The clinical pharmacist provides comprehensive medication management services that integrate clinical services, order management, patient/staff education and research. Clinical pharmacists are responsible for improving the patient's experience, safety, and achieving positive clinical and financial outcomes. Provides medication management services in a collaborative manner that consist of the following:
Review patient medication history, educate patients on drug therapy, establish desired outcomes, develop/modify therapeutic plans, determine alternatives for achieving outcomes, develop drug monitoring plans, monitor patient outcomes, document medication related issues in electronic health record, interpret laboratory data, review patient medication profiles, monitor pharmacokinetics of drug therapy, monitoring for potential adverse drug - drug, drug-lab, drug-disease state, and drug - food interactions.
Maintains and updates a patient profile with demographics (e.g. height and weight), diagnosis, allergies, and current medications.
Monitors drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interaction.
Operations/Distribution Performance
Ensures the timely and accurate dispensing of medications from the pharmacy area.
Ensures proper technique and accurate preparation of all pharmaceutical products, including oral, non-sterile and sterile compounding, chemotherapeutic, hazardous, and investigational agents.
Responsible for the accurate filling and dispensing of medications from the pharmacy area.
Cross-covers in a variety of practice areas within the pharmacy as requested by supervisor.
Financial Performance
Promotes the use of formulary drugs by converting non-formulary orders to formulary drugs when possible according to P&T protocol. Coordinates procurement of non-formulary drugs when necessary.
Conducts targeted drug program and drug usage evaluations as needed and reports results to Department Managers/ Pharmacy and Therapeutics Committee.
Identifies and acts upon cost avoidance in drug therapy by promoting cost effective regimens, drugs, and by reducing waste.
Safety and Regulatory Performance
Detects, monitors, documents, and reports adverse drug reactions and medication errors.
Utilizes drug use reports to identify adverse drug events.
Participates in Joint Commission and other accreditation and regulatory activities such as National Patient Safety Goals and Department of Pharmacy Quality Improvement Plan.
Research and Education Performance
Precepts Intermediate Pharmacy Practice Experience (IPPE) rotation students, and Advanced Pharmacy Practice Experience (APPE) rotation students, and pharmacy residents consistent with academic credentials and clinical experience.
Participates in continuing education programs, meetings, training programs and related activities.
Prepares educational materials and participates in information sharing activities for the health care community and general public.
Position Qualifications:License/Certification/Education:Required:
Doctor of Pharmacy (Pharm.D) degree;
OR
Bachelor of Science in Pharmacy (BSPharm) degree with 3 years hospital experience.
Licensure with the Massachusetts Board of Registration in Pharmacy.
Preferred:

Job Summary
The MGBMG Pharmacy Department aims to expand clinical pharmacy services across the Mass General Brigham Medical Group (MGBMG) locations in a way that enhances the patient experience, health outcomes, and quality. One of the key goals is ensuring that our patients with chronic diseases are supported in achieving clinical outcomes known to reduce morbidity and mortality from these conditions. Pharmacists can play a key role in this goal by offering higher touch services (medication prescribing, monitoring, education) to patients that are struggling with chronic disease management. Working with primary care providers, population health coordinators, high-risk nursing programs, and community health workers, the pharmacist has the potential to positively impact patients through virtual, in-person appointments, and telemedicine.
In collaboration with the MGBMG team, the Ambulatory Clinical Pharmacist will monitor the medication use process in accordance with the laws and regulations governing credentialed pharmacists and the practice of pharmacy by the appropriate state and federal agencies. They will work relatively independently, and within the limits of established pharmacy/ambulatory care practice site policies and procedures, as a member of the patient care team. The Ambulatory Clinical Pharmacist will report to the Clinical Pharmacy Manager at MGBMG and be responsible for working at the top of their licensure in collaborative drug therapy management (CDTM) work, as well as in-service presentations, clinical consult work, collaboration with our pharmacy team, and development of work to assist in the expansion of our department.

Minimum Education:

Position Summary:

* Verifies all aspects of medication order & follows AMAR policy.

* In response to new information or specific requests, educates medical, nursing, and pharmacy staff in a timely and accurate manner.

* Knowledge of specific interactions affecting All in One TPN's and completion of Performance Improvement monitor for Parenteral Nutrition.
* Knowledge of Pyxis MedStation and Pharmacy console: demonstrates ability to load/unload medications, troubleshoot common problems (drawer failure, controlled substance discrepancy, etc.), troubleshoot Pyxis/Meditech issues, and reviews "meds ordered not loaded" reports for prompt attention.

He/she is responsible for coordinating and delivering pharmacy services in compliance with all applicable policies, regulatory agency requirements and state and federal laws. Job-specific Responsibilities: - Reviews and monitors patient treatment (chemotherapy and ancillary) medications for appropriateness based on current clinical and institutional guidelines as well as patient-specific criteria. - Monitors patients' medication regimens for drugs interactions, toxicities, and cost-effectiveness. - Assesses all medication orders to ensure they meet established standards and practice guidelines of MGH Cancer Center. Reports order discrepancies to appropriate staff promptly. - Advises clinicians on drug selection based on current clinical guidelines to optimize drug therapy. - Oversees the safe handling and preparation of various medications, including IV chemotherapy, IV antiemetics, IV steroids, and investigational chemotherapy drugs for outpatient and inpatient use. - Checks admixtures for accurate preparation, including the additive(s), parent solution, and label. - In conjunction with the Senior Attending Technician, oversees technician activities and performs quality control checks. - Accesses current clinical trial documents when reviewing clinical trial orders to ensure accuracy. - Maintains investigational medication logs. - Maintains current patient information files and records in the pharmacy department to maintain and support safe and consistent drug administration. - Adheres to safety requirements in the handling and preparation of chemotherapy and other hazardous agents according to current guidelines (USP 797, ASHP, NIOSH). - Oversees the ordering, storage and handling of medications and supplies. - Participates in education activities such as inservices. - Answers drug information questions. - Participates in the Safety Reporting System. This position offers a rare and exciting opportunity to contribute to the strategic development and implementation of new clinical pharmacy services within a primary care setting! Job Summary
This is a full-time, overnight pharmacist position (schedule structure is seven consecutive days staffing, followed by seven consecutive days off) based out of central pharmacy. This pharmacist will provide both clinical and operational services for MGH acute care patients. This individual will interface with both the clinical pharmacy team and the operations team.
Clinical Pharmacists assume responsibility and accountability for the provision of safe, effective, and prompt medication therapy. Clinical Pharmacists proficiently provide direct patient-centered care and integrated pharmacy operational services with physicians, nurses, and other hospital personnel to deliver optimal medication therapy to patients with a broad range of disease states. These clinicians are aligned with target interdisciplinary programs and specialty services to deliver medication therapy management and to ensure pharmaceutical care programs are appropriately integrated throughout the institution. Pharmaceutical care services include, but are not limited to, assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. Clinical Pharmacists serve as departmental resources and liaisons to other departments, hospital personnel, or external groups. They proficiently manage the appropriate use of medications according to professional and regulatory standards. They also conduct practice-related research and patient care, quality, and regulatory compliance initiatives designed to improve medication-use processes or pharmacy practice. Required Qualifications
* Must be eligible for licensure to practice pharmacy in the Commonwealth of Massachusetts
* Board Certification by examination, or equivalent certification, in the relevant specialty is preferred upon hire, and required within 18 months of hiring
* Must meet all Board of Pharmacy requirements to enter into a collaborative drug therapy management agreement, when applicable
* Massachusetts Controlled Substance Registration license, National Provider Index number, and a Drug Enforcement Administration Registration number, when applicable
Preferred Qualifications
* Postgraduate residency/fellowship training is preferred
* Completion of ASHP-accredited PGY1 with critical care experience or a PGY2 Critical Care Pharmacy Residency preferred

* Extensive knowledge and ability in all areas of the department including: IV Admixtures, Drug Information, Ambulatory, Manufacturing, and Drug Distribution.

* Familiarity with pharmacy practice settings, legal requirements and limitations, prescription information and calculations, drug identification and generic equivalents, drug manufacturing, packaging and labeling information, and proper drug handling and storage practices.
* As it pertains to the service area in the pharmacy department, demonstrate appropriate knowledge of principles of growth and development over the life span of the neonate, adolescent, adult, and geriatric patient.
* Possess ability to assess patient data relative to age specific needs.

The clinical pharmacist provides comprehensive medication management services that integrate clinical services, order management, patient/staff education and research. Clinical pharmacists are responsible for improving the patient's experience, safety, and achieving positive clinical and financial outcomes. This position is responsible to an immediate supervisor/manager.
Major Responsibilities:
The following key performance areas (Clinical, Operations/Distribution, Financial, Safety and Regulatory, Research and Education) are performed by the clinical pharmacist:
Clinical Performance
Provides medication management services in a collaborative manner that consist of the following:
Review patient medication history, educate patients on drug therapy, establish desired outcomes, develop/modify therapeutic plans, determine alternatives for achieving outcomes, develop drug monitoring plans, monitor patient outcomes, document medication related issues in electronic health record, interpret laboratory data, review patient medication profiles, monitor pharmacokinetics of drug therapy, monitoring for potential adverse drug - drug, drug-lab, drug-disease state, and drug - food interactions.
Maintains and updates a patient profile with demographics (e.g. height and weight), diagnosis, allergies, and current medications.
Monitors drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interaction.
Provides drug information to health care professionals and to patients.
Documents clinical interventions.
Counsels patients on discharge medications as appropriate.
Performs clinical review and verification of medication orders.
Performs medication order entry when needed.
Discusses medication order clarifications with the prescriber, documents interventions, and communicates medication order changes as needed.
Facilitates medication discharge planning and smooth transition to alternate sites of care and access to an ambulatory pharmacy.
Operations/Distribution Performance
Ensures the timely and accurate dispensing of medications from the pharmacy area.
Ensures proper technique and accurate preparation of all pharmaceutical products, including oral, non-sterile and sterile compounding, chemotherapeutic, hazardous, and investigational agents.
Responsible for the accurate filling and dispensing of medications from the pharmacy area.
Cross-covers in a variety of practice areas within the pharmacy as requested by supervisor.
Ensures regulatory compliance of controlled substance distribution and monitoring.
Supervise work of pharmacy technicians.
Provide feedback to maintain and optimize pharmacy technology.
Financial Performance
Promotes the use of formulary drugs by converting non-formulary orders to formulary drugs when possible according to P&T protocol. Coordinates procurement of non-formulary drugs when necessary.
Conducts targeted drug program and drug usage evaluations as needed and reports results to Department Managers/ Pharmacy and Therapeutics Committee.
Identifies and acts upon cost avoidance in drug therapy by promoting cost effective regimens, drugs, and by reducing waste.
Facilitates cost saving programs such as IV to PO and therapeutic substitution.
Manages medication inventory, expiring products, and recalls.
Safety and Regulatory Performance
Detects, monitors, documents, and reports adverse drug reactions and medication errors.
Utilizes drug use reports to identify adverse drug events.
Participates in Joint Commission and other accreditation and regulatory activities such as National Patient Safety Goals and Department of Pharmacy Quality Improvement Plan.
Research and Education Performance
Precepts Intermediate Pharmacy Practice Experience (IPPE) rotation students, and Advanced Pharmacy Practice Experience (APPE) rotation students, and pharmacy residents consistent with academic credentials and clinical experience.
Participates in continuing education programs, meetings, training programs and related activities.
Prepares educational materials and participates in information sharing activities for the health care community and general public.

Doctor of Pharmacy (Pharm.D) degree; OR Bachelor of Science in Pharmacy (BSPharm) degree with 3 years hospital experience.
Licensure with the Massachusetts Board of Registration in Pharmacy.
