ESSENTIAL FUNCTIONS
1. Knows, understands, incorporates, and demonstrates the Trinity Health Mission, Vision, and Values in behaviors, practices, and decisions.
2. Makes available, delivers, and facilitates transformative Spiritual Care experiences to patients, residents, participants, visitors, and all colleagues across the healthcare continuum, and the communities served. Ensures that the religious beliefs, practices, and values of those served are honored and respected within the total healing process.
3. Implements systems and processes to assess religious and spiritual needs, including needs for ethics consultation, and provides interventions or referrals based on the assessed needs.
4. Makes timely, professional observations of apparent outcomes of Spiritual Care provided and adjusts the continued provision of Spiritual Care as needed.
MINIMUM QUALIFICATIONS
1. Master's degree in Theology, Religion, Spiritual Care or Spirituality, and/or equivalent combination of education and experience.
2. One (1) to three (3) years of Spiritual Care experience, including CPE training, is required. Chaplaincy or Spiritual Care ministry experience is preferred. An equivalent combination of experience may be considered. Current or recent hospice field experience preferred.
3. Board Certified Chaplain (BCC) strongly preferred via one of the following licensing groups: APC, CASC, NACC, and NAJC, which includes completion of a master's degree as required by one of the certifying agencies from a Council for Higher Education Accreditation (CHEA) academic institution with completion of 4 units (1600 hours) of ACPE accredited Spiritual Care education, and faith-based endorsement as recognized by one of the certifying agencies.

Location: Remote / Virtual Care Center (based in Athens, GA; Mishawka, IN; Silver Spring, MD; or Columbus, OH Schedule: Full-Time | Monday-Friday | 9:30am-6:00pm EST w/ every other weekend rotation As a Virtual Care Coordinator RN, you will play a pivotal role in delivering telehealth hospice care, ensuring that patients and their families receive personalized support during critical moments. Utilizing virtual platforms, you will monitor patient conditions, coordinate care plans, and provide real-time guidance to field staff and caregivers.
This role is ideal for a Registered Nurse who thrives in a fast-paced, team-based setting and is passionate about leveraging technology to improve patient outcomes in the home setting.
What You Will Do:
* Conduct virtual assessments and monitor patients' conditions remotely.
* Develop and coordinate individualized care plans in collaboration with the interdisciplinary team.
* Provide virtual education and support to patients, families, and caregivers regarding symptom management and end-of-life care.
* Liaise with field nurses, physicians, social workers, and other team members to ensure comprehensive care delivery.
* Respond promptly to urgent virtual consults and triage patient needs effectively.
* Utilize telehealth technology to conduct regular check-ins and follow-ups with patients and families.

* Pay Range Maryland Only: $36.70-$58.71 per hour
Minimum Qualifications:
* 5+ years of Home Care, Hospice, ER and/or Triage RN Experience
* Active RN license in a Compact State (eNLC)
* BSN required by an approved Nursing education program.
* Proficiency in Epic or Home Care EMR Experience
* Strong communication and interpersonal skills to build rapport with patients and families virtually
* Ability to work independently and as a part of collaborative remote team
* Able to adapt to rapid changes to meet business/department needs
* Patient/Customer Service Experience
