What You'll Do:
Provides social work services in accordance with the physician's plan of treatment.
Works with the interdisciplinary team directly, or through coordination of team members, to provide appropriate assessment and support patients and families.
Provides assessment for normal and complicated grief and provides bereavement counseling as appropriate.
Serves as a liaison with other community resources.
Job Requirements:
Master's or Bachelor's level social work degree from a school of social work accredited or approved by the Council of Social Work Education.
Currently licensed to practice as a social worker in the state of SC.
Minimum of one-year Hospice experience preferred.
Valid Driver's License.

Become the Case Manager you always wanted to be
Work with interdisciplinary team, guiding treatment plans based on patient needs and preferences.
Coordinate with interdisciplinary team to establish tentative discharge plan and contingency plans
Participate in planning for and the execution of patient discharge experience.
Monitor patient experience: quality/timeliness/service appropriateness/payors/expectations.
Facilitate team conferences weekly and coordinate all treatment plan modifications.
Complete case management addendums and all required documentation.
Maintain knowledge of regulations/standards, company policies/procedures, and department operations.
Review/analyze case management reports, including Key Care Indicators, and plan appropriate actions.
Understand commercial contract levels, exclusions, payor requirements, and recertification needs.
Attend Acute Care Transfer (ACT) meetings to identify trends and collaboratively reduce ACTs.
Meet with patient/family per Patient Arrival and Initial Visit Standard within 24 hrs. of admission.
Perform assessment of goals and complete case management addendum within 48 hours of admission.
Educate patient/family on rehabilitation and Case Manager role; establish communication plan.
Schedule and facilitate family conferences as needed.
Assist patient with timely procuring/planning of resources to avoid discharge delays or issues.
Monitor compliance with regulations for orthotics and prosthetics ordering and payment.
Make appropriate/timely referrals, including documentation to post discharge providers/physicians.
Ensure accuracy of discharge and payor-related information in the patient record.
Participate in utilization review process: data collection, trend review, and resolution actions.
Participate in case management on-call schedule as needed.
Qualifications
License or Certification:
Must be qualified to independently complete an assessment within the scope of practice of his/her discipline (for example, RN, SW, OT, PT, ST, and Rehabilitation Counseling).
If licensure is required for one's discipline within the state, individual must hold an active license.
Must meet eligibility requirements for CCM or ACM™ certification upon entry into this position OR within two years of entry into the position.
CCM or ACM™ certification required OR must be obtained within two years of being placed in the Case Manager II position.
Minimum Qualifications:
For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirement for licensure or certification is a diploma rather than an associate degree.
For all other eligible licensed or certified health care professionals, must possess a minimum of a bachelor's degree and graduate degree is preferred.
2 years of rehabilitation experience preferred.

Manages and collaborates in discharge planning for patients with simple and complex needs and coordinates with agencies providing post hospital care. Essential Functions

Screens patient using high risk indicators but not limited to frequent hospitalization, inadequate support systems, readmissions, homelessness, abuse, neglect, exploitation and domestic violence.
Assesses for immediate discharge planning needs and develops and coordinates a discharge plan as determined by the patient's medical condition, level of independence, functional capacity, family/social support systems, and financial resources.
Assesses for long term and/or future patient care needs by identifying probable changes in level of independence or functional capacity in collaboration with patient/family and interdisciplinary care team. Provides information and education on community resources to empower patient/family decision making.
Communicates discharge plan to patient/family and interdisciplinary care team by timely documentation in the medical record, active participation in discharge rounds, and/or verbal dialogue as appropriate. Collaborates with patient/family to develop the discharge plan.
Documentation in the medical record is legible and conforms to hospital department policies and procedures including but not limited to screening, initial discharge plan, weekly status updates and whenever changes in the plan occur.
Assesses the patient's behavioral health needs in collaboration with physician, psychiatrists, and other members of the mental health team.
Provides counseling, crisis intervention, and psychosocial assessments to patients/families with appropriate medical staff supervision.
Collaborates with internal and external resources to develop a short term and long-term plan of care as appropriate.
Assumes leadership role in facilitating meetings and comprehensive care planning with interdisciplinary team when necessary.
Provide clinical oversight for graduate level MSW students.
Collaborates with the department's leadership team in assessing department needs and designing systems to meet department and organizational strategic initiatives.
Performs other duties as assigned.

Participates in outreach and community events that align with program objectives and goals.
Maintains appropriate documentation utilizing computerized charting.
May attend Team Conferences and participate in interdisciplinary planning to ensure documentation and procedures including but not limited to screening, initial discharge plan, change of condition updates and final discharge plan are completed.

Assists indigent population with needed resources to aid in prevention of readmissions and support quality in patient outcomes.
Consistently handles customer complaints promptly and effectively when problems encountered. Utilizes proper escalation procedures when appropriate.
Performs all other duties as assigned.

Specific Responsibilities:
Provides and accurately documents direct social services to patients planned to restore the patient to optimum social and health adjustment; including:
Assesses social and emotional factors, to estimate impact on the patient's capacity and potential to cope with problems of daily living
Helps the patients and caregivers to understand, accept, and follow medical recommendations
Assists patients and caregivers with personal and environmental difficulties which predispose toward illness or interfere with obtaining maximum benefits from medical care; and
Identifies resources, such as caregivers and community agencies, to assist the patient to resume life in the community, including discharge planning
Provides these services according to physician order and in accordance with home care policies and procedures.
Educates and supervises designated social work assistants in the implementation of the social service program.
Provides inservice training to other home care team members, focusing on psychosocial needs of patients, families and staff, as requested.
Contributes to applicable QI activities as requested.
Serves as Utilization Review and/or Advisory Board Member as requested by Administrator/Director of Services.
Maintains knowledge base of home care social services procedures and regulations as well as professional standards through attendance at workshops and inservice and individual research and reading.
