The IFFS Case Manager assist in the development and coordination of focused, intensive supports in response to identified areas of need and difficulty while helping to build family capacity to support the individual child/young adult at home through referral, advocacy and flexible funding allocations, as needed.
This program is based out of Worcester and provides services across Central Massachusetts. Hours are flexible based on the needs of the families served. The anticipated schedule is Monday - Friday 9am-3pm with occasional flexibility to meet the needs of families, Department of Developmental Services (DDS), and outreach events.
Minimum Education Required High School Diploma/GED Responsibilities
Conduct comprehensive assessment of individual and family, including home visit, using referral and intake tools to determine and identify areas of need.
Develop an initial Family Support Plan (FSP) with each assigned family and renew annually.
Provide individually tailored support and coordination to families to ensure that goals, as identified in FSP, are met.
Broker services on behalf of all individuals and their families.
Coordinate and/or secure referrals for all necessary services.
Collaborate with DDS to ensure appropriate services for families
Provide ongoing progress reports to DDS on goals.
Transport individuals supported when and where needed, if necessary.
Report cases of reasonable suspicion of abuse to Department of Children & Families (DCF) and Disabled Persons Protection Commission (DPPC) as outlined in Mandated Reporter laws.
Attend and participate in DDS Quarterly review meetings
Attend trainings as assigned; maintain necessary certifications (CPR, First Aid, Safety Care)
Qualifications
Bachelor's degree in education, Psychology, Human Services or related field preferred. High School Diploma or equivalent and 3-5 years of experience.
1-2 years' experience in developmental disabilities field
1-2 years' experience and demonstrated success working with families, preferred
Family members (or guardians) of individuals with served by the DDS or DMH are strongly encouraged to apply for this position.

JOB SUMMARY
Provides care management for consumers receiving OPTIONS services, as well as those consumers appearing on the Office of Aging waiting list.
Works with consumer and family to assess need, develop and implement a safe and comprehensive Individual Service Plan (ISP).

Anchor House Ministries is a Christian child welfare ministry committed to helping youth and young men and women in crisis. AHM strives to provide a safe, secure, and nurturing environment to youth (ages 12-17), who have been referred to us by the State of Florida or private counselors. The young adult program for ages 18 24 serves young men and women who have aged out of foster care and need support. Residents end up at Anchor House for many reasons. Some have been orphaned, abandoned, neglected, or abused. Others are simply struggling to find their way in life. AHM staff works with residents through the treatment team, educational assessment, individual counseling, life skills training, and by simply meeting their basic human needs of nurture, food, shelter. The team focuses on the healing of the youth, so that they can manage their own emotions and build healthy relationships. The goal is family reunification or adoption, when possible, while other youth are prepared to safely navigate the transition into an adult life of independence and self-sufficiency.
Position Description
Implement quality care for the young adults in the program.
Follow policies and procedures.
Work closely with your supervisor to ensure the participants receive quality care and treatment that meets their needs.
Complete documentation in a timely and accurate manner.
Provide general supervision and coaching within the Mission of the organization.
Status: Non-Exempt, Part-Time

Compensation: $250.00 Weekly
Benefits:
Time Off: House Parents are allowed 3 consecutive days of paid time off at a time per month. On the 4th day of leave the time off will be deducted from the House Parents pay using the rate of $35.71 a day. Holidays celebrated by the organization can be taken as paid time off. See the personnel manual for a list of holidays. All time off requests must be approved by the Program Administrator.
Compensation: Weekly stipend; Housing, electric, and water included at no cost to the house parent.
Hours:
Overseeing care of the home estimated 4 hours weekly
Building relationships, assisting with life skills, and establishing rules estimated 8 hours weekly
Reporting/discussing concerns estimated 3 hours weekly
Minimum Qualifications:
Some college required.
One-year experience with at-risk youth or adults.
Support the mission and vision of Anchor House.
Possess and maintain a valid Florida Drivers License.
Required: Have and maintain a clear MVR (motor vehicle record).
Must be 21 or older for insurance purposes.
Must pass and maintain a county of residence check, federal background check, drug screen, civil check, child abuse record check, and sex registry check. (These are subject to change based on Federal and State regulations)
Should be able to demonstrate intermediate to advanced proficiency in Word processing and spreadsheet functions, preferably Microsoft Word, Excel, and Publisher as well as general email and attachment functions.
Essential Duties and Responsibilities:
Living on-site at the transition home. Sleeping at the home every night is not required. Please notify your Program Administrator if you will be away for more than 1 night.
Overseeing the care of the home. This includes reporting any maintenance needs and ensuring general needs, such as mowing and cleaning are completed regularly.
Monthly facility and bedroom checks documented and maintenance needs submitted.
Building relationships with the residents.
Assisting residents with life skills such as job searches, grocery shopping, cooking, appropriate interview attireetc.
Establishing general house rules with the residents.
Reporting any concerns or violation of the house rules to the Program Administrator.
Meeting regularly with the Program Administrator.
Completing the admission process and orientation with each new participant.
Completing an initial assessment and service plan with each new resident.
Completing monthly progress reports or service plan updates.
Completing annual training and professional development.
Work with team members of all backgrounds and make creating a culture of respect and inclusivity a priority.
Provide services in a way that is sensitive to diverse cultures and socioeconomic populations.
Make quality control a part of your daily focus and participate in data review and analysis as needed.

Of Special Consideration
Providers, whether male or female, may have to intervene in situations where residents may be unclothed such as bedrooms, restrooms or shower areas. Though not the norm, employees are expected to intercede and de-escalate in emergency situations.
Crisis Prevention Intervention training provides the knowledge and understanding for utilizing safety holds in these emergency situations. There is a high potential risk of physical harm to the employee during these circumstances.
