Uses data to drive decisions and plan/implement performance improvement strategies related to case management for assigned patients, including fiscal, clinical and patient satisfaction. Collects, analyzes and addresses variances from the plan of care path with physician and/or other members of the healthcare team. Uses concurrent variance data to drive practice changes and positively impact outcomes. Discusses payor criteria and issues on a case-by-case basis with clinical staff and follows up to resolve problems with payers as needed. Uses quality screens to identify potential issues and forwards information to clinical quality review department. Completes utilization management and quality screening for assigned patients. * Works collaboratively and maintains active communication with physicians, nursing, and other members of the inter-disciplinary care team to effect timely, appropriate patient management and eliminate barriers to efficient delivery of care in the appropriate setting. Addresses/resolves system problems impeding diagnostic or treatment progress. Proactively identifies and resolves delays and obstacles to discharge. Utilizes conflict resolution skills as necessary to ensure timely resolution of issues. Must be enrolled in externship course at nursing school program during time of externship. . BSN student preferred. Certifications, Licenses or Registration Required: NA Knowledge, Skills & Abilities Verbal and written communication skills. Interpersonal skills. Ability to comply with Mississippi student nurse externship program guidelines. Ability to comply with safety, environmental, and infection control standards and practices. Ability to work in a team environment. Responsibilities Assists with initial and ongoing assessments; develops an understanding of collecting, reporting, and documenting data related to patient care activities. Assists with treatments and other delegated tasks under the direct supervision of a registered nurse. Provides hygiene and comfort measures to patient such as feeding, drinking, ambulating, grooming, toileting, dressing, and socializing. Contributes to the assessment of patients, their needs, and plan of care under direct supervision of a registered nurse. Develops the ability to prioritize and organize patient care. Practices the principles of maintains a safe patient care environment; complies with safety, environmental, and infection control methods. Performs as an integral component of the health care team. Participates in unit meetings and continuing education programs. Performs other duties as assigned. Physical and Environmental Demands Requires frequent exposure to unpleasant or disagreeable physical environment such as high noise level and exposure to heat and cold, frequent handling or working with potentially dangerous equipment, frequent exposure to biohazardous conditions such as risk of radiation exposure, blood borne pathogens, fumes or airborne particles, and/or toxic or caustic chemicals which mandate attention to safety considerations, occasional working hours significantly beyond regularly scheduled hours, occasional travelling to offsite locations, frequent activities subject to significant volume changes of a seasonal/clinical nature, frequent work produced subject to precise measures of quantity and quality, frequent bending, occasional lifting and carrying more than 100 pounds, occasional climbing, occasional crouching/stooping, occasional kneeling, occasional pushing/pulling, frequent reaching, occasional sitting, frequent standing, frequent twisting, and frequent walking.
