Job Responsibilities
In cooperation with the referral source, provides care to individuals with orthopedic problems associated with athletic injuries, trauma, and disease process, including being involved with the development of injury prevention programs for athletes on the field and in the clinic.
Involved with the rehabilitation process by performing a wide variety of treatment procedures consistent with orthopedic care.
Responsible for keeping the sports medicine physical therapist updated as to how patients are responding to treatment and documenting this data as it relates to the goals of the plan of care when serving as a member of the physical therapy team.
Involved with the full spectrum of youth-adult outpatient orthopedic patients and the physically active individual.
Job Qualifications
Bachelor of Science degree.
Current licensure from the Ohio OT, PT and AT Board.

Documents evaluations, treatment, progress notes, and/or discharge notes in a timely and accurate manner in accordance with the KHN documentation policy. Utilizes clear, concise, language for documentation. Completes other necessary documentation including but not limited to daily charges and re-certification letters for outpatients in a timely and accurate manner.
Documentation meets standards of CMS, HFAP, and other regulatory agencies
Meets the quality improvement and assurances standards of department including but not limited to: patient/family education and timeliness of discharge notes in accordance with the KHN documentation policy.
Maintains/exceeds acceptable level of productivity based on current departmental standard. Prioritizes caseload/schedule based on patient acuity and departmental needs, assisting co-workers as needed. Accepts and completes assigned responsibilities and routine tasks in a timely manner.

HOW YOU'LL WORK:
You'll provide comprehensive skilled services as ordered by the attending physician. You will educate patients and their family members and ensures the safety of the patient. You are accountable for completing accurate documentation and remaining compliant with all legal rules and regulations.
MAJOR AREAS OF RESPONSIBILITY:
Plan of Care: Provide skilled physical therapy as ordered by the attending physician. Observe, record, and report patient reactions, changes, and progress to attending physician and any appropriate agency staff. Perform OASIS assessments to develop an individualized plan of care and makes adjustments as needs change.
Patient Outcomes: Treats patient to relieve pain, develop or restore function, and maintain maximum performance using physical means, such as, exercise, massage, heat, water, etc.
Minimize Patient Risk: Helps decrease re-hospitalizations by front loading visits for high risk patients and teaching the patient on preventative measures and making good decisions.
Family Educator/Advocate: Counsel patient and family on the disease/injury and how to manage in all phases of physical therapy. Instruct patient and family members on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.
Supervision: Oversee and supervise therapy services of patient provided by PTAs and when appropriate, nurse assistants.
Communication/Collaboration: Communicate with the PTAs and nurse aides on supervisory visit results.
Policies: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
Operations: Attend in-service training and mandatory agency meetings.
