Provide individual & group therapy to patients with varied mental health/ co-occurring disorders. 2. Responsibilities include completing initial assessments, counseling, group therapy, case presentations to the treatment team, treatment planning, and aftercare planning. 3. Clinicians are also responsible for identifying the need for medication evaluations and making referrals to the on-site providers. 4. Outpatient Substance Use Treatment is a growing specialty service within the Adult Outpatient services at Community Healthlink. Services include outpatient treatment, medication-assisted treatment, the MISSION medication-assisted treatment program, and second offender aftercare. Staff include clinicians, recovery coaches, nurses, advanced practice nurses, and psychiatrists. The Community Healthlink Adult Outpatient Counseling Centers are integrated primary and behavioral health care clinics that provide services to a diverse adult client population. Our priority population is individuals with severe and/or persistent mental health and/or substance use conditions. The provider team consists of psychiatrists, psychologists, nurses, advanced practice registered nurses, social workers, mental health counselors, recovery coaches, registered nurses, primary care clinicians, and medical assistants. The Centers work closely with referral sources such as the Department of Mental Health, local hospitals with inpatient psychiatric programs, Community Healthlink programs, MassHealth/managed care companies, and health centers. Essential Functions * Collaborate with interdisciplinary care teams (ICT)-including LTSC, GSSC, primary care providers, specialists, and behavioral health treatment facilities-to support program enhancements, process improvements, care coordination, discharge planning, and aftercare. * Participate actively in ICT meetings, contributing insights on behavioral health concerns, treatment options, care strategies, and engaging in collaborative decision-making regarding enrollees' care. * Receive and triage behavioral health and substance use referrals, coordinating with the ICT and primary care providers to refer enrollees to appropriate inpatient or outpatient services. * Develop, update, and implement enrollee-centered care plans in collaboration with enrollees and care teams, promoting self-care, shared decision-making, and addressing identified needs through routine engagement and proactive outreach. * Conduct outreach, assessments, and home visits via telephonic, electronic, or in-person contact for identified enrollees, including routine evaluations and post-transition care. * Monitor clinical status to identify early signs of deterioration, intervening proactively with the ICT to prevent unnecessary hospitalizations and escalate behavioral health needs during weekly case conferences. * Establish consistent communication and reporting schedules with providers and enrollees to review enrollee status, progress toward goals, care needs, utilization plans, and follow-up. * Ensure compliance with DSNP regulations and quality assurance standards by documenting, monitoring, and evaluating enrollee interventions and care plans accurately within electronic medical record systems. * Serve as a key resource and liaison within the ICT by addressing enrollee questions, concerns, and care needs, facilitating access to services, and performing additional duties as assigned by the supervisor. Conducts diagnostic evaluations, writes behavioral treatment plans, provides individual counseling, writes aftercare plans in accordance with department quality and productivity standards. Participates in counseling and patient care as a member of a multi-disciplinary team. I. Major Responsibilities: 1. Conducts eligibility screenings and comprehensive intake and follow up assessments to assess eligibility, gather clinical information, and inform treatment planning. 2. Conducts client evaluations including bio-psychosocial assessment, diagnostic formulation, and initial treatment plan recommendations. 3. Meets with clients to provide individual counseling and assist clients in attaining treatment goals. 4. Writes treatment plans including client's strengths with behaviorally based goals and objectives. Documents service delivery in accordance with department standards and progress of goals and objectives in progress notes. Refers clients to resources with and outside CHL 5. Participates in multi-disciplinary team meetings and case discussions. Recognize and seek consultation as needed. Develop a training plan to enhance professional growth. 6. Participates in utilization management, employs Commission on the Accreditation of Rehabilitation Facilities (CARF) standards, and assists as needed in re-accreditation processes. 7. Builds working relationships with agencies and organizations in support of clients' recovery to facilitate referrals and care coordination. 8. Works closely with MMAT clinical team (Recovery Coach, Nurse, and Psychiatrist) to coordinate client services. Standard Staffing Level Responsibilities: 1. Complies with established departmental policies, procedures, and objectives. 2. Attends variety of meetings, conferences, seminars as required or directed. 3. Demonstrates use of Quality Improvement in daily operations. 4. Complies with all health and safety regulations and requirements. 5. Position Qualifications: License/Certification/Education: Required: Grade C12- Clinician III: Master's degree in a clinical course of study in behavioral health (social work, psychology, marriage and family therapy, counseling or related field) that meet the academic requirements for independent licensure in psychology, social work, marriage and family therapy, or mental health counseling in the Commonwealth of Massachusetts. Requires licensure as LMHC, LCSW, LMFT, or is actively working towards licensure in the Commonwealth of Massachusetts. Grade C16- Clinician IV: Master's degree in a clinical course of study (social work, psychology, counseling or related field) and LICSW licensure or licensed Psychologist. Preferred: 1. Bilingual preferred. Experience/Skills: Required: 1. Ability to perform diagnostic and risk assessments using current accepted tools and practices to determine appropriate level of care. 2. Ability to develop and implement treatment plans which are behaviorally oriented, measurable, and appropriate for the identified level of care. 3. Ability and interest to learn and implement evidence-based models of treatment for opioid addiction. Preferred: 1. A minimum of one year of experience in behavioral health or addiction is preferred. 2. Clinical expertise specific to adults with substance use conditions and co-occurring severe and persistent mental illness with an ability to engage clients effectively and work with other professionals and organizations. Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements. Department-specific competencies and their measurements will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents. III. Physical Demands and Environmental Conditions: Work is considered sedentary. Position requires work indoors in patient care and office environments.
