We assist clients by providing links to wrap-around services and external resources for stable housing. Our goal is to assist our clients experiencing homelessness, unstable housing, or who are severely rent burdened living in the County of Los Angeles and Orange County to find stable/permanent housing. Under the direction of the Director of Operations of Community and Outreach, the Housing Program Manager will be responsible for in-office supervision and implementation of UAII Housing Services. The Program Manager will directly supervise case management, navigators, and specialist staff as well as subcontracted partnerships (if applicable). Additionally, the Program Manager will be responsible for meeting program and fiscal requirements; maintaining community relationships with public and private organizations; and providing reports and documentation as necessary to support the operation of program services and to maintain or secure funding or grants. Conduct outreach to property management, apartment associations, landlords and other local meetings to promote Housing services. Will have experience and a thorough understanding of all functionalities of LAHSA, HIMS, CES, etc. Essential Duties Assist in development of an organization-wide phased in Housing Plan that aligns with agency Strategic Plan Build partnerships with LAHSA, CES organizations, rental offices, housing managers, and Tribal Organizations in coordination with housing plan goals Identify and assist with development of a Master Lease Agreement for up to 10 units Develop a plan and obtain Rapid Re-Housing Vouchers/Rental Subsidies/Room Key Vouchers that UAII manages Supervise housing case management, housing navigators, and housing support specialist as assigned Supervise and facilitate training related to navigating the Los Angeles Homeless Management Information System and identify training that would be beneficial to the AI/AN community Ensure the program is in compliance with government grant requirements Navigate HIMS and identify training that would be beneficial to serve our AI/AN Develop a housing procurement, financial, and self-sufficiency case management plan to utilize with clients. This shall include an intake interview to determine client's eligibility, needs, goals Assist participants in development of a trauma informed, strength-based/solution-focused individualized goal and action plan that promotes permanent housing and self-sufficiency; develop an effective, timely internal/external referral network in order to ensure ongoing direction and support as needed Assess housing barriers of individuals experiencing homelessness to determine housing and service needs Provide mediation and advocacy with landlords on the client's behalf to develop a workable plan to obtain and or maintain housing Supervise and participate in the negotiation, preparation, and administration of leases and housing assistance contracts Apply knowledge or create workshops to educate clients of their rights and responsibilities as it relates to housing Ensure the housing team is providing proactive follow-up home visits to ensure stability and further progress towards self-sufficiency; this includes support, advocacy, reducing isolation, listening, problem solving, and identification of resources to assist with reintegration of participants in the community and the retention of permanent housing Organize and implement a culturally competent program service that aligns with the designated evidence-based model for service delivery, including curriculum implementation, and coordinate with community partners and stakeholders Ensure and maintain accurate daily logs records, monthly outcome reports, and files for each housing client Maintain contact with program participants to ensure activities occur within time limits and in accordance with program and funding source requirements Network with other agencies, coalitions, and local community partners Collaborate and work closely with Workforce Specialists and Workforce Career Counselors to assist clients in the use of resources and technology for job search, job placement and career advancement activities Conduct outreach, including cold calls, to the business community to promote program services to homeless clients Conduct client recruitment and outreach activities Process program paperwork in accordance with agency program deadlines and prepare and submit monthly reports Able to represent the agency and communicate accordingly at identified community, government, and/or business meetings Use social media to coordinate community relations and to promote Housing program Keep leadership well-informed regarding activities, pending issues and potential problems You may perform all elements of this position or identified components, other duties as assigned or required for the purpose of the position or for organizational efficiency and effective client service Requirements: Demonstrated knowledge of Federal, State and Local fair housing laws Experience managing front office including all administrative duties and events Strong ability to prioritize tasks and possesses an affinity for details and strong data entry skills. Demonstrated success in programs that involve the homeless population. Position Summary We are seeking an experienced full-time, non-exempt Registered Dental Assistant (RDA) for assisting the dentist(s) and dental hygienist(s) in the direct provision of primary care dental services. The Registered Dental Assistant (RDA) will function as a circulating assistant in chairside dental procedures, under the direction of the Healthcare Interim Co-CEO. The RDA will perform all dentistry procedures, clinic infection control procedures, cleaning and sterilization of instruments, tray setup and materials. The dental assistant is also responsible for preparing the operatory and patients for treatment, chairside assisting, sterilization, inventory control of dental instruments and supplies. Job Duties & Responsibilities · Ability to perform all phases of four (4) handed Dental Assisting duties for endodontics, oral surgery, pedodontics, prosthodontics, and restorative procedures. · Ability to perform standard and radiographic methods. · Familiarity with dental terminology, materials, medications and instruments used during dental procedures. · Perform the following supportive tasks to ensure efficiency of patient flow: clean, sterilize, instruments, and set up trays. · Maintain and test equipment (sterilizer, x-ray dosimetry, x-ray processor, etc.) including daily lubrication of equipment. · Effectively pour models and conduct other lab procedures. · Ability to instruct patients in oral hygiene and preventive dental education under the supervision of the dentist. · Review and document inventory and reorder supplies as necessary to maintain par levels. · Perform charting activities and document exam findings in patient record. · Maintain clean, orderly and well stocked operatory and laboratory. · Ability to plan work duties to ensure dental clinic efficiency. · Knowledge of patient scheduling procedures. · Work in conjunction with Staff Dentists and healthcare team to develop and implement preventive dental activities for the community · Employee Safety: Safely performs all duties; follows required protective protocols to ensure personal safety as well the safety of others. · Must maintain compliance with ergonomic safety standards; be mindful of posture and regularly practice ergonomic stretches. · Safety: Responsible for ensuring that all duties, responsibilities and operations are performed with the utmost regard for the safety and health of all personnel involved, including themselves. · Safety: Take appropriate corrective actions to address matters pertaining to employee health and safety hat have been brought to their attention. · Other duties as assigned by Supervisor. Knowledge, Skills, & Abilities · Function chairside dental procedures as a Registered Dental Assistant (RDA). · Perform mouth-mirror inspections of the oral cavity, to include charting of obvious lesions, existing restorations, and missing teeth. · Apply and activate bleaching agents using a non-laser light-curing device. · Use automated caries detection devices and materials to gather information for diagnosis by the dentist. · Pulp vitality testing and recording of findings. · Place bases, liners, and bonding agents. · Chemically prepare teeth for bonding. · Place, adjust, and finish direct provisional restorations. · Fabricate, adjust, cement, and remove indirect provisional restorations, including stainless steel crowns when used as a provisional restoration. · Place post-extraction dressings after inspection of the surgical site by the supervising licensed dentist · Place periodontal dressings. · Dry endodontically treated canals using absorbent paper points. · Remove excess cement from surfaces of teeth with a hand instrument. · Polish coronal surfaces of the teeth. · Collects patient information and health/medication data and documenting in patient record. · Educates patients on good oral hygiene habits. · Updates patient records and treatment plans as directed by dentist. · Performs procedures as delegated and directed by the dentist in accordance with state regulation and law and department directive and protocol. · Monitors patient schedule daily to ensure adequate and efficient patient flow. · Follows California State Board of Dentistry, ADA and CDC Infection Control Guidelines and department procedures and protocols. · Ensures proper disposal of all contaminated or potentially contaminated material in accordance with department directives, center policy and state and federal regulations. · Uses time wisely and maintains a courteous and professional demeanor at all times. · Effective oral and written communications skills with sensitivity to lifestyles of targeted participants. · Skills in operating personal computer using a variety of computer software (Microsoft Office, Adobe, Microsoft Teams, Zoom, etc). · Ability to maintain strict confidentiality of client and personnel information while complying with all applicable federal and state privacy laws, as well as UAII policy (HIPAA, HITECH, etc.). Position Summary Under the general supervision of the LifeSpring Program Manager, the Case Manager plays a critical role in assisting youth and young adults who are at risk of homelessness or experiencing homelessness. This role combines housing navigation with intensive case management and skill development, ensuring that services are delivered through a youth-driven, whole person, trauma-informed model. The position supports participants in achieving independence, obtaining safe housing, and accessing necessary resources, all while adhering to a strength-based, culturally relevant, trauma-informed, and outcome-based approach. Specific Activities: 1. Client Support and Engagement: Create a welcoming, supportive environment to engage youth effectively and to build rapport, identify strengths and barriers to participant self-sufficiency, and empower youth to achieve goals and permanent independence. Provide basic needs assistance, including food, clothing, hygiene products, and access to essential services. Conduct intakes, eligibility screenings, basic needs assessments, and administer tools like the TAY-VI-SPDAT. Offer crisis management, safety planning, and emergency support as needed. Help build and support participants' relationships with family and community with an emphasis on permanent connections. Coordinate care with mental health professionals, primary care providers, outside social service organizations, and other supportive services.     AGENCY DESCRIPTION Instituto Familiar de la Raza, Inc. is a multi-service community health and social service agency with emphasis in serving the Chicano/Latino community in San Francisco with a special focus on the diverse needs of the Mission District. PROGRAM DESCRIPTION The Chicano/Latino Family Resource system (FRS) is a collaborative project of four community agencies. The project's purpose is to combine the efforts and staff of the collaborating agencies to develop prevention, early intervention, and intensive case management programs and services which reinforce family preservation with high risk families residing in the Mission District and Chicano/Latino families throughout San Francisco. The Differential Response (DR) program within FRS, works with fifteen partnered community agencies throughout San Francisco and with the Human Services Agency (HSA) to provide services to families referred to CPS and in need of additional support to prevent out of home placement and re-referrals. Position Summary The Program Manager is responsible for overseeing the Enhanced Care Management Team, Community Supports provider, and Case Managers. Under the supervision of the Program Director, the Program Manager is responsible for managing the daily operations of North County Lifeline's CalAIM and CinA programs. The Program Manager is responsible for providing leadership and guidance to the staff in the Youth Development department and to serve as a liaison between the Youth Development department and other departments within Lifeline, and between Lifeline and the community. Specific Activities Assures that the program consistently provides high quality, trauma informed, professional services. Assures timely, accurate and appropriate documentation. Provides administrative daily/weekly supervision to program staff. Assists with the recruitment, hiring, training and scheduling of program staff. Monitors staff productivity and provides constructive feedback and coaching. Prepares and conducts staff performance reviews annually or as needed and ensures adherence to agency policies and procedures including disciplinary protocols if the need arises. Engages in component and program development, implementation, and evaluation. Sets parameters for documentation requirements. Maintains statistics and meets reporting requirements for CinA, and CalAIM programs on the specified database, including program-specific ETO database. Assures accurate completion and submission of all contract narrative and statistical progress reports as required. Under the direction of the Program Director, coordinates or provides substance abuse and mental health services training, and other related staff development activities. Works closely with the fiscal department to ensure accurate and timely submission of all staff Absence Request forms, Mileage Sheets, Timesheets and any other contract/budget requirements. Develops, supervises and maintains systems and procedures that encourage and support a multi-disciplinary wraparound service model which ensures timely and appropriate critical core services for youth, adults and families, including information and outreach to the community, assessment referral and follow up, case coordination and management, home visiting, rapid response to crisis, drug and alcohol counseling and gender-specific services for females. Oversees and operates within program budgets and monitors the use of wraparound fund accounts. Provides Quality Assurance including file reviews, and oversight of documentation of assessment and intake, case management and progress notes, case planning and goal setting, completion of all necessary documents. Participates in and facilitates agency and component meetings. Stays informed of and disseminates to staff information related to the Enhanced Care Management Field. Resolves programmatic issues as identified by staff. Attend required meetings and training as appropriate. Participates in other duties as assigned. The Housing Specialist will be responsible to collect rent, be the bridge for the residents and apartment manager, conduct apartment inspections at least twice a month to ensure unit cleanliness and monitor for safety.     Job Description Mental Health Therapist, Community Services for Families (CSF) Program - Bilingual About Lifeline Community Services Lifeline Community Services is a premier nonprofit organization in San Diego County, dedicated to empowering individuals and families to achieve self-reliance and long-term stability. Our diverse service portfolio spans housing, behavioral health, youth development, and anti-human trafficking initiatives, operating across multiple locations. Position Overview General Responsibilities Under the supervision of the Community Services for Families Program Supervisor, the Mental Health Therapist will provide assessment services for CSF families with children 0-18 and have domestic violence as the reason for the referral. Assessment services may include every family member (parents and/or children) living in the home and will be conducted in the family home or at other locations as directed by client or County CWS staff. Specific Activities Provides culturally competent, trauma informed assessment services for CSF families referred by Child Welfare Services and community referred families. Provide assessments within 3 business days of receipt of referral Prioritize cases by case type in as stated in the CSF contract Conduct assessments both in person and occasional virtual sessions in the office and in the community Evaluate all members of the family living in the home for domestic violence, substance abuse, mental health, trauma, and other significant needs. Make recommendations on the basis of assessment findings for preventive or treatment services to mitigate exposure to trauma (for children) and to mitigate/treat issues negatively impacting child safety (for parent/caregivers). Provide initial recommendations for treatment, mental health and/or other services to County Social Worker within 3 business days of completing assessment. Complete and submit a written follow up report of assessment findings to referring party within 10 business days including assessment tools used, outcomes for each family member, type and level of services recommended, likelihood of a successful treatment outcomes, formal and informal interventions to strengthen family functioning to ensure child safety, prevention services to mitigate future effects of current child trauma, Family Centered Meetings attended, CSF referrals, and other information. Participates in Child and Family Team Meetings to discuss client progress in services and support them in achieving their case plan goals. Participates in Multi-Disciplinary Teams with County staff as needed. Participates in quarterly CSF countywide peer review process to discuss service delivery and improve upon best practices. Maintains ongoing communication with the CSF team to ensure quality of service delivery. Maintains detailed case files and documentation utilizing County approved forms and formats. Produces monthly reports of activities and submits to the CSF Supervisor. Remains culturally sensitive and respectful in all client and staff interactions. Participates in required staff meetings, collaborative meetings, trainings, County meetings, supervision, QA processes, and all other meetings and events as requested. Completes other duties as assigned. Training provided Qualifications and Requirements LMFT/LCSW/LPCC eligible by the California Board of Behavioral Science, BBS Registered or Licensed preferred, plus 2 years of related experience in direct client service. Bilingual Required. Training in and mastery of family systems theory such as Structural Family Therapy and application of diagnostic criteria. Strong skills in interviewing, rapport building, and assessment. Certification in Domestic Violence Assessment and Treatment provided once hired. Ability to make sound clinical decisions independently and to apply clinical judgment to developing effective treatment recommendations. Ability to focus on multiple priorities and meet deadlines. Must be compatible with Lifeline's purpose and philosophy. Must have reliable transportation current auto insurance, and a valid California Driver's license.
