Unit Clerk Position Overview:
Unit Clerk team members assist with ensuring the health and well-being of our residents by providing administrative support. This position is responsible for coordinating proper placement of documents in Medical Record; performing secretarial duties for assigned unit; assist with inventory and requisitioning of nursing supplies, forms and stock medications; schedule various appointments, laboratory or other diagnostic needs.
Responsibilities
Unit Clerk:
Demonstrates understanding and implementation of regulatory guidelines; audits medical records of individuals and maintains required audit documentation.
Closes Medical Records following the hospitalization or permanent discharge of the resident.
Performs other medical record procedures under the direction of the Unit Manager.
Upholds all rules pertaining to protection, safekeeping and confidentiality of information regarding residents.
Assists Medical Records with preparation of Admission/Discharge Summary form.
Types organizational lists and performs other secretarial functions for the unit.
Distributes mail to residents and staff of unit.
Answers call light signals; directs or assists as appropriate.
Coordinates the preparation of forms and documents needed periodically for Medical Records and Treatment Books, Resident Care Planning meetings, and MDS assessments. This includes aide assignment sheets, ROM/Ambulation Records, skin and behavior records.
Coordinates obtaining of information from all disciplines for Inter-disciplinary Discharge Summary and sending information to appropriate destination in a timely manner.
Schedules appointments for consults/follow up for residents including Podiatry and Dental Consultant appointments and influenza vaccinations, if applicable.
Notifies attending physicians of due date for visits, as required.
Responsible for inventory and requisition supplies needed on the nursing unit, including nursing treatment supplies and printing of forms, as assigned.
Handles all information about residents in a confidential manner.
Maintains lab report schedule for admission, routine and annual requirements or special needs of residents. Completes appropriate forms and schedules appointments for lab draws.
Assists with obtaining statistics for nursing monthly report and for annual survey by state health department.
Photographs residents upon admission and places picture in Medication Book.
Assists with obtaining of room labels for resident rooms.
Assists with transporting residents and passing meals as necessary and directed.
Obtains approvals for transportation and schedule van transportation.
Attends facility meetings, staff meetings, and required or preferred in-services.
Qualifications
Unit Clerk:
Minimum of 16 years of age required; HS diploma or equivalent preferred
Employment Certificate/Permit required for applicable individuals
Office practice skills necessary
Knowledge of medical records processing preferred but not required
Attention to detail and accuracy in work required
Personal qualities of tact and dependability, caring and compassionate attitude toward residents
An understanding of need for confidentiality and discretion in processing of information about residents required.
Basic understanding of medical terminology and accepted abbreviations.
This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. The Lead Medical Receptionist (LMR) is required to perform all duties of a receptionist and function as the main resource person to receptionist staff at the assigned site. The LMR acts as a liaison between management and staff to assist in enhancing efficient patient check-in operations. This position may include occasional evenings and Saturdays. In close collaboration with the Location Manager, the LMR will have the following duties:
Duties
Assist in determining and coordinating staff assignments.
Schedule and arrange coverage for reception staff, including absences, lunches, and vacations.
Supervise reception staff to ensure high-quality standards, smooth facility operations, and efficient patient check-in.
Assess new staff members' strengths and identify opportunities for development.
Clearly communicate operational expectations and procedural changes to reception and provider staff.
Assist in ordering office supplies to maintain appropriate stock levels.
Support and perform direct patient check-in duties as needed.
Assist with recruiting, hiring, and training of reception staff.
Conduct comprehensive annual performance evaluations for all receptionists.
Ensure patient check-in procedures are consistently followed.
Uphold the highest level of ethical and professional conduct within the reception team.
Develop strategies to increase patient utilization of self-check-in kiosks.
Reconcile daily credit card transactions (Open Edge vs. Medent) and ensure receptionist drawers are balanced; assist with discrepancies and coordinate with Billing as needed.
Distribute weekly provider suite schedules to reception, X-ray, DME, and OC staff.
Ensure kiosks and patient tablets (Surface Pros/iPads) are charged, functional, and connected; report or route technical issues to appropriate personnel.
Support reception with front desk situations and serve as point of contact for patients requesting assistance beyond reception.
Act as liaison between reception and providers/secretaries to resolve issues and maintain smooth communication.
Restock bathroom supplies and water cooler items during patient hours when needed.
Coordinate site-related issues with the Site Manager and escalate emergencies requiring EMS or provider attention.
Ensure Patient Incident Forms are accurately completed and reviewed with the Site Manager.
Monitor and dispose of unclaimed patient paperwork (after 1 month) and purge abandoned or uncollected CDs.
Represent the reception team in meetings by sharing concerns, feedback, and suggestions for improving workflows and team efficiency.
Support special projects and tasks assigned by upper management.

Unit Clerk Position Overview:
Unit Clerk team members assist with ensuring the health and well-being of our residents by providing administrative support. Unit Clerk:
Demonstrates understanding and implementation of regulatory guidelines; audits medical records of individuals and maintains required audit documentation.
Closes Medical Records following the hospitalization or permanent discharge of the resident.
Performs other medical record procedures under the direction of the Unit Manager.
Upholds all rules pertaining to protection, safekeeping and confidentiality of information regarding residents.
Assists Medical Records with preparation of Admission/Discharge Summary form.
Types organizational lists and performs other secretarial functions for the unit.
Distributes mail to residents and staff of unit.
Answers call light signals; directs or assists as appropriate.
Coordinates the preparation of forms and documents needed periodically for Medical Records and Treatment Books, Resident Care Planning meetings, and MDS assessments. This includes aide assignment sheets, ROM/Ambulation Records, skin and behavior records.
Coordinates obtaining of information from all disciplines for Inter-disciplinary Discharge Summary and sending information to appropriate destination in a timely manner.
Schedules appointments for consults/follow up for residents including Podiatry and Dental Consultant appointments and influenza vaccinations, if applicable.
Notifies attending physicians of due date for visits, as required.
Responsible for inventory and requisition supplies needed on the nursing unit, including nursing treatment supplies and printing of forms, as assigned.
Handles all information about residents in a confidential manner.
Maintains lab report schedule for admission, routine and annual requirements or special needs of residents. Completes appropriate forms and schedules appointments for lab draws.
Assists with obtaining statistics for nursing monthly report and for annual survey by state health department.
Photographs residents upon admission and places picture in Medication Book.
Assists with obtaining of room labels for resident rooms.
Assists with transporting residents and passing meals as necessary and directed.
Obtains approvals for transportation and schedule van transportation.
Attends facility meetings, staff meetings, and required or preferred in-services.
Qualifications
Unit Clerk:
Minimum of 16 years of age required; HS diploma or equivalent preferred
Employment Certificate/Permit required for applicable individuals
Office practice skills necessary
Knowledge of medical records processing preferred but not required
Attention to detail and accuracy in work required
Personal qualities of tact and dependability, caring and compassionate attitude toward residents
An understanding of need for confidentiality and discretion in processing of information about residents required.
