This licensed clinician will provide licensed clinical social work to children, adults, and families or groups regarding psychological or emotional problems such as adjustment to illness and pain, domestic violence, post-traumatic stress and anxiety, mood disorders, psychosis, attention deficit, substance abuse, addictive gambling, or family situations. This licensed clinician will develop plans with the client and implements therapeutic treatment plan in strength-based model in both the doctors' offices and the individual counseling office. The licensed clinician will provide culturally sensitive mental health services to the Latino communities by providing assessments, referrals, and follow-up appointments. Duties/Responsibilities: Provide direct behavioral health and medical social work services to Latino individuals, families, groups, and communities, under the supervision of the Mental Health Services Director. Provide crisis counseling as needed and outreach counseling to community settings such as schools, home visits, and other settings as assigned by the Mental Health Services Director. Provide specialized counseling groups or classes for adjustment to illness, adjustment to pain, parenting classes, depression, grief, anger management, teen issues or on other topics within the scope of license and training. Participate in weekly case review meetings. Supply case files including assessments, treatment plans and progress notes and other reporting and statistical data in a timely manner as required by the law, department and Unicare. Supply court psychosocial evaluations, give reports of attendance and make court appearances when necessary. The LCSW will discusses progress toward goals with client such as controlling weight, controlling pain, adjusting to stress, or substance abuse education and support services such as observing urine tests. The LCSW will be able to conducts relaxation exercises or learn relaxation skills from supervision, facilitate peer counseling groups, and family counseling during clinical therapy sessions. The clinician will refer clients to supportive community services and in-house medical services or support programs to supplement office treatment and counseling. Adheres to accreditation and compliance standards/guidelines and gets yearly cultural supportive training. Performs other training and job-related activities mandated. Assists in development of departmental policy, procedures, and quality improvement activities within the Clinic as directed. Periodically provide community mental health presentations and power point presentations on pain management, childhood mental health issues, or stress management workshops to the community or Unicare staff. Advanced Practice Clinician, SNF External Description: The Alignment Care Anywhere program is a physician-led and advanced practice clinician-driven program that is intended to care for and support Alignment Healthcare patients by proactively providing an additional level of medical and social support in the comfort of their home, which includes private residences and long-term care facilities. This program is expanding the locations where it provides patients with medical and social support when and where they need it, with the goal of preventing unnecessary hospitalizations, health complications, and unmanaged disease progression that can occur when timely clinical interventions are not provided or are not accessible. Our home-based programs are offered to eligible patients at no cost to them. Immediate opening for a Nurse Practitioner or Physician Assistant to travel to local skilled nursing facilities. You would see approximately 10-15 patients a day depending on the daily census. Position Summary: An exempt clinical position where the nurse practitioner (NP) or Physician Assistant (PA) is responsible for providing direct patient care. The NP participates as a leader of the skilled nursing facility (SNF) care team. Visits Alignment members/patients at skilled and long-term levels of care in designated SNFs facilities. Provides appropriate evidence-based geriatric medicine. Coordinates care with hospitalists, primary care physicians and care managers. Makes home visits as directed by the medical staff to meet patient needs and provide continuity of care. Consults/coordinates with specialists as needed Addresses acute mental status changes via non-pharmacologic or pharmacologic measures, consultation or transfer Coordinates/assess rehab progress on a daily basis Discusses concerns with the patient, family, rehab, and case management. Long-Term Care Assists case management in the evaluation of selected long-term care patients Follows “new” long term patients every 30 days Assists the attending physician with management for complex long-term patients Works closely with AHC clinical leaders, regularly discussing any concerns regarding utilization of services or complicated cases. Collaborates with PCPs, IPAs, and external and AHC Case Managers to develop care plan for members. Advanced Practice Clinician, CAW External Description: The Alignment Care Anywhere program is a physician-led and advanced practice clinician-driven program that is intended to care for and support Alignment Healthcare patients by proactively providing an additional level of medical and social support in the comfort of their home. This program provides patients with medical and social support when and where they need it, with the goal of preventing unnecessary hospitalizations, health complications, and unmanaged disease progression that can occur when timely clinical interventions are not provided or are not accessible. Our home-based program is offered to eligible patients at no cost to them. In this role, you will conduct in-home health assessments on a panel of enrolled Medicare Advantage members within a geographic area of responsibility. Our members are typically aged 65 and over and home visits will range from 30 minutes to 60 minutes depending on whether it is a follow-up or a comprehensive initial visit. Position Summary: The Alignment Jump Start Assessment program is a physician-led and advanced practice clinician-driven program that is intended to care for and support Alignment Healthcare patients with complex and chronic care needs. As a JSA Advanced Practice Clinician, you will work within an interdisciplinary team environment that includes physicians, nurse practitioners, physician assistants, case managers, and other health care team members to ensure proper delivery of clinical and home-based patient care. In this role, you will assess, develop, and coordinate options and services promoting quality care in order to achieve optimal health care outcomes while also ensuring cost-effective care complying with Alignment policy and all state and federal regulations. Our members are typically aged 65 and over and home and virtual visits will be between 45 and 60 minutes depending on whether it is an annual or initial visit. General Duties/Responsibilities: (May include but are not limited to) JSA APC's responsibilities · Conduct initial or annual face-to-face patient visits in home and clinic settings · (Follow CMS guidance to make virtual visits when appropriate) · Health Risk Assessment includes: o Past medical history o Review of symptoms o Physical examination o Medication review o Cognitive/depression screenings o Fall assessment/functional assessment o Immunizations and preventive health screenings including Colonoscopy, Breast Caner screening, Bone density, and diabetes eye exams o Advance Care Planning and POLST · Update and reevaluate the year-to-year diagnoses following CMS guidelines and HCC coding · Identify diagnoses and develops proper care plan for chronic disease management · Provide patient/family/caretaker education and facilitate patient empowerment and quality of life · Matching care desired with care given while providing true, informed consent · Collaborate with HEDIS/STARS and other Alignment programs and departments to facilitate optimal program integration and continuity of care · Refer patient to an appropriate programs such as Care Anywhere Program · Refer patient to social worker for appropriate community resources as needed · Provide contact information for patient to receive medical assistance to avoid unnecessary emergency room visit and hospitalization · Comply with all HIPAA regulations and maintains security of protected health information (PHI) · Complete EMR documentation with quality that meets Alignment expectations in a timely manner. This associate clinical social worker will provide mental health and related services in a manner that upholds Unicare Community Health Center's mission to improve the health status and well-being of underserved segments of the population in the communities we serve through the direct provision or coordination of health care, health education, and services. This includes interviewing, treating, coordinating, planning and linkage to a full range of resources that meet practical, social and emotional needs, in collaboration with a multidisciplinary team and community and community-based programs. Duties/Responsibilities: Conducts clinical interviews to gather information, assess patient needs, determine the appropriate need, and develop diagnostic information. Maintains accuracy, completeness and timeliness with medical charts. All medical charts must be closed with 48 hours of the encounter In order to ensure high standards of care and compliance with regulatory requirements. Provide short-term, structured, evidence-based psychotherapy and counseling as part of a treatment plan. Provides Clinical therapeutic services to patients across the lifespan, including individual and group therapy and coordination of treatment for participants with co-occurring disorders. Provide crisis intervention as needed in different clinics. Participate in the development of patients' individualized care plans, in a timely and thorough manner. Provide treatment to assigned patients within the scope of practice, meeting program/department productivity and quality standards. Coordinates and collaborate with Primary Care Providers, Behavioral Health Coordinator, Case managers, Substance Abuse Counselor, Medical Director of Behavioral Health, Chief Medical Officer and other Staff to improve health outcomes. Facilitates referrals to other services and allied agencies as needed. Works closely with Unicare's Psychiatrists and other Clinicians when needed. Meets patient needs effectively and resolve individual barriers through follow-up, advocacy, and collaboration with other staff and community service providers. Serves as a mandated Child Protective Services (CPS) and Adult/Elder Abuse reporter. Maintains accurate and timely records of activities, case management notes, and services provided to each patient. Facilitates and participates in chart reviews and audits of behavioral health clinicians. Completes paperwork to support mental health billing in a timely manner. Participate in weekly case review meetings. Supply court psycho-social evaluations, give reports of attendance and make court appearances when necessary. The clinician will refer clients to supportive community services and in-house medical services or support programs to supplement office treatment and counseling. Adheres to accreditation and compliance standards/guidelines and gets yearly cultural supportive training. Periodically provide community mental health presentations and power point presentations on pain management, childhood mental health issues, or stress management workshops to the community or Unicare staff.
