Responsibilities*
The primary work setting for this position is in the operating room for the majority of the time where the individual will have responsibilities that include first/second assisting, conduit harvesting, and evaluation/expansion of minimally invasive equipment/approaches. The ideal candidate should have 3-5 years of cardiac surgery experience with strong interpersonal talents and technically skilled in the operating room. The cardiac surgery PhysicianAssistant exercises autonomy in medical decision making and provides a broad range of diagnostic and therapeutic services to adult heart patients. This may include managing patients with complex, acute, critical and chronic health conditions. In addition, the individual collects and assists in the interpretation of reports from diagnostic and laboratory tests, radiographic procedures and clinical studies. The position typically analyzes, compares and evaluates various courses of action and has the authority to make independent decisions on matters of significance, free from immediate direction, within the scope of their responsibilities.
Required Qualifications*
* Satisfactory completion of a PhysicianAssistant program accredited by Accreditation Review Commission on Education for the PhysicianAssistant (ARC-PA) or one of its predecessor agencies, CAHEA or CAAHEP.
* EVH and radial artery harvesting
* Certification by the National Commission on Certification for PhysicianAssistants (NCCPA).
* Current license to practice as a PhysicianAssistant in the State of Michigan.
* ACLS and BLS certification
* Current DEA registration required
* State of Michigan Controlled Substance License
* 3-5 years cardiac surgery experience
* Currently participating intraoperative portion of cardiac surgery.

Job Summary:
The Department of Ambulatory Dialysis at SUNY Downstate Health Sciences University is seeking a full-time TH PhysicianAssistant 1. The successful candidate will:
* Have attendance and availability for daily rounds on the shifts assigned.
* Conduct weekly notes on the dialysis patients; thus, at least four notes on each patient assigned; each month, one note must be detailed, addressing all the lab data and discussions with the patient about any changes made to the dialysis prescription.
* Competency will be judged by the ability of the PhysicianAssistant to detect abnormal lab data, physical assessment, and vital Sign reasoning as well as documentation of the corrective action.
* Perform admission histories and physicals and repeat yearly physicals on the assigned patients in a timely manner.
* Perform initial plan of care and individual patient assessments in a timely manner within 15-days of admission, with proper follow up for unstable patients as required under current CMS guidelines.
* PhysicianAssistant will be assessed on the ability to identify unstable patients and their desire to see that the plan of care is changed accordingly. Any close follow up for the unstable patients has to be arranged by the PhysicianAssistant for their respective unstable condition, documented in the progress notes.
* Order and review radiological studies, request appropriate consults and discuss with patients follow up of abnormal results. Document non-compliance when studies were not followed out by patient.
* Be available for prescription writing, address questions by the nursing staff for patient care related issues and any other consultative needs that the patient might have. The number of complaints received from the staff regarding availability and patient satisfaction responses will be used to assess PhysicianAssistance performance.
* Have regular attendance and willingness to participate in QA and PQI programs to enhance patient care as well as their well being.
* Conduct medication reconciliation monthly on notes and recertification of VNS/ Horne health agencies. Communicating with VNS services for continuity of care.
* Must have regular availability as the business needs require for the safe operational function of the unit.
* Conduct a full write up of new patients for presentation at the monthly QA with special emphasis on patients with catheters, whether vein mapping has been done or scheduled, whether surgery for AV access has been arranged, and whether a follow up of the patient has been scheduled post AV access placement for intervention, if not developing properly. PhysicianAssistant notes will be audited to determine whether or not AV Fistula development is being monitored and follow up appointments are being made. All new patients should have a clinical summary presented at QA for a minimum of three months (90 days), after which they can be removed from the QA description of "new patients" to make room for other new patients. The assigned PhysicianAssistant must inform the vascular access manager ahead of time of the status of their patients AV access and catheters so that the vascular access manager can have the reports prepared for IPRO in a timely manner. All new patients should have their immunization, HIV status and hepatitis A, B and C reviewed are reported to QA.
* Conduct medication order entry in a timely manner whenever there is a change in schedule of a patient for the purposes of holidays, or patient showing up on a non-dialysis day.
* Be accountable for responding to all lab data and reporting such in their notes, especially if the labs were out of range and critical in nature. Response of the PhysicianAssistant as to how they dealt with abnormal labs and addressed them with the patient will be utilized for their evaluation. All labs need to be documented in the notes even if they were done on a day different from the monthly lab.
* Conduct timely completion and presence during care plan meetings which are held every Thursday for proper completion of the care plans for you respective patients assigned to you. If the patient moved in the month assigned to you to a different shift, the responsibility is still with you for that month to finish the care plan for that particular month. This will hold true for patients that are deemed unstable and require monthly care plans for three months to determine whether their condition is deteriorating or improving.
Required Qualifications:
* Graduate of an AMA approved PhysicianAssistant Program or Baccalaureate or higher degree program that meets the licensure requirements in the State of New York.
* New York State PhysicianAssistant Licensure.
* National Certification in New York or in another state with reciprocity.
Preferred Qualifications:
* 2+ years of experience working with CKD patients, patients with ESRD on dialysis and patients who have received a renal transplant.

* Perform a broad spectrum of care regarding critical thinking, diagnosis, ordering of medications and diagnostics, and performance of corrective, preventive and therapeutic measures in the inpatient, surgical, and/or outpatient setting.
* Assume medical staff privileging under the scope of practice of a delegated physician determined by UHB system, and the department of hire.
* Be responsible for patient care, inclusive of patient assessment, management, diagnosis, treatment planning and ordering of pertinent tests, therapeutics and medications as noted by NYS law and hospital policy/by-laws (includes controlled substances).
* Write accurately, and in a timely manner into patient records for all pertinent care, status changes, orders, progress notes, procedure notes and admit/discharge notes as designated by the clinical area of hire.
* Demonstrate ability to work and communicate effectively with medical teams and other associated staffing.
* Provide consultation to nursing and ancillary medical teams and staff of patient condition and plan of care.
* Work collaboratively with supervising physician(s) and care team in a timely manner with respect, and with the goals of patient-centered safety and quality as a priority.
* Perform delegated clinical duties and tasks effectively and efficiently.
* Practice within scope of the Supervising Physician.
* Communicate assessment, treatment and care planning to patient, families and designated caretakers in effective and compassionate manner.
* Facilitate patient continuity of care across multi-disciplinary services, and participate in effective, safe and efficient throughput of patients.
* Demonstrate knowledge and adherence of clinical and institutional policy and procedures related to clinical practice and hospital/practice site operations.
* Demonstrate competency and continued practice in privileges associated to area of clinical care.
* Maintain required certifications and licensure as per medical staff policy, and maintain active UHB privileges as delegated by Medical Staff Services and Chair for the department.
Required Qualifications:
* Bachelor's Degree from an accredited PhysicianAssistant Program.
* New York State PhysicianAssistant Licensure and Registration.
* BLS and ACLS Certification.
Preferred Qualifications:
* National Certification by AAPA preferred.
* 1 to 2 years of surgery experience preferred.
* Experience working in conjunction with the attending surgeon and, non-surgical providers for medical care of surgical patients preferred.
