Responsibilities:

Qualifications:
* Graduate from a physical therapy curriculum approved by: The American Physical Therapy Association; or The Committee on Allied Health Education and Accreditation of the American Medical Association; or The Council on Medical Education of the American Medical Association and the American Physical Therapy Association; OR Prior to January 1, 1996: Was admitted to membership by the American Physical Therapy Association; or Was admitted to registration by the American Registry of Physical Therapist; or Has graduated from a physical therapy curriculum in a 4-year college or university approved by a State department of education; OR Has 2 years appropriate experience as a physical therapist and has achieved a satisfactory grade on a proficiency examination conducted, approved, or sponsored by the U.S. Public Health Service except that such determinations of proficiency do not apply with respect to persons initially licensed by a State or seeking qualification as a physical therapist after December 31, 1977;ORWas licensed or registered prior to January 1, 1966, and prior to January 1, 1970, had 15 years of full time experience in the treatment of illness or injury through the practice of physical therapy in which services were rendered under the order and direction of attending and referring doctors of medicine or osteopathy; OR If trained outside the United States, was graduated since 1928 from a physical therapy curriculum approved in the country in which there is a member organization of the World Confederation for Physical Therapy; OR Meets the requirements for membership in a member organization of the World Confederation for Physical Therapy.

We anticipate hiring at the Associate Professor or Professor rank, but will also consider exceptional candidates at the Assistant Professor stage, especially those nearing completion of their tenure review. **Responsibilities:**
GENERAL PURPOSE
Primarily responsible for providing Targeted Case Management and Psychiatric Rehabilitation services across Strong Minds. This hybrid model of care is intended to support the wellness and recovery goals of individuals with complex and/or chronic behavioral health issues and needs by implementing targeted interventions designed to provide timely, high-quality, and efficient care. The CCBHC TR will provide case management services to support patients around goals aimed at providing self-sufficiency. Additionally, the RT performs psychosocial rehabilitation services designed to assist the individual to overcome mental health and substance abuse barriers. The intent of this role is to assist the individual to restore the individual's functional level to the fullest possible. The primary focus of these services with the individuals includes assistance with vocational and educational goals.
Under general direction and with considerable independence, performs several care management and psychiatric rehabilitation services consistent with CCBHC and the Department of Psychiatry Policy. Establishes and maintains cooperative working relationships with community providers and resources to obtain needed services and support for enrolled patients. Works as part of an interdisciplinary team to support activities to engage patients in care, connection to care management, vocation and education resources and ensures necessities are met.
Utilizes community and family resources to create sustainable support systems for patients. Reviews and discussed plan with patient and the treatment team, focusing on linking individuals to clinical and social services within the system and community providers.
**ESSENTIAL FUNCTIONS**
+ Conducts assessment of identified patients for psychiatric rehabilitation and care management services.
+ Designs appropriate treatment plan goals according to patients' psychiatric rehabilitation needs.

* Performs physical therapy assessment, reassessment and treatment and supervises physical therapist assistants in the treatment of pediatric, adolescent, adult, and geriatric patients referred by a physician to the clinic. Develops plan of care including goals and treatment plan in accordance with physician referral, nature of injury/illness and results of physical therapy assessment. Modifies or adjusts plan of care based on patient's response to treatment and/or changes in patient's status. Works PTAs to adjust patient programs as patient status changes.
* Rotates through selected treatment areas when necessary to facilitate organizational needs.
* Attends and actively participates in staff meetings and in-service education programs.
* Maintains written documentation for all physical therapy services rendered including documentation of initial assessment, follow-up visits, and discharge summary consistent with department standards. Co-signs physical therapist assistant documentation biweekly.
* Completes necessary charge and statistical information to accurately reflect the service rendered to the patient.
* Provides verbal and/or written home instructions to patient and/or family members when applicable.
* Schedules patients to achieve and maintain a level of productivity which meets or exceeds the standard established for the facility by the organization while ensuring delivery of quality patient care.
Qualifications:
EDUCATIONAL/KNOWLEDGE REQUIREMENTS:
* Graduate of an accredited program approved by the American Physical Therapy Association (APTA).
Licensure, Certifications, and Clearances:
Active license in practicing state required. CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire.
* Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
* Physical Therapist (PT)
* Act 34
