Responsibilities We are looking for PRN Intake Counselors to join our team! PRN Night The Intake Counselor provides telephonic and in person risk assessment and crisis intervention, initial contact with referral sources and potential patients, coordinates the referral and intake process, determines appropriate level of care, facilitates the admission process and maintains communication with referral sources, families, patients and unit staff. The Intake Clinician also assists the Physician by providing face to face clinical assessments, including the history of presenting illness, family dynamics and supporting psychosocial information.
Qualifications
Minimum Requirements:
* Master's Degree in Counseling, Psychology, Human Services field, or Masters in Social Work required.
* Licensure through the Arizona Board of Behavioral Health Examiners preferred.
* Prior intake, assessment, admission, and crisis intervention experience with a psychiatric mental health population, preferably in an inpatient setting.
* Must have or be able to obtain level one fingerprint clearance card.
Quail Run Hospital is a 102-bed, acute care psychiatric hospital located in Phoenix, Arizona. Quail Run features individual units for adolescents, adults, and seniors, and offers inpatient acute care, partial hospitalization, and intensive outpatient programs. Exercise Physiologist Full Time Days Position Summary
* Supervising and monitoring outpatient cardiac rehab Phase 2 and 3 -observation and evaluation of adult chronic cardiovascular disease patients during exercise.
* Monitoring and assessing blood pressure and EKG rhythm strips during exercise session
* Completing individualized treatment plans including: risk factor assessment, risk factor management, goal setting and exercise prescription.

Qualifications
EDUCATION
Master's degree in Social Work, Psychology, Counseling, Behavioral Health or related field.
LICENSURE/CERTIFICATION
Current license in state as a Licensed Professional Counselor ("LPC" or "LPC-A"), Licensed Marriage and Family Therapist ("LMFT"), Licensed Master Social Worker ("LMSW"), Licensed Clinical Social Worker ("LCSW"), or Registered Nurse ("RN").

Position Summary
