**Responsibilities:**
GENERAL PURPOSE:
Under the direction of leadership, is accountable for provision of competent direct patient care based on established standards. Contributes to unit activities and supports enhancement of the care delivery system at the unit level. Supports peers in the delivery of patient care services and assumes an expanded role which may include charge nurse, preceptor, and committee member responsibilities.
ESSENTIAL FUNCTIONS
- Performs and documents patient care, medication administration, care plans, and patient education in a competent and efficient manner, following all policies and procedures. Identifies patient problems and collaborates in developing a plan of care based on existing standards and long-term goals. Implements age-specific, culturally, and spiritually competent nursing interventions to address patient/family needs and response to procedures, therapies, and medications. Evaluates basic and complex patient/family response to procedures, therapies, and medications, identifying concerns/issues and revising plan of care based on patient and/or family response. Recognizes changes in patient status and responds effectively. Communicates change in patient status to appropriate members of the care team.

MINIMUM EDUCATION & EXPERIENCE
- Associate's degree in Nursing and 6 months experience as a RegisteredNurse working in an Acute Care Setting required
- Bachelor's degree in Nursing and preferred

Position Summary
The Clinical Coordinator RN will work in the Adult and Pediatric Center for Liver Disease & Transplantation within the Division of Abdominal Organ Transplantation in the Department of Surgery. The position will work under the direction of the Senior Nurse Practitioner, Medical and Surgical Directors of the CLDT and in collaboration with Pediatric Hepatologists, Surgeons, Nurse Practitioners, Physician Assistants who see patients at all sites throughout NY and NJ. Responsibilities
* Completion, documentation and ensuring compliance of principal components of the laboratory and radiology testing and patient follow-up; providing expertise in management of immunosuppression under the supervision of attending NP and Hepatologist.
* Support on-going clinical functions of the service to provide compassionate patient-centered care.
* Coordinate the evaluation process for potential transplant candidates with the multidisciplinary team.
* Coordinate patient consults for transplant evaluation.
* List and maintain patients on liver/intestinal transplant waitlist.

Position Summary
Columbia Primary Care at Columbia University Irving Medical Center (CUIMC) is seeking a Primary Care Nurse III (RN) to join our team. The Primary Care Nurse III will report directly to the Practice Manager or Director of Nursing, Columbia Primary Care. This ambulatory nursing position will serve as a key member of the patient care team to support the pediatric and/or adult populations we serve. The Primary Care Nurse III performs nursing duties consistent with the NYS nursing scope of practice related to the support of the day-to-day clinical operations of the assigned practice. These duties include, but are not limited to, medication/immunization administration, point of care testing (POCT), clinical information abstraction, oversight of the practice(s)' infection prevention and control procedures, and patient education that can be in person, via our patient portal, or by phone. Reporting directly to the Director of Clinical Operations, the Clinical Coordinator I - RegisteredNurse, assists physicians with patient care, health education/promotion, electronic medical record (EMR) chart management, coordination of patient care and clinical protocols as needed in the Department of Neurological Surgery at Columbia University Medical Center.
Responsibilities
* Assesses and collects data related to patient health status an inputs this medical information into the EMR as instructed by the physician.
* Works with the care team to develop the plan and care across the continuum (e.g. inpatient, outpatient, home, skilled nursing facility, schools) which includes assisting with medication managmeent (e.g. medication renewals) inclusive of medical prior authorizations as needed. Administer immunications and medications..
* Under limited supervision, executes nursing interventions by collaborating with the care team and following standards of care establisehd by the unit/department.
* Supports on-going clincal functions of the service to provide compassionate patient-centered care.
* Handle telephone, My Chart and in-person triage (for symptom management) as well as patient education.
* Assists with management of urgent and emergent patient situations in conjunction with care team leadership.
* Coordinates multidisciplinary health care services and arranges diagnostic tests (i.e. lab, x-ray) as ordered by physicians and performs diagnostic testing, therapeutic, and interventional procedures as needed under physician supervision.
* Reviews and act on test results (lab, x-ray).
* Reviews and manages communications from internal and external "customers" inclusive of inbasket messages, updating immunizations, updating health maintenance items.
* Ensures infection control compliance with in the practice.
* Addresses appropriate measures to provide patient and family satisfaction.
* May float to other clinics on as needed for operations need.
* May precept new clinical staff including Medical Assistants, LPNs and more junior nurses.
* Perform other related duties and responsibilities as assigned/requested.
Minimum Qualifications
* Bachelor's degree in Nursing or equivalent in education and experience, plus 0-2 years of related experience.
* BLS certified.
* Current New York State RN license.
