**Responsibilities:**
GENERAL SUMMARY
With great latitude and the expectation to exercise independent clinical judgement to support patient movement: The Transfer Center /Admitting Clinical Coordinator RegisteredNurse (TC/ACC RN) provides direct support to Patient Flow throughout SMH and the Affiliate Hospitals. The TC/ACC RN is accountable for 24/7 patient flow. The TC/ACC RN participates in the management of patient triage, patient movement, mentoring house staff (RN and MD) and Regional MD's concerning patient transfers, the development and evaluation of census management strategies, and the monitoring of patient flow outcomes. The TC/ACC RN is accountable for supporting standards of care and providing consultation and expertise in the placement of specialty patients (Cardiology, Neurology, Transplant, Oncology, etc.) as well as managing placements of General Adult Services and Emergency Department patients. The TC/ACC RN is involved in activities at the service, institutional, and regional level, and contributes to goal achievement through personal action, effective communication and the facilitation of others. The TC/ACC RN participates in community and professional organizations, demonstrating commitment to the promotion of URMC visibility and future.
Shift rotation of days, evenings and nights required. Night shifts have option of remote work location.
**ESSENTIAL FUNCTIONS**
**Clinical Practice** :
+ Utilizes Clinical skills to Manage patient flow in an expert manner, following policies and procedures /recognizes trends or subtle changes inpatient /family response to care and responds effectively.
+ Identifies complex patient problems and collaborates in developing a plan of care based on best practice standards and service availability; determines best patient placement based on expert clinical knowledge, current occupancy and staffing.
+ Identifies patient safety issues and implements action plans or educational training based on needs.
+ Demonstrates exemplary unit/service standards of documentation.
+ Performs ongoing comprehensive assessment of regional, institution, department, service components that contribute to patient flow.
+ Provides information to update practice standards and activities based on best evidence.
+ Evaluates the effectiveness of patient placement based on clinical needs and revises as indicated.
**Clinical Coordinator:**
+ Functions effectively with clinical staff as an expert Patient Flow Manager **.**

**Clinical Enterprise:**
+ Assist Clinical Departments in implementing patient flow improvements.
+ Supports Flow initiatives i.e. Capacity Management Better Flow initiatives.
+ Supports Nursing, Specialty Service and General Services in understanding census limitations.
+ Runs effective Patient Flow, Bed Meetings, maintains patient center focus for bed placement.
+ Makes critical decision s in a timely manner exhibits knowledge of, and ensures compliance with Hospital Policy, Federal and State regulatory requirements.
+ Initiates appropriate follow up when concerns are identified.
+ Acts as on-site resource for Administrator Call for untoward situations requiring rapid patients' movement.
**Regional Enterprise** :
+ Reviews and evaluates regional needs for patient transfer to and between Affiliate Hospitals.
+ Leverages clinical knowledge and business acumen to make appropriate decisions when transferring patients within SMH, Intra Affiliate, and Regional Institution.

Position Summary

Position Summary
The Clinical Coordinator III - Nurse Practitioner will function in the outpatient setting of the Kidney/Pancreas Transplant Program in collaboration with the Kidney/Pancreas Transplant multidisciplinary team to care for and manage post-transplant kidney and pancreas recipients, post-operative kidney living donors, and patients who underwent surgical or invasive procedures by the Kidney/Pancreas transplant surgeons. Responsibilities
* Coordinate surgical and medical management of kidney and pancreas transplant patients in the outpatient setting.
* Manage living donor follow up in collaboration with living donor coordinators.
* Coordinate care of surgical patients that undergo hernia repairs, dialysis catheters, and other elective surgical procedures conducted by our kidney and pancreas surgeons.
* Assisting transplant surgeons and nephrologists in the outpatient clinic to perform comprehensive history and physical exams, develop differential diagnoses, implement plan of care and document patient's response to care.
* Conduct pre-operative history and physical exams, as well as obtain transplant and other surgical consents.
* Provide direct patient care by evaluating patients awaiting transplant, as well as post- transplant recipients, and kidney donors.
* Collect, order and interpret lab work and diagnostic studies under agreed upon protocols.
* Order and obtain medication prior authorizations for kidney transplant recipients.
* Develop practice protocols, standard of care, and patient education material for kidney/pancreas transplant recipients.

* In partnership with Quality, Research and Education group, help develop and manage detail clinical databases to evaluate and improve patient outcomes.

Position Summary
Columbia Primary Care at Columbia University Irving Medical Center (CUIMC) is seeking a Primary Care Nurse III (RN) to join our team. The Primary Care Nurse III will report directly to the Practice Manager or Director of Nursing, Columbia Primary Care. This ambulatory nursing position will serve as a key member of the patient care team to support the pediatric and/or adult populations we serve. **Responsibilities:**
GENERAL PURPOSE:
Serves as the primary internal contact for national and state registry data management questions, issues or concerns, but not limited to clinical data collection, validation, delivery, reconciliation, and data management activity timeliness. Concurrently and retrospectively reviews medical records and data to identify patient care, process, and system issues (hospital and external) related to the care of the patients. Analyzes reports and clinical data to identify trends impacting quality outcomes. Works closely with the Hospital Quality Office, the Departmental Quality Teams and providers/staff from the Division to identify opportunities for clinical safety and quality improvement, and works with the team developing initiatives to improve quality outcomes.
ESSENTIAL FUNCTIONS
- Utilizes national and state registries and other data sources to analyze patient outcomes, identify patterns and trends in care delivery to create safety/quality initiatives for patients. Participates in root cause analyses and creates required documents as needed. Completes audits on patient records and clinical documentation for quality purposes. Serves as a liaison to outpatient clinics. Collects patient reported outcome measures, which may contain patient contact via telephone, email, or face to face interaction.
- Develops project work-plans using Quality Improvement principles. Utilizes evidence-based practice in facilitating ongoing quality improvement efforts to drive better patient outcomes. Participates in initiatives for LOS reduction. Leads multidisciplinary teams on quality improvement initiatives. Leads and facilitates relevant conferences in collaboration with appropriate staff. Regularly reports quality improvement activities and quality outcomes to providers and clinical staff. Maintains knowledge of current trends, issues, and research in Quality Improvement. Participates in other Quality/PI work as assigned.
- Accurately identifies patients for inclusion in the national and state databases through the application of strict inclusion/exclusion criteria and protocols. Performs meticulous chart review and enters clinical data into the quality databases to identify opportunities for improved outcomes, reduced cost, improved LOS, and best practice utilization of resources. Performs complete and accurate collection of preoperative, operative, and postoperative data through the effective utilization of the EMR and other Hospital databases.

MINIMUM EDUCATION & EXPERIENCE
