Triage nurse job description

Awardees receive up to $50,000 in exchange for two years of service if they practice in a HPSA of 14 above (Tier I). Awardees can also receive up to $30,0000 in exchange for two years of service if they practice in a HPSA of 13 and below (Tier II). Provides direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.

PRIMARY RESPONSIBILITIES

* Implements and monitors patient care plans. Monitors, records and communicates patient condition as appropriate.
* Serves as a primary coordinator of all disciplines for well-coordinated patient care.
* Notes and carries out physician and nursing orders.
* Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
* May round with physician in an inpatient setting.
* Applies the existing body of evidence-based practice and scientific knowledge in health care to nursing practice, ensuring that nursing care is delivered based on patient's age-specific needs and clinical needs as described in the department's Scope of Service.
* As an SSM Health nurse, I will demonstrate the professional nursing standards defined in the professional practice model. Uses the ANA Code of Ethics for Nurses to guide his/her response to the current and evolving health and nursing needs of our patients and our patient populations.
* Works in a constant state of alertness and safe manner.
* Performs other duties as assigned.

* Graduate of accredited school of nursing

* Constant use of speech to share information through oral communication.

Are you an RN who has a passion for hospice care? Maybe you have family obligations during the day and you are missing nursing, helping patients? Do you think you could take patients calls, assess their clinical needs and if needed able to go out to their home during the hours from 5pm to 8am Wednesday and Thursdays? While the majority of calls can be handled over the phone there are times when the patient will need to see you. Flexibility is needed as Initial training will be weekdays. This position could be ideal for the right person! Position requires flexibility to help in holiday rotation.

Preferred minimum qualifications include one to two years of nursing or triage, oncology, hospice, palliative care or med/surge experience. We are willing to train! Must be able to work independently, have excellent communication, strong clinical and assessment skills. BSN preferred, will consider Associates RN with relevant experience. NH RN license and CPR certification required. Netsmart formerly Allscripts EMR a plus!
