Summary:
The Nursing Assistant II is responsible for assisting the professional caregiver in the delivery of patient care, transporting patient and / or equipment, setting up and maintaining patient rooms, and performing a wide range of clerical, transcription, and maintenance of the patient care unit desk area functions. Performs patient care requiring clinical and procedural knowledge and skills requiring the use of sterile techniques, infusion monitoring, and medical equipment. Coordinates the flow of communications on the patient care unit.
Responsibilities:
1. Assist physicians and other health care providers with routine examinations and procedures, as well as procurement of equipment and supplies.
2. Critically review patient care documentation making note of any significant changes in the patient's condition and make an immediate report to a registerednurse or physician.
3. Provide a variety of clerical tasks to include but not limited to organizing and prioritizing workload, greeting and directing visitors, researching information as requested by nurses and physicians, reviewing orders for logic and clarity, revising orders as requested by the nurse, answering the telephone/ patient call lights, transcribing physician orders, and maintaining medical records. Perform routine quality assurance audits, safety checks, and inventory of supplies.
4. Provide administrative and clerical support including PI audits and data entry
5. Provide clinical support assistance to include, but not limited to, cleaning the work area, maintaining office equipment, troubleshooting common software and hardware problems, changing linens, checking supply inventory, re-stocking supplies, transporting patients/ supplies/ specimens, and assisting in the orientation of new employees and students.
6. Provide direct personal care to patients as outlined in plan of care activities such as bathing, dressing, feeding, assisting with ambulation. Measure and record vital signs, and patient height and weight. May be responsible for specimen collection, skin preps/scrubs, catheter care, capillary blood glucose checks, simple wound care, range of motion exercises, and/or postural drainage. Performs Oxygen room set-up and monitors oxygen flow-rate, performs oropharyngeal and nasopharyngeal suctioning, tracheostomy care, fecal impaction removal, sterile dressing change wound irrigation, IV fluid assembly and monitoring, IV site care and discontinue peripheral IV, ostomy irrigation and care, urinary catheterization and irrigation, performs established oral, nasogastric, gastrostomy tube feeding
7. Reinforce routine teaching given to patients by the registerednurse or physician and document patient understanding of instructions. Distributes patient education materials at the request of patients and/or health care providers

Professional Experience Requirements:
● One (1) year of clerical or customer service experience and six (6) months of related nursing assistant experience, which may include a nursing assistant course or an equivalent combination of education, training and experience.
Knowledge/Skills/and Abilities Requirements:
● Must be fluent in verbal and written English language. Capability to develop computer skills if not competent.

RegisteredNurse (RN) Clinical Documentation Denials Auditor Job Responsibilities:
Evaluates specificity and completeness of physician documentation to ensure optimal coding (e.g. mortality outcomes using APR-DRG, SOI and ROM, appropriate reduction of complications based on PSI and HAC, revenue assurance outcomes based on reimbursement DRG (MS-DRG), documentation of significant chronic conditions affecting resource utilization based on HCC).
Summarizes audit findings for individual records along with specific documentation guidelines to improve expected clinical outcomes for an individual physician, physician practice, or specialty.
Conducts follow-up audits (i.e. concurrent or post-discharge) with routine feedback until documentation practice comes into line with expected clinical outcomes.
Works with Clinical Documentation Improvement (CDI) Director and Lead Auditor on other work related to physician audits and education programs.
Demonstrates proficiency with Cobius to access external audit work, record summary results and upload appeal letters.
Demonstrates proficiency with Encompass 360 and HDM audit functions to review electronic medical records with advanced functions (i.e. ex, auto-suggest and search) and record detail coding audit results.
Showcases proficiency in reviewing records in Epic electronic medical records - which may be the only option for audits of older records.
Demonstrates proficiency in writing effective appeal letters that include appropriate coding guidelines and medical references.
Identifies trends in external audit findings related to coding quality and physician documentation. Prepares educational communications related to these findings.
Evaluates physicians' documentation, diagnostic reports, and clinical findings for validation of diagnoses.
Processes the requests for second opinion reviews when clinical validity is not supported or in question.
May perform additional duties as assigned.
Minimum Qualifications:
Certification: Certified Coding Specialist / Certified Clinical Documentation Specialist; ACDIS/AHIMA certification, CCDS or CDIP
Licensure: Current RN license and eligible to practice in VA
Experience: Seven years of recent CDI, DRG validation or coding audit experience in an acute hospital setting with clinician training as RN, BSN, NP, PA or MD; Coding certification CCS and CDI certification CCDS or CDIP
Education: Associate Degree in Nursing or Medicine.
Preferred Qualifications:

· Ensures the initial and ongoing consent process is performed and documented in compliance with FDA, International Conference on Harmonization Good Clinical Practice (GCP), institutional, sponsor, IRB, and other applicable regulations and policies. Ensure patient comprehension and safety during initial and ongoing clinical trial informed consent discussions. Counsels and teaches research participants about protocol participation, treatment, and side effects, medication compliance, and administration of investigational drugs.
· Documents assessment, management, and evaluation in source documents for patients on clinical trials as appropriate to the protocol. Ensures that relevant data from the source document are abstracted and accurately recorded in the clinical trial case report forms and that every data point can be verified within the source document.
