Program Summary In the Adult Partial Hospitalization Treatment and Intensive Outpatient Treatment programs, we focus on creating a therapeutic environment where people can learn strategies for coping and improve their functioning. Each client's treatment plan is centered on group therapy which allows for sharing of experiences while gaining support and validation. When people need intensive, comprehensive treatment for psychiatric and chemical dependency issues but do not need inpatient or residential hospital care, Natchaug offers intensive, structured clinical treatment in programs that may include up to six hours of treatment a day, five days a week. These intensive treatment programs can help prevent the need for hospitalization. They can also help people make the transition to home after inpatient or residential treatment. Natchaug brings this clinical care closer to people's homes by offering programs throughout Eastern Connecticut. Job Summary Behavioral Health Clinician works in coordination with interdisciplinary teams to organize, prepare, and deliver evidence based clinical services in the Adult outpatient department. Clinical services include individual, group and family therapy. Key responsibilities of Behavioral Health Clinician: Conducts a comprehensive bio-psychosocial assessment and establishes a care plan, as appropriate. THE CARE MANAGEMENT MODEL: Our Care Management Triad Team Model is a collaboration between the following: RN Case Manager - manage patient care and drive patient progression and establish a discharge plan. Social Worker - resolve psycho-social barriers and supports discharge needs. Utilization Management - review patient status for appropriateness and anticipated payer coverage. EDUCATION AND EXPERIENCE REQUIREMENTS: Master's degree in Social Work (MSW) or Master of Social Services degree (MSS) from an accredited graduate school required prior to hire date. Licensed CSW or MSW as required by the state of Delaware prior to hire date. Acute Care Social Work experience preferred. SPECIAL REQUIREMENTS: CCM preferred within 18 months of eligibility. Hourly Pay Range: $30.34 - $48.55This pay rate/range represents ChristianaCare's good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
