    The RN Case Manager will be responsible for facilitating the discharge planning process for patients across various inpatient units. The position requires a strong understanding of acute care management and the ability to work collaboratively within a multidisciplinary team. Responsibilities Conduct medical discharge planning for a diverse caseload of 15-20 patients daily. Collaborate with the Social Work team and consult with various departments as needed. Utilize navi Health for referrals and prepare for the transition to EPIC Care Management. Ensure appropriate patient placements in alignment with established criteria. Required Experience / Certifications / Licensure Active RN license with at least 2 years of acute care management experience. Experience in discharge planning within a level 1 or large academic medical center. Proficiency in EPIC charting is mandatory.
