Your Role & Impact
As a Behavioral Health Consultant, you'll make a real difference in the lives of our patients by being part of the primary care treatment team, identifying, triaging and managing primary care patients with medical and behavioral health problems. This includes providing skill training through psychoeducation and patient education strategies and developing specific behavioral/therapeutic treatment plans for patients. The BHC also assists primary health care providers in recognizing and treating mental disorders and psychosocial problems, detecting "at risk" patients, and assessing the clinical status of patients referred by the primary care provider.
What We're Looking For
* Master's degree in Social Work, Marriage and Family Therapy, Clinical Psychology or PhD / PsyD in Psychology from an accredited program.
* Current Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT) by the California Board of Behavioral Sciences (BBS), Licensed Psychologist by the California Board of Psychology,
* Current Basic Life Support (BLS) Certification for Healthcare Providers from American Heart Association
* Valid California Driver's License, proof of insurance and clean driving record.
* Bilingual English/Spanish
* Experience providing health psychology services such as smoking cessation, weight management, stress management, substance abuse disorders, or work with chronically ill patients.
* A general understanding of trauma-informed care approaches and therapies, Adverse Childhood Experiences and their impact on brain development, suicide prevention and acute crisis assessment/intervention.
* Familiarity or work experience in integrated behavioral health models (IBH).

Role Overview:
Responsible providing individual, family, and group therapy services in acute care settings for youth and adolescent clients experiencing a range of emotional and behavioral challenges. The Clinical Psychologist will complete psychological screening and assessments across programs. Services will include providing evaluation, consultation, and monitoring of individuals receiving involuntary care. Essential Duties:

Required:

QUALIFICATIONS:

Position Summary:
HomeCare Maryland is seeking a compassionate and experienced Occupational Therapist (OT) to join our growing Home Health team. In this role, you will perform skilled occupational therapy services in patients' homes, helping them regain independence and improve their quality of life through customized, evidence-based interventions.
As an Occupational Therapist with HomeCare Maryland, you'll conduct comprehensive assessments, develop individualized treatment plans, and collaborate with an interdisciplinary care team to ensure exceptional outcomes for our patients across Baltimore City and Baltimore County.
Essential Functions:
Perform initial and ongoing assessments of patients' clinical and functional limitations, safety, and equipment needs in the home setting.
Conduct home visits according to prescribed schedules and patient needs.
Develop and implement personalized therapy plans of care based on assessment findings, including ADL and IADL functioning.
Establish realistic, individualized patient goals and revise them as conditions change.
Complete COTA (Certified Occupational Therapy Assistant) supervisory visits per agency policy and Medicare regulations.
Identify and refer patients to additional services when appropriate.
Document each visit, intervention, and patient progress accurately and timely in accordance with agency policy.
Communicate changes in patient condition to the physician and other members of the care team.
Participate in ongoing interdisciplinary collaboration and assessment for comprehensive patient care.
Qualifications:
Bachelor's degree in Occupational Therapy from an accredited college or university (Master's degree preferred).
Current Occupational Therapist license in the State of Maryland.
Current BLS certification (American Heart Association - for healthcare providers).
3-5 years of clinical experience (prior home care experience strongly preferred).
Strong clinical, assessment, and documentation skills.
Reliable transportation and willingness to travel to patient homes throughout Baltimore City and County.

HomeCare MD is seeking a compassionate and skilled PRN Speech Therapist to join our growing home care team. This role is ideal for an experienced Speech Language Pathologist who is passionate about providing exceptional patient-centered care in the comfort of a patient's home.
Position Summary
The Speech Therapist is responsible for conducting initial and ongoing patient assessments, delivering skilled Speech Language Pathology services, and acting as case manager for speech-therapy-only patients. This role will collaborate closely with physicians, caregivers, and interdisciplinary team members to support optimal patient outcomes.
Essential Functions
Develop individualized plans of care tailored to each patient's needs.
Recommend and train patients on augmentative and alternative communication methods (e.g., automated devices, sign language).
Teach patients techniques to improve speech production, voice quality, and oral/written language skills.
Educate and train patients in safe swallowing strategies or strengthening exercises to prevent aspiration.
Maintain accurate and timely documentation, including evaluations, progress notes, and reports for insurance justification.
Conduct research-based assessments and contribute to treatment methodologies.
Participate in interdisciplinary assessments and collaborate in creating and evaluating patient care plans.
Attend case conferences and required agency meetings.
Deliver services as prescribed by the patient's physician and in accordance with the plan of care.
Provide patient, caregiver, and family counseling and education.
Communicate effectively with physicians, healthcare practitioners, and the QAPI department.
Participate in agency-sponsored in-service training and work collaboratively with clinical leadership.
Education & Experience Requirements
Current Maryland Speech Language Pathology License.
Master's degree from an accredited college or university.
Current American Heart Association BLS for Healthcare Providers.
2-3 years acute care experience preferred.
2-3 years community health experience preferred.
Valid driver's license, automobile insurance, and ability to meet fleet safety requirements (travel required).
Strong knowledge of medical terminology.
Excellent interpersonal, communication, and people management skills.
Basic computer proficiency; Microsoft Office Suite preferred.
Experience with electronic medical records; Kinnser software experience preferred.
Basic knowledge of Medicare/Medicaid/OASIS/Commercial Insurance requirements.

As an Outpatient Physical Therapist at Rocky Mountain Orthopedics, you will play a vital role in supporting our clinical team, ensuring our patients receive the best care by:
Competent Services: Performing and documenting initial and ongoing assessments of patients' conditions. Developing plans describing a treatment strategy, its purposes, and its anticipated outcome. Plans often include exercise regimens at the clinic and at home to increase flexibility, strength, or endurance. Using various modalities to relieve pain and reduce swelling.
Documentation and Monitoring: Providing information regarding appropriate selection/use of adaptive equipment and support programs. Helping to restore physical function, improve mobility, relieve pain, and prevent permanent disability. Determining patients' ability to be independent and reintegrate into the workplace or community after injury or illness.
Communication: Promoting the mission, vision, and values of Intermountain Health, and abiding by service behavior standards. Communicating effectively with patients, families, physicians, and healthcare providers about patient needs and goals.
Environment Maintenance: Maintaining a clean and inviting work environment according to health department codes and facility standards.
Research and Development: Assisting in research initiatives as assigned.
Qualifications
Current Physical Therapist license in states where you work.
Basic Life Support (BLS) for healthcare providers.
Basic Computer skills.
Exceptional interpersonal and communication skills.
Possess skill sets and experience with target population of job setting.
Current driver's license, reliable transportation, and acceptable driving record.
Physical Requirements:
Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.
Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.
Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc.)
May be expected to stand in a stationary position for an extended period of time.

As a Pediatric Occupational Therapist at St. Mary's Regional Hospital, you will play a vital role in supporting our clinical team, ensuring our young patients receive the best care by:
Competent Services: Provides skilled occupational therapy services in an outpatient setting, staying updated on standard practices for different patient groups. Conducts evaluations and treatments according to professional standards (AOTA), considering diagnosis, history, and physician referrals, and develops care plans with specific goals based on patient assessments.
Documentation and Billing: Keeps thorough and timely patient records as required by regulations and facility policies. Completes patient billing accurately and promptly.
Communication: Communicates effectively with patients, families, physicians, and healthcare providers about patient needs and goals. Ensures smooth operations and optimal use of rehabilitation services.
Patient Care and Supervision: Manages all aspects of patient care and education, including supervising assistants, aides, office staff, and students to ensure high-quality care.
