PRIMARY RESPONSIBILITIES
* Implements and monitors patient care plans. Monitors, records and communicates patient condition as appropriate.

Are you an RN who has a passion for hospice care? Maybe you have family obligations during the day and you are missing nursing, helping patients? Do you think you could take patients calls, assess their clinical needs and if needed able to go out to their home during the hours from 5pm to 8am Wednesday and Thursdays? While the majority of calls can be handled over the phone there are times when the patient will need to see you. Flexibility is needed as Initial training will be weekdays. This position could be ideal for the right person! Position requires flexibility to help in holiday rotation.
Preferred minimum qualifications include one to two years of nursing or triage, oncology, hospice, palliative care or med/surge experience. We are willing to train! Must be able to work independently, have excellent communication, strong clinical and assessment skills. BSN preferred, will consider Associates RN with relevant experience. NH RN license and CPR certification required. Netsmart formerly Allscripts EMR a plus!
