Hours: Part-Time, 20 Hours/Week Shift: First Physical Therapist (PT), Part-Time, First Shift, UCGNI Pelvic Floor Therapist- Outpatient UC Health is hiring a part-time Physical Therapist (PT) for the Outpatient Therapy service's department for the first shift at UC Gardner Neuroscience Institute.
Evaluate, plan, and implement Physical Therapy treatment programs for referred patients in order to restore function, prevent disability, reduce pain, and return to his/her maximum function. Follow hospital department approved policies related to documentation, attendance, education, etc. Show respect and courtesy to all staff, patients, and visitors.
* Complete functional and comprehensive and physical therapy assessments.
* Develop individualized treatment programs consistent with patient's diagnosis, status, and goals.
* Implement treatment plans in collaboration with the therapy team.
* Engage in professional and population-appropriate communication.
* Adhere to hospital and department policies.
Minimum Required:
* Degree in Physical Therapy from approved PT School.
* License to practice Physical Therapy or licensure eligible in the applicable state.
* Recently certified, licensed or graduates within past 12 months.

Summary:

Provides treatment in accordance with physician's orders and established plan of care.
Evaluates effectiveness of treatment plan and revises plan of care as indicated.
Communicates/collaborates with patient, family, caregivers, significant other and other members of the healthcare team to promote maximum benefit of care.
Communicates/collaborates with the patient's referral source such as the physician, NCM, ADJ, etc. regarding the patient's care and progress to promote maximum benefit of care.
Assess educational needs of the patient, family, caregiver, or significant other and other members of healthcare team to promote maximum benefit of care.
Demonstrates appropriate knowledge and correct application of clinical techniques, procedures and programs for clinical setting and patient/client populations served.
Plans and makes appropriate discharge plans.
Assesses the risks for safety and implements appropriate precautions.
Complies with appropriate and approved security and safety standards.
Utilizes appropriate leadership skills in delegating, organizing, and educating coworkers and staff.

ACT is an interdisciplinary team-based approach to the provision of treatment for individuals with severe mental illness and often co- occurring substance use disorders, focusing on promoting symptom stability, increasing coping skills, social skill development and community integration. Services include wraparound case management to assist individuals with connection to community resources, skills training to help increase independent living, coordination of care with relevant supports, individual and family therapy, substance use counseling, medication management, psychoeducation, peer support, and vocational services.
Utilize key concepts of recovery including client choice, empowerment; hope, creating meaningful activity, advocacy, and consultation with key supports, to facilitate client centered care that will help individuals achieve their therapeutic goals.
