The PhysicalTherapist II demonstrates strong clinical knowledge and skills in the evaluation, treatment planning and care of patients with physical dysfunction. This advanced clinician participates in activities that support self-development as well as the development of the department and others within the department, UMMHC and/ or the community. Performs advanced clinical and non-clinical duties including treating complex cases, educating staff and assisting with department leadership.
I. Major Responsibilities:
1. Administers Physical Therapy for physically, developmentally and/or functionally disabled patient populations, including infants and children, adolescents, adults, geriatrics and frail elderly, inpatient and outpatient as assigned.
2. Examines and evaluates patient's physical therapy needs. Formulates a physical therapy diagnosis and develops a treatment plan with treatment goals directed towards maximizing function. Completes documentation that evidences a high degree of clinical knowledge, attention to individual patient complexities and needs and modifications to treatment plan to assure best functional outcome.
3. Delegates caseload to therapist assistants, overseeing quality of care and supervising performance. Collaborates with team members to assure patient coverage and equitable work distribution.
4. Meets and often exceeds department standards for productivity, timely completion of clinical and non-clinical documentation. Attends to details in meeting timeframes for patient care in relation to patient's need and overall plan for hospitalization or insurance rules and authorization procedures. Demonstrates high level of commitment to patients and position through excellent attendance, efforts to improve processes, and willingness to extend beyond normal work hours when necessary to meet patient or department needs.
5. Acts as a clinical resource for a designated clinical population or functional unit. Researches clinical protocols in specialty practice area to evaluate outcomes, assure evidence based practice and up-to-date procedures. Presents results of research to clinical staff and oversees staff training when new protocols are implemented.
6. Educates patient, family and other caregivers in therapy program and goals. Participates in patient care meetings, collaborates with team members, and assists with discharge planning.
7. Consults with other members of the health care team to ensure coordination and effectiveness of therapies provided. May act as resource to medical team via consultation in clinic, participation in patient care rounds or provision of in-service education to non-rehabilitation departments to promote awareness of Physical Therapy.
8. Is active in orientation of new staff and supervision of students and aides. Accepts a minimum of one preceptor assignment or clinical affiliate / year. Participates regularly in specific orientation of new staff in area of clinical expertise.
9. Participates in implementation of quality assurance program for clinical service, including routine chart audits to assure compliance to protocols, review of staff documentation and clinical activities to mentor improved performance and / or improve clinical or business processes.
10. Assists in implementation of triaging, prioritizing, monitoring admissions, referrals, and discharges to and from the service as assigned.
11. Develops own clinical knowledge and skills through participation in professional organizations, attending continuing education programs, seeking advanced certification, peer review and other related internal and external resources.
12. Participates in special interest group or committee within the profession, department or hospital in order to promote staff education and program development. May present at community, state or national conferences to represent Physical Therapy.
13. Assists in proper care and distribution of department supplies and equipment, making recommendations for adequate inventory and appropriate and new equipment.
14. Volunteers for a minimum of one community service activity per year, and may lead divisional activities to support UMMHC community service commitments.
15. In the absence of the clinical supervisor, assumes duties and responsibilities as assigned by the Director.
16. Participates in departmental meetings, which help to formulate policies, procedures and program development.

II. Position Qualifications:

Required:
1. Graduate of an accredited physical therapy program and current unrestricted license to practice physical therapy from the Commonwealth of Massachusetts.

1. Must have a minimum of 2 years of clinical experience and a performance record that consistently exceeds expectations.
2. Effective verbal and written communication skill; demonstrated advanced clinical knowledge and ability to instruct / mentor others.
3. Excellent customer service skills and ability to work cooperatively with all members of the healthcare team.
Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.

ESSENTIAL RESPONSIBILITIES / DUTIES:
Stress Testing Management
Knowledge of ACSM stress testing guidelines
Evaluates patients for appropriateness of testing, patient safety, and correct type of test and protocol
Effectively communicates pertinent information with supervising physicians, nurses, sonographers and other physiologists and ensures communication with referring physicians as needed.
Evaluates and performs stress testing of outpatients and inpatients; including, exercise tolerance tests, stress echocardiograms, dobutamine echocardiograms, metabolic stress tests, exercise nuclear stress tests, pharmacological nuclear stress tests.
Performs Pediatric stress testing. Exhibits knowledge of testing protocols and proper equipment usage for pediatric patients.
Provides patient information, explains testing methods and procedures, and obtains patient consent
Selects appropriate testing protocol according to patient clinical presentation
Ensures recordings are of the best quality for diagnosis according to departmental practices and procedures.
Performs ongoing clinical assessment throughout the test by monitoring cardiovascular and pulmonary parameters including symptoms, appearance, heart rate, blood pressure, and electrocardiogram
Determines test duration and test termination as per BMC Stress Testing Lab Policy and ACSM guidelines
Calculates and records all necessary parameters, and provides initial interpretation of electrocardiographic tracings for diagnostic purposes
Creates the electronic record of exam, documentation of pertinent exam information, and data entry for charge capture
Evaluates and communicates findings to medical personnel as required, following departmental practices and procedures
Responds to medical emergencies for Stress Testing Labs and BMC Non Invasive Cardiovascular Laboratories, and assists-physicians according to BMC Non-Invasive Lab policy.
Provides education and instruction to hospital personnel and students in methods and theories of stress testing
Inserts peripheral intravenous lines as needed according to BMC Non Invasive Lab Policy
Nuclear Stress Testing (specific additional duties to those as outlined above)
Coordinates patient-specific imaging protocol with physician staff (Cardiology and/or Radiology)
Maintain Radiation safety standards per BMC Radiation policy and procedure guidelines
Enters basic pre-test patient data and imaging data into Apollo (medical history, radiopharmaceutical doses and administration time, etc.)

Provides patient orientation to the Rehab Programs

Assists in the scheduling of ancillary patient needs: interpreters, transportation, parking

Collaborates and communicates with referring providers, including the provision of a written discharge summary
Documents and reports exercise sessions, manages appointment scheduling, and initiates and ensures charge capture processes
Maintains Cardiac Rehab Program standards to meet American Association of Cardiovascular and Pulmonary Rehabilitation (AACVPR) accreditation requirements
Develops individual student programs including, supervision, instruction, and training all student interns

Has a working knowledge of 12-lead EKGs and rhythm strip interpretation
Communicates/collaborates with members of multidisciplinary team (nursing, physicians, sonographers, local health centers, dieticians, behavioral medicine, respiratory therapists)
Maintains necessary records and files according to departmental practices and procedures.
Orders equipment and supplies as needed.

Monitors Daily Quality Control including: glucometer, electronic refrigerator indicators, and defibrillator

The Inpatient Rehabilitation Services Department at Brigham and Women's Hospital provides physical therapy, occupational therapy, speech language pathology, and orthopedic technician services to patients on a consult basis to our inpatient floors, ICUs, and the Emergency Department. Senior PhysicalTherapists must have at least three years of clinical experience with strong clinical knowledge and skills. They will be joining a team with diverse clinical backgrounds and expertise. Therapists should have an unrestricted license to practice physical therapy in the Commonwealth of Massachusetts.
Their primary responsibility is to provide patient care in accordance with State licensing regulations, departmental standards and policies, and accepted professional standards of care. Therapists will treat adult patients with a variety of diagnoses across a range of acuity.
The Brigham offers physicaltherapists the opportunity to rotate to seven teams: Orthopedics/Trauma, Cardiovascular, Thoracic/Pulmonary, Oncology, Neurology, General Medicine, and the Emergency Department. Additionally, we are looking for clinicians who will be able to treat patients with a variety of different backgrounds and identities.
In addition to providing exemplary patient care, Senior physicaltherapists serve as mentors to other staff in the department. Alongside Clinical Specialists, Senior therapists collaborate regularly with other rehabilitation disciplines and the multidisciplinary team. They will assist in staff education and training, triaging patient consults, program development, quality initiatives, and optimizing departmental policies and procedures. Senior therapists will also have opportunities for clinical growth through internal continuing education programming, financial support for external continuing education courses and conferences, and access to a formal mentorship program.
Senior therapists can also participate in a variety of Rehabilitation Department Committees to enhance their professional growth in addition to their clinical care. These committees include: The Community Resource Committee, Practice Standards Committee, Staff Development Committee, Rehab Recognition Committee, Research Committee, and Diversity, Equity, and Inclusion Committee. PRINCIPAL DUTIES AND RESPONSIBILITIES:
* Examines and evaluates patient's physical therapy needs. Formulates a physical therapy diagnosis and develops a treatment plan with treatment goals directed towards maximizing function.
* Demonstrates competency in all clinical knowledge and skills specific to the patient case type(s) in assigned area of care.
* Acts as a clinical or non-clinical resource.
* Consults with other members of the health care team to ensure coordination and effectiveness of therapies provided.
* Is assigned additional tasks and responsibilities which may include, but not be limited to: orientation of new staff, participation on committees, involvement in QI activities, supervision of students and aides, and involvement in developing/revising practice standards.
* Participates in implementation of quality assurance program for clinical service.
* Assists in implementation of triaging, prioritizing, monitoring admissions, referrals, and discharges to and from the service as assigned.
* Participates in special interest group or committee within the department in order to promote staff education and program development.
* Assists in proper care and distribution of department supplies and equipment, making recommendations for adequate inventory and appropriate and new equipment.
* In the absence of the clinical supervisor, assumes duties and responsibilities as assigned by the Director.
* Participates in departmental meetings, which help to formulate policies, procedures and program development.

Qualifications
QUALIFICATIONS
* Must be a graduate of an accredited physical therapy program.
* Must possess a current unrestricted license to practice physical therapy from the Commonwealth of Massachusetts.
* Must have a minimum of 4 years of clinical experience and a performance record that consistently exceeds expectations.
SKILLS/ ABILITIES/ COMPETENCIES REQUIRED:
* Must possess communication and interpersonal skills sufficient for effective leadership and training of assigned personnel and working relationships within the department and assigned unit, in addition to those skills required of a staff therapist (up to 90% of work time).
* Must possess analytical abilities and sound knowledge in Physical Therapy to act as an education and clinical resource, in order to assist with development and implementation of programs, policies, and procedures and for orientation and training of new staff therapist (up to 90% of work time).
* Work requires up to three months orientation to acquire necessary familiarity with department and hospital policies and procedures, and the specific quality standards of care and procedures unique to the assigned patient area.
* Must possess effective communication skills (written and verbal) to interact with patients (up to 70% of work time), rehabilitation and medical team (up to 20% of work time), and patients' families (up to 10% of work time).
* Work requires prolonged walking and standing (up to 95% of work time); lifting, transferring, and positioning of patients and equipment (10% - 50% of work time), and the transportation of patients (up to 10% of work time).
* Work with critically ill patients (up to 10% of work time) produces a high level of stress and mental fatigue.
* Must be able to work with newborns (up to 10% of the work time) to the oldest old (up to 10% of the work time). The majority of patient care (70% of work time) is the 18- to 65-year-olds.
* The individual in this position must demonstrate appropriate knowledge of the principles of growth and development, which would include neonates to the oldest old. In addition, the therapist must demonstrate competency in assessment and treatment relative to age specific needs for the patient population for which they provide therapy, and as described in the department's practice standards.
WORKING CONDITIONS:
* Works in hospital environments where there is some exposure to blood/body fluids/hazardous materials, communicable diseases (10% - 25% of work time) and unpleasant odors (up to 25% of work time).
* Work requiring physical effort for lifting, transferring, and positioning of patients and/or equipment (up to 50% of work time) may result in musculoskeletal strain or injury with improper body mechanics.
* Work requires use of sharp and/or electrical equipment (up to 20%), which could result in, cuts or burns with improper safety.
SUPERVISORY RESPONSIBILITIES:
Assists with supervision and education of staff therapists, assistants, and aides.
FISCAL RESPONSIBILITIES:
* Patient charges are entered in an accurate and timely manner as appropriately supported by the medical documentation.
* Hospital and department equipment is managed in an appropriate and cost-efficient manner.
HOSPITAL WIDE RESPONSIBILITIES:
Works within legal, regulatory, accreditation and ethical practice standards relevant to the position and as established by BWH/Partners; follows safe practices required for the position; complies with appropriate BWH and Partners policies and procedures; fulfills any training required by BWH and/or Partners, as appropriate; brings potential matters of non-compliance to the attention of the supervisor or other appropriate hospital staff.

Under the general supervision of the Associate Director of Care Innovation/PhysicalTherapist, the ALS/MND Clinical Research PhysicalTherapist is responsible for clinical care of patients with ALS, SMA, and other neuromuscular diseases, as well as actively participating in ALS and other neuromuscular clinical research, within the scope of their license in Massachusetts. The ALS/MND Clinical Research PhysicalTherapist is familiar with Motor Neuron Diseases and Clinical Research within this population and is a member of the outpatient multidisciplinary clinical care and clinical research teams at the Sean Healey and AMG ALS Center ALS Clinic, MGH Adult SMA clinic, and the Neurological Clinical Research Institute.
Qualifications
CLINICAL RESPONSIBILITIES:
* Provide competent, compassionate physical therapy care based on scientific principles, physical therapy knowledge/education, and clinical judgement
* Provide Physical Therapy assessment, consultation, recommendations, care coordination, PT treatment, and education to patients with ALS and other neuromuscular/neurological diseases, and their families/caregivers
* Identify, facilitate, and evaluate the outcomes of physical therapy care for individual patients or groups of patients.
* Coordinate involvement of the patient, family/caregivers, and health team members in patient care, including patient/family education, and provide and recommend resources (verbal, demonstrative, written, community) for patients and their families
* Order, in collaboration with MDs/APPs, appropriate equipment and services for patients.
* Provide patient follow-up (including phone calls, email, and paperwork) between in-person clinical visits, including communication with patients, caregivers, and internal/external healthcare providers (PT, OT, as well as MD/NP/RN), including community outreach to outside organizations
* Participate in Virtual Visits with providers (MD/NP) as needed
* Document relevant patient information
* May require occasional visits to patient homes for clinical care or travel to clinical meetings
* Appropriately utilize physical therapy and other clinical resources to provide care
* Train and mentor other clinical staff, including PTs, as appropriate
* Participation in, and coordination of, clinical and department projects as appropriate,

RESEARCH RESPONSIBILITIES:
* Perform outcome measures as outlined in the protocol. This may include any appropriate outcome measure, including but not limited to strength and other physical testing, Vital Capacity Testing, ALS Functional Rating Scale Questionnaire, Interview Questionnaires, gait and balance measures, and other tests and measures appropriate to the role
* Act as primary outcomes trainer for NCRI and MND clinic staff for outcomes measures for clinical trials and for the ALS clinic, and monitor the overall outcome process
* Act as a resource for study staff regarding outcomes and other appropriate study visit and study-related questions per GCP training, PT clinical knowledge, and outcomes measures expertise
* Coordinate and implement research studies according to the study protocols
* Collaborate with PIs and study teams on new studies to ensure timely start-up, and be available to assist the study team with the coordination of services needed to implement the study, as well as be available to assess study progress and problem-solve as needed
* Assess patient participation on a continuous basis in light of ethical considerations. Act as a liaison between the patient/participant and other members of the team regarding patient care activities related to the protocol.
* Work with physicians, nurses, research coordinators, and patients in the selection of and participation by appropriate patients/participants
* Develop, implement, and support systems to promote efficiency, productivity, and operating systems to economize resources
* Facilitate integration of regulatory guidelines to ensure safe practice
* Collaborate with the study team in the development of new research studies, including the use of outcome measures and data form development, as well as participate in PT or rehab-based studies
* Present study findings to interested groups both inside and outside of the hospital, including but not limited to presentations, posters, and collaboration on papers
* May require travel to research study meetings and/or research study participants' homes for study visits.
* May also be involved in protocol development and implementation, data compilation and analysis, and presentation/publication of data.
* May be involved in obtaining study approvals and informed consent; screening and enrolling patients into the studies as appropriate. With assistance from research coordinators, may be responsible for IRB submissions for protocols

GENERAL SUMMARY/ OVERVIEW STATEMENT:
Inpatient Rehabilitation Services at Brigham and Women's Hospital provides physical therapy, occupational therapy, speech and swallow services, and orthopedic technology services to patients on a consult basis to those on our inpatient floors, ICUs, and the Emergency Department.
Brigham and Women's Hospital is a nationally ranked academic medical center located in the Longwood Medical Area of Boston, Massachusetts. Staff physicaltherapists will be joining a team with a diverse background in clinical care and expertise.
The primary responsibility of the staff physicaltherapist is to provide patient care in accordance to State licensing regulations, departmental standards, and policies, and accepted professional standards of care.
The Brigham offers physicaltherapists the opportunity to rotate to seven teams with diverse diagnoses across multiple adult populations, including Orthopedics/Trauma, Cardiovascular, Thoracic/Pulmonary, Oncology, Neurology, General Medicine, and the Emergency Department.
Staff therapists will receive mentorship from experienced clinicians, many of whom are Board Certified Clinical Specialists. In addition to regular collaboration with other rehab disciplines as well as the multidisciplinary team, therapists will have many opportunities for clinical growth through regular internal continuing education programming offered, financial support for continuing education courses and conferences, and access to a formal mentorship program.

Primary Assigned Clinical Area:
Inpatient Acute Care and Dana Farber Inpatient Hospital within BWH Rotation through weekend coverage is expected in this role.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
1. Examines and evaluates patient's physical therapy needs. In collaboration with the patient and/or caregiver, formulates a physical therapy diagnosis, and develops a treatment plan with treatment goals directed towards maximizing function.
2. Demonstrates competency in all clinical knowledge and skills specific to the patient case type(s) in assigned area of care.
3. Consults with other members of the health care team to ensure coordination and effectiveness of therapies provided.
4. Assists in implementation of triaging, prioritizing, monitoring admissions, referrals, and discharges to and from the service as assigned.

QUALIFICATIONS
1. Must be a graduate of an accredited physical therapy program.
2. Must possess a current unrestricted license to practice physical therapy from the Commonwealth of Massachusetts.
SKILLS/ ABILITIES/ COMPETENCIES REQUIRED:
1. Must possess communication and interpersonal skills sufficient to manage a diverse patient population of varying diagnosis and case types.
2. Must possess analytical abilities and sound knowledge in Physical Therapy to effectively manage a varied caseload demonstrating sound clinical decision-making and judgment (up to 90% of work time).
3. Work requires up to three months orientation to acquire necessary familiarity with department and hospital policies and procedures, and the specific quality standards of care and competencies, and procedures unique to the assigned patient area.
4. Must possess effective communication skills (written and verbal) to interact with patients (up to 70% of work time), rehabilitation and medical team (up to 20% of work time), and patients' families (up to 10% of work time).
5. Work requires prolonged walking and standing (up to 95% of work time); lifting, transferring, and positioning of patients and equipment (10% - 50% of work time), and the transportation of patients (up to 10% of work time).
6. Work with critically ill patients (up to 10% of work time) produces a high level of stress and mental fatigue.
7. Must be able to work with newborns (if assigned) to the oldest old (up to 20% of the work time). The majority of patient care (70% of work time) is the 18- to 65-year-olds.
8. The individual in this position must demonstrate appropriate knowledge of the principles of growth and development, which would include neonates to the oldest old. In addition, the therapist must demonstrate competency in examination/evaluation and treatment relative to age specific needs for the patient population for which they provide therapy, and as described in the department's practice standards.
WORKING CONDITIONS:
1. Works in hospital environments where there is some exposure to blood/body fluids/hazardous materials, communicable diseases (10% - 25% of work time) and unpleasant odors (up to 25% of work time).
2. Work requiring physical effort for lifting, transferring, and positioning of patients and/or equipment (up to 50% of work time) may result in musculoskeletal strain or injury with improper body mechanics.

NWH is recruiting for a physicaltherapist, per diem, in the acute care/inpatient setting, to cover shifts on both weekdays and weekends. Broad availability to work weekday shifts is needed and the ability to cover 2 weekend shifts per month (Saturdays, Sundays, or 1 of each) is required. Comprehensive orientation and mentoring program is available to all, especially new graduate clinicians.
At least 1 year of clinical experience in either acute care, acute rehab, or home care clinical settings is preferred; will consider new graduates with acute care or acute rehab clinical experiences.
In this role, the PT will:
* Provide care to adult med/surg patients, with majority in general and complex medical, post-op orthopedics, post-op general and vascular surgery, and other areas as needed (such as ICU, Emergency Department)
* Perform examinations/evaluations, develop plans of care, provide recommendations for ongoing therapy needs during hospitalization and post-discharge
* Perform and apply interventions during follow-up visits
* Independently manage a patient assignment

This position provides strength-based, family-centered services for young children under three years of age who have developmental delays or are at risk of a developmental delay. Services are designed to meet the needs of each infant or toddler, and the needs of their family. Provides services with the goal of enhancing the child's communication, physical, cognitive, social-emotional and adaptive skills.
I. Major Responsibilities:
1. Provides direct intervention services to children 0 - 3 and their families in natural settings.
2. Carries out service coordination functions when appropriate as defined by the Early Intervention standards.
3. Conducts evaluations and other assessments.
4. Develops and implements Individual Family Service Plans.
5. Assists in transition planning prior to discharge from the program.
6. Referrals to outside agencies as needed.
7. Maintains client records and other administrative duties according to DPH and Lipton Early Intervention policies.
Specialty Position Responsibilities:
1. Evaluates and treats children with musculoskeletal, neurological, and/or cardiopulmonary deficits which may block normal movement.
2. Demonstrates knowledge of typical infant/toddler development in perceptual/fine motor, gross motor, cognition, language and communication, self-care and personal/social areas.
3. Demonstrates the ability to support positive child and family development.
4. Develops functional intervention techniques appropriate for infants and toddler with atypical development.
II. Position Qualifications:
License/Certification/Education:
Required:
1. Bachelor's degree or higher prepared graduate of a curriculum in Physical Therapy approved by the American Physical Therapy Association.
2. Current PhysicalTherapist license by the Massachusetts Board of Allied Health Professions; license in good standing.
3. Satisfactory CORI review as outlined in the regulations set by the Massachusetts Executive Office of Health and Human Services.
4. Current valid US-Issued driver's license and the ability to provide a registered and insured automobile for work related purposes, including travel between in-person visits/sites.
5. DPH's CEIS Certification or eligible for certification.
6. NPI (National Provider Identifier number) or apply for NPI.
7. CPR and First Aid Certification or obtain certification within six (6) months on the job.
