The therapist is expected to be able to perform all modes of Respiratory care as defined in the department policy and procedure manual. These tasks will include, but are not limited too, delivery of small volume nebulizers, drawing of arterial blood gases, maintaining artificial airways and mechanical ventilation. The therapist must be competent to deliver these therapies to all age groups ranging from perinatal to geriatric. I. Major Responsibilities: 1. Demonstrates excellent customer service skills. 2. Demonstrates competency in proper set up and utilization of the V60 NIPPV 7. Demonstrates competency in proper set up and utilization of all High Flow Oxygen Therapy systems 8. Demonstrates competency in the disinfection, transport and set up of all non-disposable equipment. 9. Adheres to the Hospitals' Standards of Behavior. 10. And that starts with our employees. Job Description This position is responsible for respiratory care for patients of all age groups, who are diagnosed with cardio-pulmonary dysfunction. Services are provided throughout the Tufts Medical Center are complex. This includes the ICUs, in-patient rooms, the Emergency Room, and outpatient clinics. A request for service originates from the medical staff of Tufts Medical Center. It may be written or verbal and can be communicated to the therapist by direct telephone conversation and/or beeper page. The patient's nurse or unit secretary conveys the order. All patients are assessed for their needs initially and on an on-going basis until no longer required. Documentation of services provided is recorded in the Respiratory Therapy Progress Notes section located on the department flow sheet. A copy of this document is filed in the patient Medical Record. A list of the specific practices available by this department is noted below. A Respiratory Therapist is under the direction of the Director and Assistant Director of Respiratory Care. Patient services are available 24 hours/day, 365 days/year. Services are provided in accordance to policies and procedures agreed upon by the Medical Directors of the department. Clinical Practice Guidelines have and continue to evolve as the framework of clinical respiratory care practice. These practices are well referenced and sponsored by the American Association for Respiratory Care. One needs to demonstrate the ability to communicate with and assess the patient needs of different age populations to provide the necessary care. This will be demonstrated by supervisory review of Daily Shift Notes documented in all areas of clinical practice. Skills will be rated as part of your Performance Appraisal. Ability to perform all required duties under emergency and extreme stress situations. When cross-trained to perform neonatal and pediatric services, one is expected to acquire and maintain NRP and PALS certification. Demonstration of competency is required on an annual basis. This will include computer based training and evaluation in adult, pediatric and neonatal mechanical ventilators. EDUCATION: Bachelor's degree level in health profession, science or equivalent is preferred. Associates degree in Respiratory Therapy is required. Must possess the RRT credential. This position reports to the Clinical Manager, Respiratory Care and operates within established organizational and departmental policies and procedures. Flex RT's will generally work 24 hours per week. However, based upon the hospital's patient care requirements as determined at the discretion of the flex RT's manager or designee, a flex RT may be required to flex her/his hours either up 12 hours or down 12 hours within a week. Flex time RT's in a 24 hour position will receive health and dental benefits consistent with how regularly scheduled 36 hour RT's receive those benefits. Attends daily bedside rounds and administers Respiratory modalities and medications including bronchodilator therapy, surfactant, oxygen, and nitric oxide. Assesses patients' conditions, progress and reaction to the therapy. Modifies treatment care plan accordingly. 5. Performs moderate complexity point of care (POC) testing for Blood Gas Analysis. Requires an Associate or Bachelors degree in chemical, physical, or biological science or medical laboratory technology or other qualifications as described in the Federal Register, 42 CFR, Subpart M 493.1403 - 1495. 6. Uses professional judgment regarding appropriateness of therapy; modifies treatment and/or informs physicians with recommendations for change. Administers appropriate tests and procedures as ordered including; oximetry, hyperoxia tests utilizing transcutaneous O2 monitoring, radial arterial blood gases. 7. Completes necessary documentation of all pertinent observations related to patient care and treatment administered with patients' responses to treatment. Pulmonary ALS Clinic Responsibilities: * Supports early detection of neuromuscular weakness through forced vital capacity, maximum inspiratory pressure, and peak cough flow measurements, as well as ongoing disease monitoring. * Assists with initiation and troubleshooting of non-invasive ventilation therapies. * Provides patient education regarding the use of home ventilators and PAP devices for both invasive and non-invasive ventilation. * Aids with mask fitting for non-invasive ventilation. * Educates patients on the operation of mechanical insufflation-exsufflation (MIE) devices and portable suction equipment for airway clearance. * Instructs patients in tracheostomy care and management of tracheostomy tubes. * Provides guidance on oxygen concentrator usage and nebulized medication administration via mask-based or machine-based systems. * Assists in the collection, download, and interpretation of home ventilation and positive airway pressure therapy data, utilizing remote monitoring technologies. * Coordinates with Respiratory Therapists from durable medical equipment providers to support home ventilation needs.
