What the role will entail:
Perform initial and ongoing patient/family, physical, emotional, psycho-social, cultural and spiritual assessments to ensure and coordinate the delivery of comprehensive, holistic services by the Interdisciplinary Hospice Team.
Provide palliative nursing care and symptom control in accordance with physician orders and IDG plan of care.
Documents and enters into EMR all assessments and interventions before the end of the shift.
Develop and prioritize the interdisciplinary care plan goals of the patient/family, hospice team and attending physician, prior to team meetings.
Provide anticipatory guidance to the patient/family to assist in preparing for coping with events as they occur.
Can effectively communicate in English, both verbally (in person and by phone) and in writing.
Revises nursing diagnosis, goals and interventions, expected outcomes, and plan of care based on assessment of data and input of patients and family members prior to the weekly IDG meeting.
Who we are looking for:
Graduate of an accredited nursing school.
Valid New York State nursing license.
Minimum of at least 2 years of nursing experience required
Minimum of at least 1 year ICU, Med-Surg, ER or similar required; one year of hospice experience preferred
Venipuncture and IV placement certification preferred
Ability to exercise critical thinking skills
Skill to prioritize and handle multiple tasks effectively
WORK ENVIRONMENT/PHYSICAL DEMANDS:

Work in fast paced environment
Ability to lift at least 30 pounds of supplies and/or equipment
Moderate/High level of possible exposure to infectious agents.

Pay: Starting $87,500/yr; 5% night differential for night shifts
