What You Will Do:

Makes decisions relevant to the operation and management of the facility during the assigned shift.

Verifies that the resident's physician and family are notified when there is a significant clinical development in the resident's condition or if the resident requires immediate medical care.

Directs proper handling of resident care emergencies.

Assumes other duties as assigned by ADNS.

What You Will Need:

Interest in the needs of the aged and the chronically ill with the ability to work with both.

Registered Nurse licensed in the State of employment. Minimum of one year of education or experience in geriatric nursing or rehabilitation preferred.

Knowledge of nursing theory and practice including the administration of medications.

Prestige Care Center of Morrison is immediately hiring a Care Coordinator for our 180 bed skilled nursing facility. We are under new ownership and management and are we committed to providing quality care and the best experience for our patients. Prestige Care Center of Morrison is nestled in the foothills near Red Rocks. With beautiful scenery, wildlife, and mountain views, you are sure to find our location a great place to work. We are located off multiple interstates and highways, which makes getting to the facility an easier task.

Major Duties and Responsibilities

Assists the Director of Nursing in planning, developing, organizing, implementing, evaluating and directing the day-to-day operations of the Nursing Services department, as well as its programs and activities, in accordance with current state and federal laws and regulations.

Interprets and communicates policies and procedures to nursing staff, and monitors staff practices and implementation.

Participates in all admission decisions as indicated.

Participates in daily or weekly management team meetings to discuss census changes, resident changes in status, complaints or concerns.

Assists in evaluating work performance of all nursing personnel and implements discipline according to operational policies.

Participates in QAPI or facility assessment activities as needed, such as carrying out duties assigned as part of a performance improvement committee.

Facilitates, serves, attends or participates in various committees of the facility as appointed.Ensures delivery of compassionate quality care and nursing supervision as evidenced by adequate staff coverage on the units, general cleanliness, and maintaining optimal resident functions.

Collaborates with members of the interdisciplinary team, physicians, consultants, and community agencies to identify and resolve issues and improve the quality of services.

Assists with oversight of nursing schedules to ensure resident needs, regulatory and budget standards are met.

Performs rounds to observe residents and ensure nursing needs are being met.

Conducts observations of nursing care and supervises development of in-service education to ensure nursing staff is competent in current knowledge and skills.

Promotes teamwork, mutual respect, and effective communication.

Monitors, assists and implements the infection control program in accordance to current infection control guidelines to prevent the development and transmission of disease and infection.

Participates in budget development for the nursing department, medical, nursing and central supplies as indicated.

Assists in the preparation of inspection surveys, including staff in-services on survey processes, and participation in mock surveys as indicated.

Participates in the facility’s plan of correction response to an inspection survey and implements any follow-up required for nursing allegations.

Communicates directly with residents, medical and nursing staff, family members, department heads and members of the interdisciplinary team to coordinate care and services and respond to and resolve complaints and concerns.

Assists in oversight of resident incidents and concerns daily to identify any unusual occurrences and reports them promptly to the Director of Nursing, Administrator, and/or state agency for appropriate action.

Monitors for allegations of potential abuse or neglect, or misappropriation of resident property and participates in the investigative process.

Promotes safe work practices, safety rules, and accident prevention procedures to prevent employee injury and illness.

May provide direct nursing care as necessary as well as rotaion with an on call schedule.

Required Qualifications

v A Nursing Degree from an accredited college or university.

v Must have, as a minimum, three (3) years of experience as a supervisor in a hospital, nursing home, or other related health care facility.

v Current unrestricted license as a Registered Nurse (RN) in practicing state.

v Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations and guidelines pertaining to long-term care.

Current CPR certification.

Alaris Health at West Orange, a 120 bed-skilled nursing Center in West Orange, New Jersey, seeks a unit manager at a professional nursing level to lead the department and assume 24-hour responsibility for the continuity of nursing care and management of the residents’ welfare. Additional responsibilities include:

collaborating with the director of nursing in maintaining adequate nursing coverage to provide safe nursing care 24 hours a day, seven days a week, covering on weekends and holidays as requested

orienting, demonstrating, teaching and evaluating nursing staff’s skills and assessing their work performance

implementing the progressive counseling process and making recommendations or taking disciplinary action as needed

participating in the development, implementation and evaluation of quality assurance activities for the Center and/or unit

developing and implementing discharge plans with residents, their families and the interdisciplinary team

developing a program to facilitate continuity of care when residents are transferred to other nursing Center or the hospital
preparing written reports, reviewing records and participating in data collection as needed submitting budget requests and above all, ensuring that residents and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals’ needs and rights
JOB REQUIREMENTS:
Graduated from an accredited school of nursing Minimum of one year in a leadership role (e.g. head nurse, supervisor or similar) Currently licensed by the New Jersey State Board of Nursing Examiners of Nurses Association of Professional Nurses membership is recommended Strong leadership and organizational skills Ability to work independently, problem solve and make responsible decisions Ability to positively interact with everyone from residents to government agencies and the general public Experience developing and implementing programs, policies and procedures necessary to providing quality care Highly knowledgeable about nursing and medical practices and procedures as well as Federal and State regulations laws, regulations and guidelines pertaining to long-term care
