The Care Companion Caregiver is responsible for client satisfaction by assisting them with activities of daily living, providing companionship, and performing light housekeeping duties. The caregiver also helps with simple meal preparation, socialization, and client transport.
Schedule: Morning, afternoon, evening, and weekend hours, with availability required every other weekend. We can be flexible with your schedule; however, hours need to be consistent week to week. The role is roughly 10-15 hours per week.
Pay: Based on prior caregiving experience, between $16.44-$24.65 per hour

Ability to follow client and Care Companions directions
Ability to be on time to visits and complete tasks as scheduled.
Ability to work effectively, efficiently, and cooperatively with others.
Must have compassion for Care Companions clients and a desire to help others.
Ability to communicate effectively verbally and in writing.
Ability to work a minimum of 15 hours a week, including every other weekend (this would include a Saturday and Sunday consecutively).
Ability to complete all training and pre-employment background checks within required deadlines.
Ability to maintain confidentiality about clients, families, caregivers, support staff, donors, and organizational intellectual property.
Ability to use proper body mechanics and standard precautions.
Ability to be flexible in time and task to meet organizational needs.

Communicating regularly with patients, families, caregivers, and facility partners.
Assessing bereavement risk and supporting grief services as needed.
Coordinating community resources and referrals to ensure safe, appropriate care in the home.
Assisting families with funeral planning upon request.
Participating in IDT meetings, plan-of-care reviews, and team collaboration.
Working with Volunteer Services to align volunteers and coordinate Veteran pinning ceremonies.
Supporting process improvements and using Unity's specialty programs (Gifted Wishes, Holistic Harmonies, Respecting Valor).
Maintaining accurate, timely documentation and adhering to the Social Work Code of Ethics.

About the Program Externships are available to veterinary students in any year of school. Scheduling priority will be given to 4th year students. Student externs gain broad experience working in the Emergency department of a busy emergency/referral setting. Externs work in the ER department primarily unless a specialty focus is prearranged.
Externs shadow veterinary technicians, veterinary interns and emergency and critical care staff veterinarians. Externs are expected to be interactive with the staff, attend technician and DVM rounds when available, proactively discuss cases, and actively contribute to the general workload as appropriate. Hands on experiences are dependent on the individual, caseload and supervising veterinarians.
Availability permitting, externs can gain exposure to additional specialties during their externship.
Program Location
We currently offer opportunities at our Waukesha location in Wisconsin.
Program Schedule
Veterinary student externships are generally a 2-week block, although 1-4 weeks are also an option.
Externs work 8-12hr shifts. There are multiple shifts available, mostly swing and 2nd shift. The variability is to minimize overlap when more than 1 student is on during that week. WVRC has multiple DVMs and interns on, and our caseload is high enough to support all extern shifts.
About You
Applicants must be students of accredited domestic or international veterinary schools, pursuing a DVM, VMD or foreign equivalent. First, second and third-year veterinary students who are interested in emergency/critical care are encouraged to apply.

What you'll do:
The Social Worker for the Supportive Care Management program provides ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patients and family needs. The Social Worker will participate in the provision of hospice and bereavement services as needed. This role provides care and support to patients and families and will travel across Unity's full-service area.
1. Comprehensively develop and continuously assess patients' plan of care as well as the psychosocial needs of the patient, family and caregivers
2. Provide supportive, educational, and crisis counseling as appropriate to patients and their families
3. Engage in ongoing communication with patients, families, facility staff and team members to expedite the plan of care
4. Coordinate and participate in patient and family care planning meetings
5. Develop and document a thorough care plan through collaboration with the interdisciplinary team. Ensure the plan is modified based on the patient's changing needs
6. Exhibit flexibility and creative problem-solving for patients with complex psychosocial needs
7. Serve as a patient advocate. Enhance a collaborative relationship to maximize the patient's and family's ability to make informed decisions and be an active member of the interdisciplinary team
8. Serve as a resource and provide education to patient, family, facility staff, physicians, and professional staff relative to the psychosocial needs of patients and families
9. Assess the family unit of care for bereavement risk throughout care
10. Foster positive internal and external customer relations, positively representing the social work profession throughout the organization and in the community
11. Adhere to the Social Work Code of Ethics and consistently practice within a framework guided by ethical principles
12. Prepare for and participate in interdisciplinary team meetings as scheduled
13. Maintain positive relationships with contracted facilities
