The Pediatric Nurse Case Manager coordinates resources and services in response to individual patient's needs and fiscal realities, performs utilization review activities and oversees coordination of complex discharge planning for assigned case load. The Nurse Case Manager is an active member of the interdisciplinary team contributing to team goal setting. The Nurse Case manager acts as a consultant to the clinical team, service lines and other departments and participates in program development and quality improvement initiatives. The Nurse Case Manager provides age-appropriate assessments, interpretation of data, and delivery of interventions. Demonstrates the ability to work with any patient regardless of race, gender, religious affiliation, cultural beliefs, lifestyle, and disease process or treatment plan.
The final salary and offer components are subject to additional approvals based on UC policy.
Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. Your Role on the Team Position Summary: Incumbent is responsible for a high level assessment and reassessment process for the patient/family to set priorities, plan, organize, and implement care that is goal-directed toward self-care outcomes.

Time spent in the other areas of expertise will be a mix of inpatient and outpatient child neurology focused in their area of specialty interest if applicable. The position is based in the Division of Child Neurology, and will work in a team setting, bridging neurology, neuro-oncology, pediatric oncology, neuroimmunology, critical care, and neurosurgery.

The successful applicant should have ABPN Board Certification, at least a two-year Movement Disorders Fellowship, or equivalent, currently being completed or previously completed, clinical experience and expertise diagnosing and treating a wide range of movement disorders, be able to perform botulinum toxin injections for movement disorders, perform assessments to determine eligibility for neuromodulation interventions, such as Deep Brain Stimulation and Focused Ultrasound, and perform Deep Brain Stimulation programming.
We are particularly interested in candidates who have clinical trials or clinical research experience, including performing clinical research assessments.
