Job Description: Responsible for providing case management services to participants within the program and will act as an advocate for the participant providing resources, case management, transportation as well as working with participants on pre- and post-treatment planning. ROLES AND RESPONSIBILITIES: Maintains a relationship with all partnering agencies and referral sources. Efficiently document all billable interactions into OPUS, utilizing DAP note formatting. Coordinates with referral sources, involved family members, community agencies and other treatment providers to provide the best treatment and case management options for the individual participant's needs. Provides motivational interviewing, social, and emotional support to participants entering and engaging in program. Create crisis stabilization plans as needed for participants exploring treatment and coordinate assistance from peer supports to accomplish immediate/crisis goals. JOB SUMMARY: Under direct supervision of the Residential Peer Supervisor, the Food Service Coordinator will run the daily operations of the organization's kitchens in compliance with the True North Recovery mission and vision, Nutrition Program, USDA, local and state policies, procedures, rules and regulations. The Food Service Coordinator will be responsible for collaborating with residents to implement the high standards of nutrition, food production and food services. The Food Service Coordinator will be responsible the oversight and leadership of all kitchen staff. The Food Service Coordinator will work with the Residential Peer Supervisor to ensure that all records, policies, regulations, services, health, and safety factor procedures of the organization are being followed.
