Your Role & Impact
- Champion Patient Care - Provide Primary Care coverage to patients in need within our FQHC network. Possibility to float between FM, PEDS, and Urgent Care. Join a 1:4 Saturday Clinic Rotation (replaces one weekday clinic).
- Enhance Clinical Operations - Streamline processes to improve patient flow, satisfaction, and provider efficiency. 18 locations open 8am-5pm Monday-Saturday.
- See 20 patients per day in 15 or 20 min scheduling blocks | 2:1 MA to Provider Ratio, 3 Exam Rooms to Rotate between.

What We're Looking For
- Strong background in Primary Care experience. At least 2 years of practice experience, post-training experience
- Active California Medical License. Current and Unrestricted DEA
- Board Certified with either AANP or NCCPA
- Commitment to Patient-Centered Care & Health Equity.
- Bilingual in Spanish & English (Preferred but not required).

Requirements:

As a Home Care Aide/Caregiver, here's a big-picture view of what you'll do:
Provide the personal care and support seniors need to live safely at home
Assist with activities of daily living such as bathing, dressing, grooming, light housekeeping, meal preparation, shopping, transportation and companionship
Help with ambulation, transferring and range of motion exercises
Provide medication reminders, document their condition and notify a supervisor of any concerns
Ensure a safe home environment with unobstructed pathways
Participate in activities that bring clients joy such as puzzles, games, reading and hobbies
A few must-haves for Home Care Aides/Caregivers:
High school diploma (or equivalent) and active Home Care Aide (HCA) registration in CA
Six (6) months of experience in a healthcare role, preferred
CPR certification
Covid-19 vaccination card
Valid CA driver's license, auto insurance and transportation
Compassionate and helping nature, good communicator and ability to lift up to 50 lbs.
