* Provide individual/family counseling, and social work intervention, as it relates to complex crisis management, mental illness, adjustment to illness, material assistance, healthcare decision-making, sexual assault, domestic violence, partner violence, elder abuse, competency, and any other psychosocial barriers in order to maximize the health status of patients.
* Advocate, mediate and negotiate to formulate a cohesive plan for maintaining or enhancing patient's health status, improving social supports, and moving patients safely across the continuum of care.
* Provide family education.
* Provide education about Advance Care Planning and assist with completing Advance Directives paperwork.
* Initiate referrals to community resources as indicated.
* Facilitate discharge process to home, nursing facility, rehabilitation facility, LTAC, group home, treatment or other facility.
Evaluation and Termination:
* Briefly summarize care plan with patient/family and respond to questions and concerns.

* Collaborate with co-workers and management to ensure the on-going development of the Social Work Department.
Competency:
* Participate in activities that support enhanced customer service and results in increased Press Ganey scores.
* Attend staff meetings and mandatory departmental in-service training sessions & continuing education in-services; maintain positive working relationships with and knowledge about community agencies and services.
* Maintain timely, clear and concise documentation; and write reports as needed.
* Respond to E-mail requests and provide information as requested.
* Participate in quality and safety initiatives.
Education: Master of Social Work from an accredited School of Social Work, minimum required.
License and Certification: Licensed by the State of Ohio Social Work Board (LSW, LISW, or LISW-S)
Years of experience: One-two Years equivalent experience, minimum required. Three-five years, preferred.
Required Skills and Knowledge:
Casework principles, including diagnosis, assessment, crisis intervention, and treatment planning; techniques in individual, family and group therapy; awareness of community resources, public assistance systems, Medicare/Medicaid; accepted professional and ethical standards within the existing medical community and professional organizations.

Behavioral Health Specialist, Community Case Manager

This position requires meeting clients in their homes, community, schools, etc. In addition, transporting clients in their own vehicle to/from appointments, school, community events, etc. Community Psychiatric Support Treatment (CPST) and Therapeutic Behavioral Services (TBS) provide services to children and adolescents with mental illness within all settings that meet the needs of the individual. These services may involve collateral contacts. CPST and TBS services provides an array of services delivered by community based providers. Services address the individualized mental health needs of the client which vary in hours, type, and intensity of services. CPST/TBS services are designed to provide specific, measurable, and individualized services to each client served.
Activities of the service shall consist of one or more of the following:
Ongoing assessment of needs;
Assistance in achieving personal independence in managing basic needs as identified by the individual and/or parent or guardian;
Facilitation of further development of daily living skills, if identified by the individual and/or parent or guardian;
Assistance with accessing natural support systems in the community; and linkages to formal community service/systems;
Symptom monitoring;
Coordination and/or assistance in crisis management and stabilization as needed;
Advocacy and outreach;

REPRESENTATIVE RESPONSIBILITIES

Provides therapeutic intervention to assigned caseload of patients based upon an Individualized Treatment Plan (ITP) developed with the families, at the frequency specified in the ITP, under the supervision of the Clinical Manager. Critical thinking skills are employed in implementing these therapeutic interventions, which are provided in the patients home, school and other natural environments in the community as appropriate. Transports patient and/or caregiver in personal automobile (after conducting a safety assessment) at times to provide individualized supportive services and to help ensure attendance at all appointments. Provides coaching and psychoeducation to families in a culturally competent manner, including safety plans as indicated. Documents accurately and timely the therapeutic interventions provided and response of patients/families/significant adults in the electronic medical record (Epic) per state requirements and established quality standards. At Cincinnati Children's, our clinical social workers understand that managing any type of illness or hospitalization is a stressful experience. As members on every medical team, the clinical social workers provide ongoing support and counseling, crisis intervention, advocacy, education, and connection to community resources.

The clinical social workers are all licensed professionals who provide specialized expertise on the medical teams. About Inpatient Psychiatry, located at the College Hill Campus
The Division of Child and Adolescent Psychiatry at Cincinnati Children's acute crisis units focus on stabilizing patients, most of whom stay an average of three to five days. In total, our hospital has 92 inpatient beds available to serve children and adolescents who are 17 years old and younger, who are experiencing a mental health crisis. The units are explained below:
* Adolescent Unit 4.200 (ages 12-17): Patients with a mental/behavioral disorder
* Youth Unit 5.400 (ages 8-17) Patients with both a mental/behavioral diagnosis and developmental disability
* Young Child Unit 5.200 (ages 2-8) Patients who have a mental/behavioral health diagnosis
* Youth with chronic medical conditions in addition to mental health diagnosis
Youth admitted for inpatient treatment can participate in various therapeutic and medical treatment programs depending on their personal needs. Their stay can include group therapy, music therapy, occupational therapy, speech pathology, and chemical dependency education.

Job Responsibilities

Support patients with a variety of severe psychological/psychiatric difficulties, including mood disorders, disruptive/aggressive behavior, self-harm, anxiety, psychosis, eating disorders, intellectual disability and neurodevelopmental disorders who often have histories including significant psychosocial stressors and trauma. Proactively prevent and respond to crises using Therapeutic Crisis Intervention (TCI) strategies, including verbal de-escalation and physical interventions when necessary due to imminent risk. Lead and intervene in behavioral crises, apply appropriate strategies based on the Stress Model of Crisis and individualized treatment plans. Conduct safety checks, room searches, and patient safety searches. Follow policies and collaborate with the interdisciplinary team.
* Plan of Care: Implement individualized care plans, including behavioral interventions and support for activities of daily living (bathing, feeding, toileting). Provide care primarily in group settings, with individualized programming as needed per treatment team recommendations. Maintain designated observation status (e.g., continuous observation, 15-minute checks, remaining within arm's-length of patient).
* Group Programming: Lead structured group programming and facilitate patient engagement; adjust delivery to meet developmental needs of patients, observe behavior and participate in problem-solving
* Therapeutic Relationships: Build rapport while maintaining professional boundaries. Recognize trauma impacts and use developmentally appropriate therapeutic communication. Contribute to team planning and patient-centered care discussions.
