Keep accurate, concise, and thorough documentation of all medical history, exam results, and discussions with patients in relation to their health care. Close all notes in the EMR in a timely manner.
Review and sign off on all laboratory and radiology results. Ensure that there is patient follow up when necessary
Initiate and follow through on specialist referrals. Communicate with specialist providers when necessary.
Serve as a clinic supervisor for mid-level providers.
Meet productivity standards for care providers, including an goal of seeing 16-24 patients daily.
Participate in care team huddles, provider meetings, and all-staff meetings.
Perform all other related duties as assigned.
Education
Licensed to practice medicine in the state of California
DEA certificate
CPR Certification
Skills / Experience
1. A keen interest in helping the underserved population (e.g. the homeless population).
2. FQHC Experience a plus
3. Familiarity with Quality Measures a plus
4. Able to be successfully credentialed by BACHC.
5. Strong communication and interpersonal skills
6. Bilingual (English and Spanish) a plus
Physical Demands
This is a physical position that will require standing on your feet, and walking. This would require the ability sit, walk, bend or stand as necessary, and ability to lift up to 25lbs.

Virtual Care Physician (aka Virtualist)

The Permanente Medical Group, Inc. (TPMG) is seeking physicians who are board certified or board eligible by an American Specialty Board to provide virtual care in a variety of ways that may include, but not limited to, scheduled and unscheduled video visits, telephone visits, E-visits, chat, and secure messages.
Positions can be fully virtual/remote.
Applicants must reside in and be licensed to practice in California
.
Five years of post-residency experience required.
Applicants may also be required to receive licenses in multiple other states.
Ideal candidates:
Must be flexible with the duration of work shifts and 24/7 availability, including weekdays, weekends, and holidays.
Must be willing to work shifts in flexible increments (i.e. 1-10 hour units).
Ability to deliver exceptional care experience.
Possess excellent documentation and coding skills in the EMR accurately and efficiently.
Embody a commitment to high-quality medical care with the ability to contribute to organizational quality goals on population health.
Handle a fast-paced environment with competing priorities efficiently.
Adhere to established guidelines for evidence-based care with a focus on resource management.

Full-time annual salary range is $214,440 to $221.100.

Employed by PAGNY for The Division of Correctional Health Services (CHS) of New York City Health + Hospitals (H+H) is responsible for the delivery of medical care, mental health care, and discharge planning in the jail system. Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. This service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re- entry services, and relationships with multiple training institutions throughout the city.
Staff physicians provide primary care to patients in jail-based clinics, perform intake evaluation and treatment and provide emergency care with the support of Emergency Medicine physicians.
The healthcare system is a single-payer system that requires no physician time dedicated to billing. No administrative work is expected outside of work hours, promoting career/life balance.
Physicians work as part of a care delivery team with strong administrative support staff, integrated nursing and mental health.
Responsibilities:
Completes comprehensive histories and physicals on all new admissions.
Diagnose and treating acute and chronic illness
Evaluates the need for consult services and submitting the prioritized consult when indicated
Reviews all on island/off-island specialty consult clinic and hospital returns.
Evaluates the recommendations to ensure that the standard of care is met and implements the recommendations of the consultant
Responds to emergencies and fire drills on a priority basis and in a timely and professional manner
Be familiar with performance indicators (P.I.) and taking appropriate measures to comply with these standards
Requests radiology exams, lab-tests, and EKG's when indicated and interpreting these results in conjunction with the patient's clinical status
Notifying Urgicare for 3 hour/EMS runs
Generates medical modifications as needed
Maintains clinical competency by reviewing CHS protocols, current medical literature and C.M.E.'s
Provides clinical supervision for P.A.'s/N.P.'s which includes co-signing of notes and medication orders
Ensure that standard of care is maintained
Adhere to policies and procedures of CHS and be familiar with them by reviewing them as needed
Attends all mandatory in-services
Complies with and maintaining Employee Health requirements
This position is located on Rikers Island

Registered to practice medicine in the State of New York and have Federal DEA, BCLS or ACLS, and Infection Control Certification

The candidate will join three physicians and one Nurse Practitioner in a busy outpatient practice. The practice has a good mix of general internal medicine patients as well as medically complex patients. There is also an opportunity for candidates to treat palliative medicine patients, if desired.
The schedule for this opportunity is 36 patient contact hours and four administrative hours. Call is 1:3.
