Responsibilities * Coordinate surgical and medical management of kidney and pancreas transplant patients in the outpatient setting. * Manage living donor follow up in collaboration with living donor coordinators. * Coordinate care of surgical patients that undergo hernia repairs, dialysis catheters, and other elective surgical procedures conducted by our kidney and pancreas surgeons. * Assisting transplant surgeons and nephrologists in the outpatient clinic to perform comprehensive history and physical exams, develop differential diagnoses, implement plan of care and document patient's response to care. * Conduct pre-operative history and physical exams, as well as obtain transplant and other surgical consents. * Provide direct patient care by evaluating patients awaiting transplant, as well as post- transplant recipients, and kidney donors. * Collect, order and interpret lab work and diagnostic studies under agreed upon protocols. * Order and obtain medication prior authorizations for kidney transplant recipients. * Develop practice protocols, standard of care, and patient education material for kidney/pancreas transplant recipients. * In conjunction with the research team, monitor and provide care to patients participating in research studies. * In partnership with Quality, Research and Education group, help develop and manage detail clinical databases to evaluate and improve patient outcomes. * Attend professional meetings and conferences, maintain professional affiliations and enhances professional development to stay current with trends in health care administration. Position Summary The Department of Obstetric and Gynecology is seeking a Clinical Coordinator / NP who will collaborate and report to the Division Chief. Duties related to the functioning and development of The Division of Gynecologic Oncology and will have frequent contact/ communication with all patients. Provide ambulatory chemotherapy administration, foley care; IVs, wound care, etc. * Initiate and maintain positive relationships with patients, families, and hospital staff. * Assist with triaging patient phone calls and processing prescription requests; signing off on all normal labs, reviewing all labs and collaborating with physicians in contacting patients with abnormal results and ensuring appropriate follow up is scheduled and handled appropriately, coordinate specimen drawing and handling of all STAT specimens, working with specialized laboratories to arrange specimen pick up and serve as the research liaison. * Assist in the development and implementation of division initiatives including education programs, QA and CQI. * The incumbent will organize tumor conference, didactic work with medical students. Inpatient management if GYN Oncology patients as needed. * Comply with all hospital, department and division policies and procedures. * Demonstrate effective organization and interpersonal skills to create a team environment and an efficient workflow. * Miscellaneous duties as assigned. Minimum Qualifications * Requires a bachelor's degree or equivalent in education and experience, plus five years of related experience. * Must have a license to practice as a Nurse Practitioner New York State * Excellent customer service.
