Responsibilities * Manages the care and treatment of patients as indicated in the professional scope of practice and within current standards and protocols. * Develops a comprehensive plan of care with the patient/family consistent with established guidelines * Practices within the guidelines of a Standard Care Arrangement with a collaborating physician. * Initiates and leads performance improvement activities with a focus on achieving top quartile on clinical indicators. Requirements/Qualification * Education: Masters required in Science, Nursing or specialty field. Graduate of Advanced Practitioner Program. * Or completed a Physician Assistant program and licensed. * 2 plus years of advance practice experience in cardiology highly preferred * Current License as RN in Ohio. Certified as Advance Practice Nurse in designated specialty. Observes and reports changes in the residents' physical condition and cognitive/emotional status to ALSA Supervisor or Director of Assisted Living Services, as needed 5. Conducts room checks on resident rounds, monitors for environmental safety hazards, records incidents, errors or accidents and records resident and resident related events. 5. Assists in a variety of tasks related to dining (escorting residents, set-up and clean-up, serving meals) 6. Responsible for participating in and supporting the resident centered activity program. Completes required written documentation concerning resident care and activities during shift 7. Is a resident advocate at all times 8. Follows agency policies concerning confidentiality 9. Uses tactful, diplomatic communication in potentially sensitive or emotionally charged situations and follows up with appropriate staff, resident or others regarding reported complaints, problems and concerns. 10. Acts as ambassador to guest and other off-campus visitors and maintains certifications required for employment 11. Performs other duties as required Requirements: No experience required. Provides and maintains safe environment for patient. Uses equipment safely and properly (footstools, side rails, Oxygen, etc.). Practices Universal Infection Control Precautions in all contacts with patients. Performs or assists patient/family with home maintenance activities as prescribed in the care plan, including but not limited to light housekeeping and meal preparation. Monitors adequate fluid and nutrition intake, temperature, pulse, respiration, skin and basic elements of body functioning and changes in body functioning and record/report appropriately. Practices safe transfer, ambulation, normal range of motion and positioning techniques. Identifies need for intervention by other members of the home health team on an ongoing basis and reports to the RN or Director of Clinical Services. Respects the autonomy and right to confidentiality of all patients and families. Document scare provided and response of consumer/family. Submits documentation in a timely manner. Performs within the limits of preparation and experience. Ensures compliance with local, state and federal laws, Medicare regulations and established agency policies and procedures. Manages a caseload and delivers care in a cost-effective manner. Consistently meets or exceeds 365 Health Services' established service standards. Conducts all business activities in a professional and ethical manner. Uses supplies and equipment effectively and efficiently. Completes a minimum of 12 hours of in services per year. Maintains regular attendance. Adheres to 365 Health Services' Code of Conduct. Interacts with all staff in a positive fashion supporting 365 Health Services' mission and vision. Conducts all business activities in a professional and ethical manner. Complies with 365 Health Services' general orientation and with agency policies and procedures including confidentiality and HIPAA guidelines.
