As a Juvenile Youth Worker, you will be responsible for the safety and welfare of resident of the Westmoreland County Juvenile Services Center. Responsibilities include appropriate supervisor, coordinator, and implementation of resident activities and functions. The Juvenile Youth Worker will coordinate daily resident activities and functions including meal times, school support, recreational activities, and life skills. The position will implement reinforcement of positive youth behavior and enforce facility rules. The position will also be responsible for the preparation of verbal and written reports.
Qualifications:
Candidate must possess an Associate degree in Human Services, Criminal Justice, Administration of Justice, Psychology, Sociology, Education, or a related field. Have a minimum of five to eleven months childcare experience or any combination of education and experience.
Candidate must have the ability to read and follow directions, protocols, and procedures. Ability to use a variety of trained crisis intervention techniques to physically manage an acute acting out youth appropriately, safely, and securely.
Candidate must have and maintain a current valid Pennsylvania's Driver's license. Regulations require candidate be twenty-one years of age. Current FBI, State Police, and DPW Child Abuse Clearances will be required.

In this position, the duties may include serving as the care manager and psychotherapist to a patient population with highly complex health and mental health problems. The LPMHC is assigned to work within an interdisciplinary team to provide these services ensuring Veterans timely access to mental health and psychosocial services.
As a member of the interdisciplinary Psychosocial Rehabilitation and Recovery Center (PRRC) team, the Licensed Professional Mental Health Counselor must form and maintain effective working relationships with staff from the Behavioral Health Clinic, Community Based Outpatient Clinics and Primary Care Clinics in order to provide patients with an integrated and comprehensive treatment program.Major duties include, but are not limited to:
* Conducting psychosocial assessments to determine needs of Veterans and family.
* Serving as an active participant in interdisciplinary treatment planning, to include creation and review of treatment plans, as needed.
* Participating in interdisciplinary discharge planning, as needed, and ensure that discharge plans are executed in a timely and appropriate manner.
* Providing individual, family, and group counseling as needed.
* Providing consultation and education services to Veterans and families regarding community resources, advance directives, and VA benefits.
* Acting as a consultant for interdisciplinary team members.
* Providing consultation and education to medical residents, interns, and other VAMC personnel regarding associated VA policies and procedures.
* Acting as a marketing and public relations liaison for VA and community resources.
* Providing case management to Veterans and families, as needed, throughout the continuum and course of care.
* Providing the full range of Licensed Professional Mental Health Counselor within commonly accepted standards of practice, including: case work; individual, family, and group counseling; discharge planning; case management; and other needed services.
* Addressing Advance Directives and Organ Donation in accordance with VAMC standards.
* Providing wellness/prevention education and associated support groups.
* Coordinating treatment and communicate with care lines about continuity of care.
* Participating in committees as assigned.
* Maintaining data and statistics to comply with Joint Commission, VHA, and VAMC policies and procedures. Help as needed to prepare for associated reviews and surveys.
* Helping patients and significant others cope and deal with the psychosocial issues and the loss and grief associated with disability, terminal illness, and death.
* Documenting interventions and activities in each patient's clinical record utilizing CPRS and ensure appropriate hand off when transferring patient care.
* Participating in an on-call rotation, including modification of tour of duty to accommodate patient care needs on weekends, as needed.
* Performs other duties as assigned.
Work Schedule: 8:00am-4:30pm, Monday-Friday with on call coverage as necessary. Note: There may be instances when your tour of duty, work location and responsibilities may change to meet the needs of the Medical Center.

Essential Duties and Responsibilities
Manage an LSRP practice area including staff, clients, and revenue.
Support new and ongoing remediation projects as a LSRP.

JOB SUMMARY:

SPECIFIC DUTIES:
Provide service coordination (case management) for assigned group of clients including coordinating and monitoring the activities of the individual treatment team; assume primary responsibility for developing, writing, implementing, evaluating and revising overall treatment goals and plans in collaboration with the client and the ITT; provide individual supportive therapy and symptom management; ensure immediate changes; are made in treatment plans as clients' need change, educate and support clients' families; and advocate for clients' rights and preferences.

Provide ongoing assessment of client's mental illness symptoms and clients' responses to treatment plans to ensure immediate and appropriate interventions are provided in response to changes in mental status or behavior that put clients at risk (e.g., suicidality). Provide symptom education to enable clients to identify their mental illness symptoms.
Provide direct clinical services including individual supportive therapy and psychotherapy to clients on an individual, group and family basis in the office and in community settings to teach behavioral symptom- management techniques to alleviate and manage symptoms not reduced with medication and to promote personal growth and development by assisting clients to adapt to and cope with internal and external stresses.
Provide treatment, rehabilitation, and support services, with some interventions directed or performed by staff with specialty training and skills (e.g., vocational specialists).
Perform shift management in coordination with other shift managers according to established policies and procedures.
Provide on-call crisis intervention covering nighttime hours and serve as a back-up to evening and weekend staff.
Document client progress to maintain a permanent record of client activity according to established methods and procedures.
Provide individual and group treatment in the office and in community settings to a stage-based treatment model that is non-confrontational, considers interactions of mental illness and substance abuse, and has client-determined goals.

· A graduate degree in psychiatry, psychology, social work, nursing, education, rehabilitation or a graduate degree in the field of human services plus 2 year's experience in a CASSP system program.
· Be a licensed registered nurse (RN) with 5 years of experience including 2 years of experience in a CASSP system program plus have certification by the Department's Office of Mental Health (OMH) as a mental health family-based worker.
· A bachelor's degree in psychology, sociology, social work, nursing, rehabilitation, education, pre-med, theology, anthropology or a degree in the field of human services plus certification by OMH as a mental health family-based worker.
Certifications and licenses:
· Current and valid driver's license, vehicle insurance, and the ability to travel.
· Acts 33 & 34 Clearance and criminal record checks.
· Position requires enrollment in the WPIC FBMH Training Program.
· Reliable transportation is necessary.

WORK ENVIRONMENT
Primary work is performed in various community settings such as consumer's homes and in the schools. Work is also performed in a climate-controlled office building.
