Job Description Summary
As a Home Health RN, you will play a vital role in delivering compassionate, skilled nursing care to patients in the comfort of their homes. You will work closely with patients, families, caregivers, and physicians to ensure high-quality, patient-centered care in compliance with California regulations and agency policies.
Key Responsibilities:
Provide professional nursing care as prescribed by the physician and in accordance with state, federal, and agency standards.
Conduct comprehensive initial assessments and ongoing evaluations of patient status and care needs.
Develop, implement, and adjust individualized care plans based on patient condition and progress.
Administer medications and treatments safely and accurately, monitoring for effectiveness and side effects.
Deliver high-quality respite shift care for vulnerable patients, which may include:
Tracheostomy management and suctioning
Ventilator monitoring and support
G-tube feedings and medication administration
Ongoing assessment and emergency response as needed
Educate patients and their families/caregivers on disease management, medication use, self-care techniques, and safety practices.
Collaborate with the interdisciplinary team to review medical necessity, coordinate services, and support discharge planning.
Maintain accurate, timely, and thorough documentation of all patient care activities.
Promote and maintain a safe environment for patients in the home setting.
Serve as a resource and provide hands-on supervision when needed to ensure quality and continuity of care.

The Emergency Department at Valley Children's provides emergency pediatric and trauma care to infants, children, and adolescents (up to 21 years of age). We operate a 39-bed Level II Pediatric Trauma Center that sees about 120,000 patient visits annually. More than two-thirds of the Hospital's patients are admitted through the Emergency Department. We have access to all of the Hospital's pediatric subspecialties and their associated physicians.
The Department is staffed by board-certified pediatric emergency physicians. In addition to their years of experience in providing pediatric emergency care, our emergency specialists have undergone a minimum of three years of general pediatric training plus an additional two to three years of fellowship training in pediatric emergency medicine.
