CHARACTERISTIC DUTIES AND RESPONSIBILITIES * Provide clinical care to adult patients with acute and chronic medical conditions * Provide guideline-concordant preventive health services * Obtain accurate medical histories and perform appropriate physical examinations. * Prescribe medications and treatments * Order, follow up, and interpret diagnostic tests * May perform office procedures including the suturing and care of wounds, removal of foreign bodies, ear and eye irrigation, skin biopsy, mole or wart removal, toenail removal, arthrocentesis, incision and drainage of abscesses. * Ensure timely completion of medical records, including progress notes, and updating problem lists and health maintenance records * Ensure timely completion of visit coding and billing * Ensure timely responses to patient and staff questions * Instruct and counsel patients regarding behavioral and lifestyle interventions * Refer patients to appropriate specialty and subspecialty physicians as indicated by the patients' problems. * Maintain awareness of health facilities, agencies and resources in order to facilitate the appropriate referral of patients. * Order and interpret EKGs. * Direct other medical personnel and health professionals in the execution of patient care. * Assist with and participate in clinical research and/or trials. * Participate in committees within the division, department, College of Medicine and University of Iowa Health Care. * Maintain effective working relationships with faculty, staff, students and the public. * Establish and maintain standards of collaborative interaction among peers and employees that is characterized by respect, honesty, and service; assure that all unit members are held to similar standards and ethics; constructively manages conflict. * Inspire and motivate others to high performance by exercising strong stewardship of University resources, setting expectations, measuring success through individual performance evaluations, and driving organizational results. * Identify opportunities for and create development plans that encourage employees to attend to the growth of their personal and professional capacity; engage self and staff in collective reflection of the University's greater role in society.     The Psychosocial Oncology Program (POP) is a team of multidisciplinary professionals working with children and families affected by cancer. Commonly known as the POP Team, our mission is to help children, teens, young adults, and their families navigate emotional, social, and cognitive concerns after cancer diagnosis. The UI Stead Family Department of Pediatrics is seeking an APP to join our division of Hematology-Oncology as a member of the POP Team. In this role, the APP works in an integrated care model with the medical oncology team to assess and facilitate management of mental health aspects of pediatric cancer care. POP APPs perform comprehensive biopsychosocial assessments for children following cancer diagnosis and periodically throughout the cancer trajectory. Based on assessment findings, the APP facilitates appropriate referrals and connection to several psychosocial resources. The APP provides therapeutic support with all visits and, when necessary, prescribes and manages psychiatric medications as an intervention. This position will be required to work either Monday through Friday from 8 am to 5 pm or 4 days per week 10-hour days.     BASIC FUNCTION AND RESPONSIBILITY Provide clinical evaluation and ongoing medical follow-up for patients with chronic renal failure under the supervision of the Internal Medicine Nephrology Physician primarily in the Nephrology Outpatient Clinic and Center and Satellite Hemodialysis Unit sites. Services will include participating in the outpatient services by assisting in the management of pre-ESRD patients and other renal diagnoses, supporting patients receiving chronic renal dialysis, and working with surgeons and interventional radiologists for vascular access. Services will also include participating in the care of ESRD patients undergoing Hemodialysis as part of the UI Healthcare System. This will be a full-time outpatient position but will require at least one year of training on the inpatient ESRD service to clinically orient and prepare for the role in MSC. CHARACTERISTIC DUTIES AND RESPONSIBILITIES: Clinical: * Function independently and/or collaboratively as a member of an interdisciplinary team to plan, deliver, and evaluate health care for renal patients. * Provide clinical services and continuity of renal care in the Nephrology Outpatient Clinic. * Make rounds on a regularly scheduled basis for dialysis outpatients, including regular travel to the satellite hemodialysis units. This includes hemodialysis and peritoneal dialysis patients at our home dialysis unit. Address health problems of outpatients, write prescriptions, participates in regular patient reviews, and patient care conferences. * Write initial dialysis orders, alter existing dialysis orders per patient labs, or as medically necessary. Assess, examine, and write dialysis orders for transient dialysis patients. * Practice preventive medicine, establish and follow guidelines which are age appropriate for routine health maintenance for patients on chronic dialysis. * Order appropriate lab and diagnostic tests, review monthly lab tests and medications, interpret results and recommend or consult with primary nephrologist and other members of the health care team (dietitian, pharmacist, social worker, etc.) to make appropriate changes in dialysis prescription, medications, or diet. * Handle telephone calls from dialysis outpatients and care providers concerning changes in medical condition, medication adjustment, and prescription renewals. Communicate with patients, primary physician and nephrologist as needed regarding changes in medical status and treatment plan. * Collaborate with nephrologist and pre-ESRD advanced practice nurse in management of pre- ESRD patients. CHARACTERISTIC DUTIES AND RESPONSIBILITIES Patient Assessment and Diagnosis: * Conduct comprehensive cardiovascular assessments, including detailed history-taking, physical examinations, and review of diagnostic imaging and lab results. * Diagnose and manage conditions such as coronary artery disease, heart failure, arrhythmias, valvular heart disease, hypertension, hyperlipidemia, and peripheral vascular disease. * Evaluate and stratify cardiovascular risk using evidence-based tools and guidelines. Clinical Management and Treatment: * Develop and implement individualized treatment plans in collaboration with cardiologists and multidisciplinary teams. * Prescribe and titrate cardiac medications including beta-blockers, ACE inhibitors, anticoagulants, antiarrhythmics, and lipid-lowering agents. * Monitor therapeutic response and adjust treatment plans based on clinical findings and diagnostic results. * Order, interpret, and act upon results from diagnostic tests such as: * Electrocardiograms (ECGs) * Echocardiograms * Stress testing (exercise and pharmacologic) * Holter and event monitors * Cardiac CT and MRI Patient Education and Preventive Care: * Provide education on cardiovascular disease prevention, lifestyle modifications, medication adherence, and symptom management. * Counsel patients on smoking cessation, dietary changes, physical activity, and weight management. Care Coordination and Follow-Up: * Coordinate care transitions from inpatient to outpatient settings, ensuring continuity and adherence to discharge plans. * Collaborate with primary care providers, cardiologists, cardiac surgeons, dietitians, pharmacists, and social workers to optimize patient outcomes. Professional Practice and Quality Improvement: * Stay current with emerging cardiovascular therapies and technologies through continuing education and professional development. * Adhere to ACC clinical competencies and institutional protocols for cardiovascular care delivery. * Participate in clinically relevant activities and attend UI Health Care APP council meetings and other conferences to enhance clinical and professional expertise. * Provide education and serve as a resource for nursing staff, medical and advanced practice provider students, and residents in area of clinical expertise * Serve in a consultative resource capacity by providing expert knowledge and behavioral skills that impact clinical practices and patient outcomes The department intends to hire for one position as either an ARNP, requisition 25005933 or PA, requisition 25006066, based on the qualifications of the successful candidate.     BASIC FUNCTION Provide patient care for the adult outpatient Oncology service, a multi-specialty service, the Physician Assistant performs direct patient care, including diagnostic and therapeutic services. Within the APP/Physician relationship, the PA will exercise autonomy in medical decision making and services provided. Plan, deliver, and evaluate the health care required by the patient, consistent with accreditation guidelines. In addition to the ability to exercise autonomy and function independently, the individual needs to be able to work under the direction of the attending physician. Patient care responsibilities will include working weekday clinic hours. This position is part of a dynamic and fast paced environment. We utilize a multidisciplinary approach to patient care that encourages team work between multiple disciplinary team to provide the most comprehensive care to our patients. DUTIES AND RESPONSIBILITIES * Perform overall medical care and medical management in the Holden Comprehensive Cancer Center Clinic under the direction of the attending physician, exercising independence in medical decision-making and services provided primarily in a clinic setting under a collaborative practice agreement. * Perform and document initial evaluation of new and returning patients, including complete history and physical examination and, appropriate orders for treatment. This includes initial evaluations of patient symptoms. Provide specialized care independently or jointly with attending physician as appropriate. * Collaborate/consult in the optimization of care delivery through coordination and transitions of care as a part of a team.
