practice. Performs reassessment relative to initial evaluation and modifies plan of care as needed with outpatient hand therapy patients. Demonstrates clinical skills with upper extremity splinting and hand therapy treatment interventions.

Required:
