We're looking for a service-oriented Help Desk Technician to join our IT team and ensure smooth daily operations for staff across our programs. This is an on-site position which requires you to be able to travel in and around all Unity House of Troy's locations.
Job Summary:
The Help Desk Technician provides first-level technical support for all Unity House staff. This includes managing IT equipment, onboarding/offboarding employees, supporting video conferencing, maintaining inventory, and managing devices using Microsoft Intune.
Key Responsibilities:
• Respond to help desk requests via phone, email, and ticketing system
• Troubleshoot hardware, software, printers, mobile devices, and connectivity issues
• Set up and maintain laptops, desktops, iPhones, and tablets
• Use Microsoft Intune to manage and secure devices (Windows, iOS, mac OS, Android)
• Assist with onboarding/offboarding processes (accounts, devices, email, etc.)
• Support Microsoft 365, Teams, SharePoint, Zoom, and other platforms
• Maintain IT inventory and accurate documentation
• Support special IT projects and organizational initiatives
Qualifications:
• High School diploma or equivalent
• 1-3 years of experience in IT support or help desk role
• Knowledge of Microsoft AD
• Proficiency with Windows, mac OS, iOS, Microsoft 365, Teams, Zoom, and Intune
• Strong troubleshooting
• Valid NYS Driver License and reliable transportation

Primary Job Functions
Provide overall support to youth to ensure that they are getting the services need to meet the overall health and wellness goals.

Assures that job-related activities are in compliance with Glove House policies and procedures, Department of Health, Care Management Agencies, State and Federal regulations, and relevant professional association, ethical standards, accreditation standards, and the law.
Perform care management tasks as defined by the Department of Health and contracted Care Management Agencies (i.e. CHHUNY) (may include assessments, goal plans, safety plans, and other assessments).
Link, advocate, and support youth and families by identifying current strengths and barriers while providing referrals and other interventions to assist with current needs such as psychosocial supports and linkages with medical, dental, and behavioral health care providers, as well as, educational, employment, transportation community resources.
Participate as a team member of Health Homes Department and the Finger Lakes Regional Office, supporting other teams when necessary.
Coordinate services with other professionals and paraprofessionals and liaise with outside social service agencies and other organizations, where appropriate.
Provide comprehensive, client-centered, trauma-informed, collaborative care planning for the development and management with the youth and parent/guardian to assist in the integration of medical and behavioral health services, and social health services.
Build and use effective communications strategies among peers, medical staff, addiction and mental health providers, and other community agencies using electronic assisted devices including Telehealth and other interactive technology.
Help improve, measure, monitor, and sustain quality outcomes that focus on clinical indicators/performance measures, patient satisfaction, and plan adherence.
Participate in interdisciplinary team meetings and conduct regular face-to-face contact with youth and families.
May monitor interns and/or volunteers.
Develop and maintain records and program documentation, such as assessments, care plans, visitation plans, progress notes and summaries, according to contract and Glove House standards.
Generate and maintain necessary reports and paperwork (i.e., Quality Assurance and program reports).

The Interventionist is a critical member of the HBCI (Home Based Crisis Intervention) team. The Interventionist is responsible for providing treatment to youths aged 5-20 years and their family/caregivers to address the clinical needs of the child and the complex needs of the family unit. The Interventionist will provide crisis intervention and stabilization with the child/youth and family, and case management services such as referrals to other services and ensuring the structural home environment is safe. Interventionists use individualized treatment interventions and evidence-based practices to address identified clinical and family system needs. The Interventionist will carry a caseload of up to 3 recipients.
Primary Job Functions
As a member of the HBCI team, provides crisis intervention and intensive case management to youths aged 5 - 20 years to ensure safety and stabilization with the child/youth and caregivers/family in the home, school, and community environments.
Works collaboratively with the HBCI team to respond to referrals and ensure the referred youth meets the criteria for receiving HBCI services as defined in HBCI guidelines provided by the NYS Office of Mental Health.
Carries a caseload of up to 3 families providing 4-6 weeks of intensive intervention to include meetings with the child/youth and parents/guardians 3-5 days per week.
Implements individualized treatment interventions developed collaboratively with the family and based on identified clinical and family system needs.
Works with the team to plan and facilitate group therapy and psychoeducation for parents as well as youths.
Works with the HBCI team to ensure implementation of crisis plan and after care plan is scheduled and agreed upon.
Participates in on-call rotation to ensure that crisis intervention services are available to families 24 hours per day, 7 days per week.
Works within the HBCI team to ensure post-discharge follow-up with families occurs in order to assess progress and to evaluate the program's success. Records follow-up data so that program success can be tracked over time.
Utilizes vast array of referral sources, linking families and youth to services available to them including CHHUNY, Parent Education, Outpatient Counseling Services, and School-Based services.
Participates in community meetings as needed, networking and becoming familiar with referral sources and community resources available to youth and their caregivers.
Contributes to agency committees, participates in provision of agency-wide employee wellness.
Is required to safely travel on agency business, as required by job demands. Maintain accurate mileage documentation and submit in required agency timeframes.
Consistently attends mandated agency and department meetings and participates on standing agency committees and community committees as assigned.

Education, Licenses and Certifications
Bachelor's degree in social work or human service related field required.
Must have a valid driver's license and driving record in accordance with agency standards.
Experience
2+ years direct Human Services, crisis management, person-centered services and community-based care experience required.
